Uttarakhand Medical Council Registration Name Mismatch
For doctors whose registration, degree, Aadhaar or passport do not carry one name: what the council registers you on, how its change of name service is used, and what a married doctor should think about first.
An Uttarakhand Medical Council registration name mismatch is settled by bringing your papers to the name on your medical degree, which is what the council registers you on. Where you have in fact changed your name, the council has a change of name service. Ask it for its list. Councils commonly want a Gazette notification, or a marriage certificate.

An Uttarakhand Medical Council registration name mismatch tends to appear when a doctor moves: to a postgraduate seat, to another state's council, to a job abroad. A registration that sat quietly for years is suddenly read beside a degree, an Aadhaar card and a passport. This page explains Uttarakhand Medical Council registration name mismatch and how doctors in the state deal with it.
We do not deal with the council. We prepare the Gazette notification, which medical councils commonly ask for with a change of name. For the degree itself, read name change in education documents in Uttarakhand.
What Is the Council, and What Does It Register?
A doctor of modern medicine has to be registered with a state medical council to practise. In this state that is the Uttarakhand Medical Council, at Dehradun.
The council's website lists the services it offers. Among them:
- Provisional registration, for the internship.
- Permanent registration, and its renewal.
- Registration of a postgraduate or additional qualification.
- A no objection certificate, for moving to another state's council.
- A good standing certificate, usually needed abroad.
- A duplicate registration certificate.
- Change of name or address.
So the council does provide for a change of name in its register. What it asks for with that application is in its own list, which you should take from the council before preparing anything.
Where Does the Registered Name Come From?
From the medical degree. The council's general instructions say the name of the candidate, and of the father, should be spelt in the application as it appears in the certificates. Its list of documents asks for the original degrees and for Aadhaar, in original.
- The class 10 certificate fixed the name at school.
- The entrance test and admission copied it.
- The university printed the degree in it.
- The council registered the doctor on the degree.
A registration is therefore right if the degree is right. Where the two differ, either the council's entry was typed wrongly, or the doctor applied in a name the degree does not carry. Where the registration and the degree agree and it is Aadhaar or the passport that differs, the registration is not the problem.
Where Does a Mismatch Show Up?
| Occasion | What the registration is compared with |
|---|---|
| Postgraduate counselling and admission | The degree, the internship certificate, the entrance score card and identity proof |
| Registering a postgraduate degree | The first registration and the new degree |
| Moving to another state's council | The no objection certificate, the degree and identity proof |
| The national register | Aadhaar, the degree and the state registration |
| A Government or hospital appointment | All certificates, at document verification |
| A licence to run a clinic or to hold certain drugs | The registration and the applicant's identity papers |
| Work or study abroad | The good standing certificate and the passport |
| Indemnity insurance and a medico-legal case | The name on prescriptions and the registration |
A doctor signs under a registration number every working day. The name printed on the prescription pad and the name in the register should be the same.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The registration has a typing slip, and the degree is right | Ask the council to correct its entry on the degree | No |
| The registration and degree agree, and Aadhaar differs | Correct Aadhaar to the degree's name | No |
| The degree itself has a mistake | The university corrects the degree first, then the council | Ask the university |
| The passport differs from the degree and registration | Correct the passport, or link the names for use abroad | Sometimes |
| A woman doctor has taken a married surname on her identity papers | Keep the registration and carry the marriage certificate, or use the change of name service | Ask the council |
| A doctor has changed a name by choice | The council's change of name service | Commonly yes |
| The MBBS and postgraduate degrees are in two names | Link them, and ask the council how it will register the second | Often |
Most doctors who think they need their registration changed are in the second row. Their registration is right, and one identity paper has wandered.
How Do You Use the Council's Change of Name Service?
We could not confirm the council's list of papers or its fee for this service. This is the general method, built on how the council takes its other applications.
Step 1: Lay out the degree, the registration, Aadhaar and the passport
Note the name on each, and your father's name.
Step 2: Decide if the register has to change at all
If the registration matches the degree, the other papers are the ones to move.
Step 3: Ask the council for the list and fee for change of name
From its website, by email or at its office. Ask what proof it accepts for a marriage and for any other change.
Step 4: Prepare the proof it names
Commonly a Gazette notification, or a marriage certificate, with an affidavit and identity proof in the new name.
Step 5: Apply on the council's portal
Its applications are made online, and an acknowledgement is printed.
Step 6: Submit the acknowledgement with the fee and the documents
As the council directs, with the original registration certificate where it asks for it.
Step 7: Collect the amended certificate and read it
Check the name, the father's name, the qualifications and the registration number, which stays the same.
Step 8: Carry the change to everything that quotes the registration
The national register, your employer, the prescription pad and seal, indemnity insurance and the clinic's licence.
Which Proof Should You Carry?
The council decides. Guides on medical councils in general report these being asked for.
For a correction of the council's entry
- The degree and the internship certificate
- The registration certificate with the mistake
- Aadhaar
- A written request stating the entry as it stands and as it should read
For a change of name
- The Gazette notification of the change
- The marriage certificate, for a change on marriage
- An affidavit, and the newspaper notice where asked
- Identity proof in the new name
- The original registration certificate
- Photographs and the fee
Your degree is not reissued. The amended registration and the Gazette notification link it to your new name. See gazette name change in Uttarakhand.
What Should a Married Doctor Consider?
A woman doctor's degree and registration are in her maiden name. Whether to change the registration after marriage is her decision, and it deserves a little thought.
| Point | Keep the maiden name | Change to the married name |
|---|---|---|
| The degree and registration | Stay matched, as they are | The registration changes. The degree does not |
| Later degrees and fellowships | All in one name | Earlier and later papers in two names, linked by a paper |
| Work to do | None at the council | The council, the national register, employer, insurance, licences |
| Identity papers | Keep Aadhaar, PAN and the passport in the maiden name too, or carry the marriage certificate | All moved to the married name |
| Abroad | Passport and registration match if the passport is unchanged | Match once both are changed |
Many doctors keep the professional name they qualified in. The difficulty comes from doing half: a married name on Aadhaar and the passport, and a maiden name on the registration. Choose one course and carry it through every paper.
What About the National Register?
The National Medical Commission keeps a national register of doctors, built from the state councils' registers. Enrolment in it is made online, with an identity check through Aadhaar.
- The name taken at the identity check is the name on Aadhaar. The qualification and registration details come from the degree and the state council.
- Where Aadhaar and the state registration differ, the application may be queried or held for the state council to verify.
- The state council's register comes first. A change of name is made there, and the national record follows.
- So before enrolling, make sure Aadhaar carries the name on your degree and registration.
The commission's portal and rules have been changing. Read the current instructions when you sign in. See name change in Aadhaar card in Uttarakhand.
What About a Good Standing Certificate and Work Abroad?
A regulator or employer abroad asks for a certificate of good standing from the council, and compares it with the passport and the degree.
- The certificate is issued in the name on the register.
- Foreign regulators are strict about names. A difference of a surname between the certificate and the passport is usually queried.
- Where you have changed your name, they commonly ask for the legal paper recording it. A Gazette notification serves, often with a certified translation or attestation.
- Where the difference is a married name, the marriage certificate, attested as the country requires.
- Credential verification of the degree is done separately, with the university, in the name on the degree.
Plan this months ahead. Bring the passport, the registration and the degree to one name, or have the linking paper ready and attested, before you apply. See name change in passport in Uttarakhand.
What About Dentists, Nurses, Pharmacists and Others?
Each health profession has its own council or board in the state, with its own register.
| Profession | Registered with |
|---|---|
| Dentists | The state dental council |
| Nurses and midwives | The state nurses and midwives council |
| Pharmacists | The state pharmacy council |
| Practitioners of Indian systems of medicine and homoeopathy | The state boards for those systems |
| Paramedical and allied health workers | The state paramedical council |
The pattern is the same for all of them. The register takes the name from the qualifying certificate. A slip is corrected on that certificate. A changed name is recorded on the proof the council asks for, commonly a Gazette notification or a marriage certificate. Ask the council concerned for its own list.
Nurses going abroad face the strictest checks of all, with the register, the diploma or degree and the passport compared by the foreign board.
How Does the Central Gazette Route Work?
Use this where you have changed your name, or where your papers carry two forms that have to be linked. A doctor's papers travel across states and abroad, so the Gazette of India is the one to use.
Step 1: Ask the council what it accepts
For a marriage, the certificate may be enough.
Step 2: Fix the exact name
As it should appear on the registration, the prescription pad and the passport. List the other form as on the degree.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary. If you are a Government doctor, a deed is used under your service's procedure.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper, and spares.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult, with Rs 250 more for each alias. It is not refunded.
Step 8: Send the file yourself, then apply to the council
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar, then apply to the council, and the passport after that.
Would the Council Accept the Uttarakhand Gazette?
The council is a state body and may accept a state notice. Another state's council, the national commission or a foreign regulator may not.
Step 1: Ask the council which Gazette it accepts
In writing.
Step 2: Think about where you may practise
For another state, the national register or abroad, choose the Gazette of India.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.
To weigh the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Do Doctors Make?
| Mistake | What to do |
|---|---|
| Changing Aadhaar and the passport after marriage, and nothing else | Choose one name and carry it through every paper |
| Asking the council to change a registration that matches the degree | Move the identity papers |
| Preparing a Gazette before asking the council | Take its list first |
| Expecting the degree to be reissued | The Gazette links it to the new name |
| Registering a postgraduate degree in a second name | Settle the name before the second registration |
| Using a name on the prescription pad that the register does not carry | Print the registered name and number |
| Finding the mismatch at a foreign regulator's desk | Check passport, degree and registration months ahead |
| Enrolling in the national register with a different Aadhaar name | Align Aadhaar with the degree first |
| Forgetting indemnity insurance and the clinic licence | Update everything that quotes the registration |
A typical case we handle: a doctor in Dehradun, qualified and registered in her maiden name, had moved her Aadhaar and passport to her married name. An overseas regulator then asked why her good standing certificate and her passport differed. She had two courses: put the passport back, or change the registration. She chose to change the registration. We prepared the Gazette of India notification, she applied to the council with it and her marriage certificate, and the amended registration and the Gazette together answered the regulator. This is an illustrative example of the type of case we handle, not a named client review.
What is a lawyer's job, not ours
A complaint before the council, a dispute about registration itself, and a refusal to act on complete papers.
When you do not need us
For a slip in the council's entry, the degree settles it. Where your registration matches your degree and Aadhaar differs, an Aadhaar centre. Where a marriage certificate is accepted, that. We are useful where the council, another regulator or an employer asks for a Gazette notification.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The council is at Dehradun and takes applications online. The Gazette file is prepared on WhatsApp, so a doctor posted in the hills need not take leave.

Yes. The Health Ministry said all MBBS doctors already registered on the Indian Medical Register have to register again on the National Medical Register. Your Aadhaar name should match your state registration before you do.
Is an NMR ID compulsory for a doctor?
The Government told Parliament that applying for an NMR ID is voluntary. But an NMC official later said doctors whose state council registration is not linked with Aadhaar will have to register on the national portal and apply for an NMR ID.
Who verifies a National Medical Register application?
The State Medical Council screens the application first and sends it to the NMC after verification. After the NMC's verification a unique NMR ID is issued. A name that differs from the state register stops it at the first step.
Is there a fee for registering on the National Medical Register?
An NMC official told ThePrint there is no separate fee for NMR registration. Fees for the state council's own services, including a change of name, are set by the state council.
Can the public check a doctor's registration online?
An NMC official said the unified portal is expected to become publicly searchable, so patients can verify a doctor's registration. Whatever name the register shows is the name patients and employers will see.
