Uttarakhand Pharmacy Council Registration Name Mismatch
For pharmacists and pharmacy students whose name on the diploma or degree, the registration certificate and their identity papers do not agree: where each name comes from, what to settle before registering, and why the shop's drug licence depends on it.
An Uttarakhand Pharmacy Council registration name mismatch is best cured before registering, by bringing Aadhaar to the name on the pharmacy qualification and the class 10 certificate. After registration, the council corrects its own slip on application. A change of name on the register is asked for separately, with a marriage certificate or a Gazette notification.

A pharmacist's registration is what lets a medical store open its shutters, so its name is read by more offices than most professional papers. An Uttarakhand Pharmacy Council registration name mismatch can hold up a job, a renewal or a shop's licence. This page explains Uttarakhand Pharmacy Council registration name mismatch for pharmacists in the state.
We prepare the Gazette notification for a change of name. We could not find the state council's own procedure for a correction or a change of name, so this page describes what the Act says and what other councils ask for, and tells you to confirm with the council. For other professions, read medical council and nursing council registration name mismatch.
Which Records Does a Pharmacist's Name Sit In?
| Record | Kept by | What it is for |
|---|---|---|
| The diploma or degree in pharmacy, and its marksheets | The board or university | The qualification to register |
| The state register of pharmacists | The state pharmacy council | The entry that makes a person a registered pharmacist |
| The registration certificate and renewals | The state pharmacy council | Proof of the entry |
| The drug licence of the shop where the pharmacist works | The state's drug control office | Names the registered pharmacist in charge |
| The national registers | The central pharmacy council and the national health registry | A countrywide record of pharmacists |
| Service records | A hospital, a company or the Government as employer | Appointment and pay |
The register is the root. The certificate, the renewals and the national records copy it, and the drug licence quotes it.
What Does the Act Say About the Register?
From the text of the Pharmacy Act, 1948, as the state council's own site reproduces it.
- The state council maintains a register of pharmacists for the state.
- The register includes the full name and residential address of the registered person, the date of first admission to the register, the qualifications for registration, and the professional address with the employer's name.
- A person who wants to be entered applies to the state council with the prescribed fee. On entry, the Registrar issues a certificate of registration.
- A renewal fee may be made payable to keep a name on the register.
- A name removed from the register can be restored by the council's order, on the prescribed fee.
The Act asks for the full name. It does not set out, in the parts we read, how a name on the register is corrected or changed. That is left to the council's rules and practice, which is why the council's office is the place to ask.
Where Does the Name Come From?
- The class 10 certificate fixes the name and the date of birth.
- The pharmacy college admits the student in that name, and the diploma or degree carries it.
- The registration application is filled by the candidate, with the qualification, marksheets, identity proof and address proof. Guides to the state council's registration list these.
- The council enters the register from that application and issues the certificate.
- Every later record copies the certificate.
The candidate types the name once, in the registration form. Type it from the pharmacy qualification, with Aadhaar already brought into line, and the chain holds.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| Not yet registered, and Aadhaar differs from the qualification | Correct Aadhaar to the qualification, then register | No |
| The diploma or degree misspells the name against class 10 | A correction by the board or university, before registering | No |
| The certificate has a typing slip against the application | An application to the council to correct its slip | No |
| A woman pharmacist wants her married name on the register | An application for a change of name, with the marriage certificate and Aadhaar in the new name | Ask the council |
| A pharmacist has changed a name by choice | An application for a change of name, with the Gazette notification | Yes |
| The drug licence names the pharmacist differently from the register | An amendment of the licence, on the registration certificate | No |
| The name was removed for not renewing | Restoration, on the fee. Not a name problem | No |
Most rows need a form and patience. Only one needs a Gazette.
How Do You Put the Papers in Order Before Registering?
Step 1: Lay out class 10, class 12 and the pharmacy qualification
With every marksheet. Write the name and the father's name from each.
Step 2: Mark any paper that differs from class 10
Have it corrected by the board or university that issued it.
Step 3: Correct Aadhaar to the qualification name
The council takes identity proof, and the national registers read Aadhaar.
Step 4: If your name has changed, complete the Gazette
Before the registration form, and ask the council how it will enter you.
Step 5: Read the council's current registration instructions
For its list of papers and its fee.
Step 6: Fill the form in the qualification name
In full, with the father's name as on the qualification.
Step 7: Check the certificate the day you receive it
A slip pointed out at once is the council's to correct.
Step 8: Give your employer and the drug licence the name as on the certificate
Letter by letter.
How Is a Name Corrected or Changed After Registration?
We could not find this state council's procedure. Councils elsewhere treat these as two services, and one lists "change of name" and "correction of name" separately on its site.
A correction
- The register or the certificate does not match what you applied in
- You are asking the council to put right its own entry
- The proof is your qualification, your class 10 certificate and identity proof
- No Gazette is involved
A change of name
- The register is right for the name you had, and you now have another
- For marriage, another state's council asks for the marriage certificate issued by the registrar of marriages and Aadhaar in the new name
- For a change by choice, expect to be asked for a Gazette notification, with an affidavit
- The registration number stays, and a fresh certificate is issued
Ask the council's office for its form, fee and list before you prepare anything. Where it asks for a Gazette notification, that is the paper we prepare. Where a marriage certificate is enough, you do not need us.
What About Renewal?
- Under the Act, a renewal fee may be required to keep a name on the register. The period and the fee are the state council's to set.
- A renewal is made in the name on the register. It is a natural moment to ask for a correction or a change, with the papers.
- A registration that has lapsed is restored on the council's order and the fee. Do that first, and ask for the name to be put right alongside.
- Guides list identity proof with a renewal. Where it is now in a different name from the register, expect a question.
- Keep every renewal slip. They show an unbroken registration.
Why Does the Shop's Drug Licence Depend on It?
- A retail medical store's drug licence is granted on the footing that a registered pharmacist is in charge. The licence, or the papers behind it, name that pharmacist with the registration number.
- The drug control office reads the pharmacist's name on the licence application against the registration certificate.
- A pharmacist who is also the owner appears twice: as proprietor, on PAN and the business papers, and as pharmacist, on the registration. All should be one name.
- When the register is corrected or the name changed, the licence is amended by the drug control office on the fresh certificate.
- A pharmacist who leaves a shop should see that the licence is updated. A name left on a licence is a liability.
We could not confirm the state drug control office's form for this. Ask it directly. Read name change in trade licence and shop establishment and name change in Udyam and GST registration.
What About the National Registers?
- The central pharmacy council keeps a central register, built from the state registers.
- Guides report that pharmacist registration is being linked to a national digital registry of health professionals, through the central council's online system.
- They stress that a national registry profile is not the same as state registration. The state register is the legal registration, and one does not renew the other.
- The national profile is built on Aadhaar. Where Aadhaar and the state register differ, the linking can fail.
- So the order is the same as everywhere: Aadhaar and the state register in one name first, then the national profile.
What About Moving in From Another State?
- A pharmacist registered in one state who moves to another applies to the new state's council, with the earlier registration and the papers it lists.
- The new council enters the name from those papers. A slip in the old state's certificate is carried over unless it is corrected first.
- A correction of another state's register is asked for in that state.
- A woman who married and moved in the same year often has a qualification in one name, an old registration in the same, and an Aadhaar in another. Decide which name the new registration is to carry, and take the marriage certificate.
How Does the Central Gazette Route Work?
Use this where a pharmacist or a student has changed a name by choice. It does nothing for a slip on a certificate, which the issuing body corrects.
Step 1: Ask the council what it requires for a change of name
And which Gazette it accepts.
Step 2: Fix the exact new name
The spelling that will go on the register, Aadhaar and the drug licence.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then apply to the council
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar and PAN, then apply to the council, and then have the drug licence amended.
A pharmacist in Government service follows the service procedure for a change of name as well. See name change for government employees.
Would the Council Accept the Uttarakhand Gazette?
A state council may. A pharmacist also needs the national registers, Aadhaar and another state's council, on a later move, to read the same paper. That points to the Gazette of India.
Step 1: Ask the council which Gazette it accepts
In writing.
Step 2: Think about Aadhaar and work in other states
The Gazette of India serves everywhere.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Pharmacists Make?
| Mistake | What to do |
|---|---|
| Registering on a diploma that misspells the name | Have the board correct it first |
| Registering in the Aadhaar name where it differs from the qualification | Bring Aadhaar to the qualification |
| Asking for a "correction" when the name has in fact changed | Apply for a change of name, with the right proof |
| Getting a Gazette for a typing slip on the certificate | The council corrects its own slip |
| Changing the register and forgetting the drug licence | Have the licence amended on the fresh certificate |
| Letting the registration lapse | Renew in time. Restoration costs more |
| Treating the national profile as the registration | The state register is the legal one |
| Carrying an old state's slip into a new state | Correct it at the old council first |
| Not reading the certificate on receipt | Check it that day |
A typical case we handle: a pharmacist in Haridwar was registered as "Pooja Kumari", the name on her diploma. After marriage her Aadhaar read "Pooja Saini", and the drug control office questioned the licence application for the store she was joining. She asked us for a Gazette. We told her to ask the council first, since a marriage certificate is commonly enough for a married name. She applied for a change of name with the certificate and her Aadhaar, and the licence was then issued on her fresh registration certificate. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us

What the register contains, and the provisions on renewal and restoration, are from the text of the Pharmacy Act, 1948 as the state council's site reproduces it. We could not find the state council's own procedure, form or fee for a correction or a change of name. The proof described for marriage is another state council's. The notes on the drug licence, the national registers and a move between states are general guidance. The council's office and the state drug control office are the authority.

The register is written from the pharmacy qualification, in the full name. Bring Aadhaar to that name, and correct the qualification itself if it is wrong, before registering. A slip on the certificate is corrected by the council. A change of name is asked for separately, usually on a marriage certificate for a married name and a Gazette notification for a change by choice. Have the shop's drug licence amended to match.
