Name Change in ABHA Health ID in Uttarakhand
When the name on your ABHA health account is wrong or out of date: where the name comes from, how it is brought in line with Aadhaar, and how ABHA differs from the Ayushman card.
A name change in ABHA health ID records follows your Aadhaar. An ABHA number made with Aadhaar takes its name from Aadhaar, and verifying again with Aadhaar brings the profile in line. So correct Aadhaar first, then refresh ABHA. No Gazette or affidavit is asked for. The Ayushman card is a different record, corrected separately.

A name change in ABHA health ID records is one of the simplest on this site, because the health account does not keep a name of its own choosing. It copies your identity paper. This page explains name change in ABHA health ID records for people in Uttarakhand: where the name comes from, how to refresh it, and what to do about hospital records and the Ayushman card.
We do not handle health accounts, and none of this needs a Gazette. Our work is the Gazette for names. The link is Aadhaar, so if your name is changing, start with name change in Aadhaar card in Uttarakhand.
What Is ABHA?
ABHA stands for Ayushman Bharat Health Account. It is part of the Ayushman Bharat Digital Mission, run by the National Health Authority. It gives a person a number under which health records from different hospitals and laboratories can be linked and shared with consent.
- The ABHA number is a fourteen-digit number that identifies you in the system.
- The ABHA address is an easy name, a little like an email address, used to share records.
- An ABHA number is created by verifying yourself with Aadhaar, or with a driving licence.
- Creating one is voluntary. It is a way to keep records. It is not health insurance, and it gives no free treatment by itself.
Because it is made from an identity check, the name on it is the name on the paper you verified with.
How Is ABHA Different From the Ayushman Card?
| Point | ABHA health account | Ayushman card |
|---|---|---|
| What it is | A number for linking your health records | A card showing you are covered under a health cover scheme |
| Who can have it | Anyone who chooses to make one | Those entitled under the scheme |
| What it gives | Records in one place, shared with your consent | Cashless treatment at empanelled hospitals, up to the scheme's limit |
| Where the name comes from | Aadhaar or a driving licence | The scheme's list of beneficiaries, matched with Aadhaar |
| Who corrects it | You, on the ABHA portal or app | The scheme's own system, through its help desks |
People say "Ayushman card" for both. Before you start, look at the paper in your hand. If it shows a fourteen-digit ABHA number, this page's first half is for you. If it is a card for treatment under a health cover scheme, go to the section on the Ayushman card below.
Where Does the Name on ABHA Come From?
| How the account was made | Where the name came from |
|---|---|
| With Aadhaar | From Aadhaar, as it stood on that day |
| With a driving licence | From the licence, checked at a participating facility |
| An ABHA address made with only a mobile number | From what was typed at the time, by you or by a hospital desk |
The mission's own technical forum explains two points that matter here. First, the ABHA number and the ABHA address are separate things, and an address made with a mobile number may carry whatever name the person gave. Second, once an identity check is done with Aadhaar, the profile is overwritten with the Aadhaar details.
So there are two ways an ABHA name goes wrong. Either Aadhaar itself was wrong, or has changed since. Or the account was made without Aadhaar, from a name someone typed.
Which Case Is Yours?
| The situation | Example | Route |
|---|---|---|
| ABHA carries the same mistake as Aadhaar | Both say Bist for Bisht | Correct Aadhaar, then verify ABHA again |
| Aadhaar has been updated and ABHA still shows the old name | A surname after marriage | Verify ABHA again with Aadhaar |
| The account was made with a typed name | A hospital desk typed a short form | Verify with Aadhaar, which overwrites the profile |
| The account was made with a driving licence that has a different spelling | The licence and Aadhaar differ | Correct the odd paper, then verify with Aadhaar |
| Only the ABHA address looks wrong | An address made from a pet name | Make a new address under the same number, as the app allows |
| A hospital's own record has a different name | The hospital file does not match ABHA | Ask the hospital to correct its registration |
Every row but the last comes back to Aadhaar. Get that right and the rest follows.
How Do You Bring ABHA in Line With Your Name?
The screens of the portal and the app change from time to time. This is the general method.
Step 1: Check the name on your Aadhaar
If Aadhaar is wrong or out of date, correct it first at an Aadhaar centre.
Step 2: Wait for the updated Aadhaar
Download it and confirm the name, in both scripts.
Step 3: Sign in to your ABHA account
On the official ABHA portal or the official app, with your ABHA number or address.
Step 4: Look at the profile
Note the name, date of birth and gender it shows now.
Step 5: Use the option to verify again with Aadhaar
The account offers a way to redo the identity check. Complete it with the code sent to the mobile number linked with your Aadhaar.
Step 6: Check the profile again
It should now carry the name as on Aadhaar.
Step 7: Download the ABHA card afresh
And give the hospitals you use the corrected card at your next visit.
If you cannot find the option, or the profile does not change, use the helpline or the grievance link on the official portal. Do not make a second ABHA number to get round it.
What About the ABHA Address?
The ABHA address is the short name you share with a hospital, in place of the long number. You choose it, and it need not be your legal name.
- A person may have more than one address under one ABHA number, as the system allows.
- If your address was made from an old name or a nickname, you can make a new one and use that from now on.
- An address is only a label. What counts for identity is the profile behind the ABHA number, which follows Aadhaar.
- An address made with only a mobile number, and not linked to a verified ABHA number, may carry a name nobody checked. Link it to your verified number.
What If a Hospital's Record Has a Different Name?
A hospital keeps its own registration of each patient, made at the counter, often in a hurry. Linking that file to your ABHA works by matching the two sets of details. If the hospital typed a different spelling, the link can fail.
- Correcting ABHA does not correct the hospital's file. Each hospital changes its own registration.
- Ask the registration or medical records desk to correct the name, with your Aadhaar.
- For reports and discharge papers already issued, ask the hospital for a corrected copy, or for a letter that the papers relate to you.
- At a new hospital, hand over your Aadhaar or ABHA card at registration, and read the slip before you leave the counter.
Old medical papers in an earlier name are still yours. After a marriage or a change of name, keep the marriage certificate or the Gazette with them, to show the link.
What About a Name Mismatch on the Ayushman Card?
The Ayushman card is issued under a health cover scheme, to people on the scheme's list. In Uttarakhand, the national scheme runs alongside the state's own schemes, under the State Health Authority.
- The card is made by matching a name on the scheme's list with the person's Aadhaar. A difference between the two is the usual reason a card is not made, or carries a wrong name.
- A correction is made within the scheme's own system. It is not made on the ABHA portal.
- The help desk at an empanelled hospital, or a common service centre, can raise the request. So can the scheme's helpline.
- Carry Aadhaar, and the family paper the scheme uses, such as the ration card, so that the right person in the family can be picked out.
We could not confirm the current procedure of the State Health Authority for a name correction, so ask its help desk. Where the difference is a spelling between the ration card and Aadhaar, correcting the odd paper at its own office is the lasting answer. See name spelling correction in Uttarakhand.
What About a Child's Health Account?
- A child's ABHA follows the child's own Aadhaar, where one has been made.
- Where a child's Aadhaar still carries a temporary or wrong name, correct that first. See Baal Aadhaar child name correction.
- A newborn's hospital file is usually opened in the mother's name. Ask the hospital how the baby's own record will be made once the baby is named.
- Keep the child's vaccination card and the birth certificate in the same name.
What About Health Insurance and Claims?
A private health insurance policy is a separate record again, held by the insurer. At the time of a claim it compares three names: the policy, the hospital's papers and your identity proof.
- After a change of name, ask the insurer for an endorsement on the policy, with your updated identity proof and the linking paper.
- At admission, give the hospital the same identity proof the insurer has.
- If the discharge summary carries a wrong spelling, ask the hospital to correct it before you file the claim.
- An employer's group policy is corrected through the employer.
None of these asks for a Gazette for a spelling slip. An insurer may ask for one where you have taken a new name.
How Does the Central Gazette Route Come In?
Only at one remove. ABHA does not ask for a Gazette. If you are taking a new name, Aadhaar asks for one for a major change, and ABHA then follows Aadhaar.
Step 1: Confirm that your change is one Aadhaar treats as major
A spelling correction or a surname after marriage usually is not.
Step 2: Fix the exact new name
The same spelling you will use on every paper.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your Aadhaar address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and an undertaking showing the old and the new name.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update Aadhaar and ABHA
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar, and then verify ABHA again.
Does the Uttarakhand Gazette Matter Here?
Not to ABHA. The question is only which Gazette Aadhaar and your other records accept.
Step 1: Ask the Aadhaar centre which Gazette it accepts
ABHA will follow Aadhaar.
Step 2: Think about PAN, the passport and your insurer
If one Gazette has to serve them all, the Gazette of India is the safer choice.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do People Make?
| Mistake | What to do |
|---|---|
| Trying to correct ABHA while Aadhaar is still wrong | Correct Aadhaar first |
| Making a second ABHA number to get the name right | Verify the one you have again with Aadhaar |
| Mixing up ABHA and the Ayushman card | Check which paper you hold. They are corrected in different places |
| Paying for an affidavit or a Gazette for ABHA | Neither is asked for |
| Paying a shop to "update" the account | Do it yourself on the official portal or app, free |
| Letting a hospital desk type your name from hearing | Hand over your Aadhaar or ABHA card |
| Expecting ABHA to correct the hospital's file | Ask each hospital to correct its own registration |
| Finding an Ayushman card mismatch at admission | Check every family member's card now |
| Using an unofficial website that asks for your Aadhaar | Use only the official portal |
A typical case we handle: a woman in Rishikesh updated her surname on Aadhaar after marriage, and a laboratory then could not link her reports because her ABHA still showed her maiden name. She asked us if she needed a Gazette for the health card. She did not. She signed in to her ABHA account, verified again with Aadhaar, and the profile took her married name. This is an illustrative example of the type of case we handle, not a named client review.

That an identity check with Aadhaar overwrites the ABHA profile, and that the ABHA number and address are separate, is from the digital health mission's own technical forum. The steps on the portal and app, and the route for the Ayushman card, are general guidance, since screens and procedures change. The official portal and the scheme's help desk are the authority.
