Uttarakhand Nursing Council Registration Name Mismatch

For nurses whose registration, diploma or degree, Aadhaar and passport do not carry one name: the state council's form, the national register and its identity card, and what a nurse going abroad has to settle first.

Quick answer

An Uttarakhand nursing council registration name mismatch is taken up with the Uttarakhand Nurses and Midwives Council, which offers an application form for a change of name after marriage. The national nurses' register is built on Aadhaar authentication, so Aadhaar, the qualifying certificate and the state registration should all read alike before you apply anywhere.

A nurse registered in her maiden name brings her state council registration, Aadhaar and passport to one name, using the council form for a name change after marriage.

An Uttarakhand nursing council registration name mismatch is very often a married name. A nurse qualifies and registers before marriage, changes her Aadhaar after it, and then finds that the national register, a Government recruitment or a hospital abroad wants every paper to agree. This page explains Uttarakhand nursing council registration name mismatch and how to close it.

We do not deal with the council. We prepare the Gazette notification, where a council, a recruiter or a foreign board asks for one. For doctors, read Uttarakhand Medical Council registration name mismatch.

What Are the State Council and the National Register?

The state nursing council and the national nurses' register compared

WhoThe Uttarakhand Nurses and Midwives CouncilThe Indian Nursing Council's registration and tracking system
What it doesRegisters nurses, midwives and auxiliary nurse midwives who qualify in the state, and renews registrationKeeps a live register of nurses across the country, fed by the state councils

Who approvesThe councilThe state council verifies and approves each entry

The two are tied together. The national system's own description says the state councils use it to keep the live register. So a name has to be right in the state council's record for it to be right in the national one.

Where Does the Registered Name Come From?

  1. The class 10 certificate, on which the nurse was admitted to the course.
  2. The nursing school's or college's record, copied from it.
  3. The diploma, degree or certificate for the nursing qualification.
  4. The state council's registration, made on that qualification.
  5. The national register, where the name is checked against Aadhaar and the state registration.

The first four are usually in one name, because each copied the one before. The break comes at the fifth, where Aadhaar enters. A nurse whose Aadhaar now carries a married surname, or was made in a home name, finds the national entry will not go through.

The national portal also tells applicants not to prefix a title to the name. Write the name alone, as on the certificate.

What Is the Council's Form for a Name Change After Marriage?

The Uttarakhand Nurses and Midwives Council's website carries, with its renewal papers, an application form for a change of name after marriage. So the council has a set route for the commonest case.

  • Download the form from the council's website. It is in Hindi.
  • We did not read the form's list of enclosures. Take the list from the form itself, or from the council's office, before you prepare anything.
  • Expect to give proof of the marriage, your identity papers in the married name, and your original registration certificate.
  • Ask the council if it wants a Gazette notification or a newspaper notice as well, or if the marriage certificate is enough.

For any other change of name, such as a new first name, or a surname taken for a reason other than marriage, ask the council what it requires. We could not confirm a separate form for that.

A marriage registered in the state gives you the cleanest proof. If your marriage was never registered, do that first. See the page on name change after marriage.

How Does the National Register Treat a Name?

The Indian Nursing Council's regulations of 2019 set up the registration and tracking system. These points are from the regulations and from the portal.

  • Nurses' data is collected on standard forms, with authentication through Aadhaar.
  • After verification and approval by the state council, and enrolment in the national register, the nurse is given a unique identity card and a register passbook.
  • Newly qualified nurses are registered by the state council on the same platform.
  • Where the state council rejects an application, it gives its reasons, and the nurse updates the details as the remarks say and submits again.
  • Movement between states, by a no objection certificate and reciprocal registration, is handled on the same system.

So the national register cannot be corrected by itself. If your entry was rejected for a name, read the state council's remark. It will point to the paper that differs: usually Aadhaar against the registration certificate.

Then decide which of the two is to move, and move it. See the next two sections.

In the first row a nurse has a real choice. She can take her registration to her married name, or keep her professional name and bring her identity papers back to it. Either works. What does not work is leaving them apart.

How Do You Put It Right?

Step 1: Lay out your papers

Class 10 certificate, nursing diploma or degree, state registration, the national identity card if you have one, Aadhaar, PAN and passport.

Step 2: Write the name from each, and mark the odd ones

Usually the certificates agree, and one or two identity papers differ.

Step 3: Decide the name you will practise under

The name on your qualification, or your married name. Think about where you may work in ten years.

Step 4: If you are keeping the qualification's name, correct the identity papers

Aadhaar first, with the certificate as proof, then PAN and the passport.

Step 5: If you are taking the married name, apply to the state council

On its form for a change of name after marriage, with the enclosures it lists and your original registration.

Step 6: Collect the amended registration and check it

The name, the qualification and the registration number.

Step 7: Update or resubmit the national register entry

Sign in, update the details and authenticate, so that the state council can approve it.

Step 8: Tell your employer, and update the rest

The hospital's personal file, the provident fund or pension account, the bank and any licence that quotes your registration.

Why Is Renewal the Time to Check?

A nurse's registration is renewed from time to time. The council's website lists what to send for a renewal, in order:

  1. The application form, downloaded from the website.
  2. The appointment letter or a salary slip, for a nurse working in the state.
  3. A copy of the seminar certificate.
  4. The Aadhaar card.
  5. The renewal fee, paid through the payment gateway.
  6. The original registration.

Look at that list as an examiner would. The appointment letter, the Aadhaar card and the original registration are put side by side. If the three do not carry one name, the renewal is where it will be noticed.

That is why the council keeps the form for a name change after marriage with its renewal papers. Send it with the renewal, or before it, and not after a query.

What Must a Nurse Going Abroad Settle First?

Nurses from the state work in the Gulf, the United Kingdom, Ireland, Australia and elsewhere. Foreign nursing boards read names very strictly.

  • The foreign board asks the Indian council to verify the registration, and compares the reply with the passport, the diploma or degree and the transcripts.
  • Every one of those has to carry the same name, or be linked by a legal paper the board accepts.
  • A marriage certificate is the usual link for a married surname. For any other change, a Gazette notification, attested or apostilled as that country requires.
  • The nursing school's transcript is issued in the name on its record. Ask the school if it can note your present name in the covering letter.
  • Verification fees abroad can be high, and may not be refunded if an application fails on a mismatch.
Fix the names before you pay a foreign board. Bring the passport, Aadhaar, the state registration and the national entry to one name first. An application opened in a mismatch can take a long time to untangle.

See name change in passport in Uttarakhand.

What About Moving Between States?

  • A nurse registered in one state who takes a job in another applies for a no objection certificate from the first council and a reciprocal registration with the second.
  • Both are processed on the national system, and both are issued in the name on the register.
  • A nurse who qualified in another state and works in Uttarakhand has her first registration there. A correction of that entry is made by that state's council.
  • Settle the name with the council that holds your first registration before you ask for the certificate. The second council registers what the first one certifies.

A nurse who trained in the state before it was formed on 9 November 2000 may hold a registration from the earlier council. Ask the present council how such registrations are treated.

What About Recruitment and Hospital Jobs?

  • Government recruitment of nursing officers and community health workers checks the registration against the class 10 certificate, the qualification and identity proof.
  • Private hospitals verify the registration number with the council before appointing.
  • A registration in a maiden name and an application in a married name has to be explained with the marriage certificate, in original.
  • The permanent residence certificate is read as well, for state posts.

See name mismatch in document verification.

How Does the Central Gazette Route Work?

Use this where the council, a recruiter or a foreign board asks for a Gazette notification, or where you have changed your name for a reason other than marriage. A nurse's papers travel, so the Gazette of India is the one to use.

Step 1: Ask who wants it, and in what form

The council may accept the marriage certificate. A foreign board may want attestation as well.

Step 2: Fix the exact name

As it should read on the registration and the passport. List the other form as on the diploma.

Step 3: Make the affidavit

State the old name, the new name, your father's or husband's name and your address. Sign before a notary.

Step 4: Publish the newspaper notice

In a daily local leading newspaper. Keep the original newspaper, and spares.

Step 5: Type the form and sign the undertaking

Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.

Step 6: Prepare the CD, photographs and certificate

The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.

Step 7: Pay the charge on BharatKosh

Commonly Rs 1,100 for an adult, with Rs 250 more for each alias. It is not refunded.

Step 8: Send the file yourself, then use the Gazette

Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar, apply to the council, and then the passport.

Would the Council Accept the Uttarakhand Gazette?

The council is a state body and may accept a state notice. Another state's council or a foreign board may not.

Step 1: Ask the council which Gazette it accepts

If it asks for one at all.

Step 2: Think about where you may work

For another state or abroad, choose the Gazette of India.

Step 3: Contact the Roorkee press

Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.

Step 4: Ask for the papers, the format and the fee, and submit

Keep photocopies of everything.

Step 5: Download the notice

Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Nurses Make?

Common mistakes with a name in a nursing registration
MistakeWhat to do

Trying the national register again and again with a different Aadhaar nameRead the remark, and align the papers first
Paying a foreign board before the names agreeFix the names first

Letting an agent fill the national entryRead every field yourself before authenticating
Sending the renewal with papers in two namesSend the name change form with it
Expecting the diploma to be reissuedThe linking paper connects it to the new name

A typical case we handle: a staff nurse in Haldwani, registered in her maiden name, had moved her Aadhaar and passport to her married name before applying to a nursing board abroad. The board held her file because the council's verification and her passport did not match. She asked us for a Gazette. We asked her to take the council's form first. She applied to the council for a change of name after marriage with her marriage certificate, received the amended registration, and the board's query was answered with that and the marriage certificate, attested. This is an illustrative example of the type of case we handle, not a named client review.

When you do not need us

For a married surname, the council's own form and your marriage certificate. Where your registration matches your diploma and Aadhaar differs, an Aadhaar centre. We are useful where a council, a recruiter or a foreign board asks for a Gazette notification, or your change of name has nothing to do with marriage.

People Also Ask About a Nursing Registration Name Mismatch

How do I change my name in my nursing council registration after marriage?

On the council's application form for a change of name after marriage, with the enclosures it lists.

Why was my nurses' register entry rejected?

Often because the name on Aadhaar differs from the state registration. Read the council's remark.

Is a gazette required for a nurse's name change?

Ask the council. For a marriage it has its own form. For other changes a Gazette is commonly asked for.

Should I change Aadhaar or my registration?

Either, but bring both to one name.

What do foreign nursing boards check?

The council's verification against the passport, the qualification and the transcripts.

Will my diploma be reissued in my married name?

No. The marriage certificate links it.

Has the Indian Nursing Council been replaced?

In law, yes. The National Nursing and Midwifery Commission Act, 2023 repeals the Indian Nursing Council Act, 1947, and the Act and its rules came into force in February and March 2024. But a report of August 2026 says the Commission had not been constituted and the Indian Nursing Council was still doing the work, and the Delhi High Court asked the Centre to decide within two months.

Who will keep the national register of nurses under the new law?

The Nursing and Midwifery Ethics and Registration Board of the Commission. It is to keep an online Indian Nurses and Midwives' Register with each professional's details and qualifications.

Must a nurse be on a register to practise?

Yes. Under the 2023 Act, a person must be enrolled in the National or a State Register to practise nursing or midwifery as a qualified professional. So the name on that register has to be right.

Will the Uttarakhand council become a State Commission?

The 2023 Act requires every state to constitute a State Nursing and Midwifery Commission where none exists under state law. Until that happens here, the Uttarakhand Nurses and Midwives Council continues to register and renew.

Can a registration decision be appealed?

Under the 2023 Act, a decision of the Ethics and Registration Board binds the State Commission unless a second appeal is filed with the National Commission. Keep every refusal on a name in writing.

Does the new law help nurses who want to work abroad?

It aims to support the global mobility and employability of Indian nurses. A foreign board will still compare the register entry with the passport, so bring both to one name first.
