Vaccination Certificate Name Mismatch in Uttarakhand

For people whose vaccination certificate carries a name that differs from their passport or Aadhaar, and parents whose child's immunisation record has no proper name: which papers these are, who corrects each, what travel abroad asks for, and why a correction should be made with care.

Quick answer

A vaccination certificate name mismatch is corrected where the record is kept: on the national portal for a Covid certificate, at the health centre for a child's record, and at the issuing centre for a travel certificate. For travel abroad, the name should read as the passport does. No Gazette is needed.

A vaccination certificate is read against the passport abroad, so the name is corrected on the portal or at the centre to match it.

A vaccination certificate was once a slip nobody looked at twice. It is now shown to airlines, foreign borders and employers abroad, and scanned by a machine that compares it with a passport. A vaccination certificate name mismatch is a name typed at a busy counter that does not survive that scan. This page explains vaccination certificate name mismatch for people in Uttarakhand.

We do not correct vaccination records, and this subject needs no Gazette in the ordinary case. What the national portal allowed by way of correction is from the health ministry's announcements of 2021, as reported. The portal may have changed since. For hospital papers, read hospital discharge summary and medical records name correction.

What Are the Three Kinds of Vaccination Paper?

Kinds of vaccination record and who keeps and corrects each
PaperKept byCorrected by
A Covid vaccination certificateThe national vaccination portalThe holder, through the portal's own correction
A child's immunisation card or recordThe health centre and its health workerThe health centre
A travel vaccination certificateThe centre authorised to give that vaccineThat centre

They are alike in one thing. Each was written at the moment of the injection, by someone with a queue behind you, from what you said or the card you held out. None was made as an identity paper, and each is now read as if it were.

What Was Announced?

From the health ministry's announcements about the national vaccination portal in 2021, as news agencies and guides reported them. Check the portal for what it allows today.

What was announced about correcting a Covid vaccination certificate on the national portal
PointWhat was announced or reported
The featureA raise an issue feature on the portal, so that a person can correct inadvertent errors in the certificate
What could be correctedThe name, the year of birth, the gender and the photo identity number
How muchReports said only two of those fields could be chosen, and that the certificate could be edited once
HowBy logging in with the mobile number registered at vaccination
PassportA passport number could be added to the certificate for travel, and the name had to be the same as on the passport
Where it is foundOn the portal and in the Government's digital locker, among other places

The third row is the one to take to heart. Where a correction can be used once, it has to be right. A person who corrects a spelling to a second wrong spelling may have nothing left to try.

Where Does the Name Come From?

Where the name on each vaccination record comes from and what it is read against
RecordName came fromRead against
A Covid certificateThe identity paper shown, or what was typed at registrationThe passport abroad. Aadhaar at home
A child's immunisation recordThe mother's word, often before the child was namedThe birth certificate
A travel vaccination certificateWhat the traveller wrote on the centre's formThe passport, at a foreign border
A health account's recordAadhaarAadhaar

Many people registered for a Covid vaccine on a relative's phone, four to a number, typed by whoever was quickest. That is why so many certificates carry a home name or half a name.

What Kinds of Mismatch Are There?

A slip, or a short form

The wrong identity paper behind it

  • Registered on Aadhaar, and now needed against a passport
  • The two papers read differently
  • The papers are brought to one name. A passport number is added

No proper name at all

  • A child recorded as "Baby of" the mother
  • Or an adult under a name no paper carries
  • The centre adds the name from the birth certificate or identity paper

The second is the one that stops a trip abroad. The certificate is not wrong against the card it was made on. It is wrong against the passport, which was made years apart from that card.

How Do You Put It Right?

This is the general method. Do it well before you need the certificate.

Step 1: Decide which paper the certificate will be read against

A passport for travel abroad. Aadhaar or a birth certificate for most uses at home.

Step 2: Read the name on the certificate against that paper

Letter by letter, with the year of birth and the identity number printed on it.

Step 3: See which kind of certificate it is

A Covid certificate from the national portal, a child's immunisation record, or a travel vaccination certificate.

Step 4: For a Covid certificate, use the portal's own correction

Log in with the mobile number used at vaccination, and raise a correction. Choose the fields with care.

Step 5: For a child's record, go to the health centre that keeps it

With the birth certificate and the mother's identity paper.

Step 6: For a travel certificate, go back to the centre that issued it

With the passport, before you fly.

Step 7: Download or collect the corrected certificate, and read it again

A correction that is itself wrong may not be open to a second try.

Step 8: Keep it with the paper it matches

In the passport's cover, or in the child's file.

What About the Covid Certificate?

  • The certificate is a record on the national portal. The printout and the code on it are drawn from that record.
  • The ministry announced that a person can correct inadvertent errors through the portal, with the mobile number used at vaccination.
  • Reports said the correction covered the name, year of birth, gender and identity number, two of them at most, and once.
  • So write the corrected name on paper first, copied from the passport or Aadhaar, and type from that.
  • After submitting, download the certificate afresh and read it. The old one is replaced.
  • We could not confirm what the portal allows today, or what a person does where a correction has already been used. Ask a Government vaccination centre.

The certificate is also kept in the Government's digital locker. Read APAAR ID and DigiLocker and name change in ABHA health ID.

What About Travel Abroad?

  • An airline or a border officer reads a vaccination certificate against the passport in the same hand.
  • As reported, the portal let a passport number be added to the Covid certificate, with the name the same as on the passport.
  • Some countries ask for proof of a particular vaccine before entry. That certificate is issued by an authorised centre, on a set form.
  • Carry the passport when you are vaccinated for travel, and have the name copied from its first page.
  • A passport has two name boxes. The certificate should carry both, in an order that plainly matches.
  • Check the country's present rule before you travel. Health rules at borders change quickly.

Read visa application name mismatch, name change in passport and flight ticket name mismatch.

What About a Child's Immunisation Record?

  • A child's first vaccines are given within days of birth, often before a name is chosen.
  • The card and the health worker's register then say "Baby of" and the mother's name.
  • Later doses are entered on the same card, and nobody goes back to add the child's name.
  • A school, a hostel or a foreign visa may ask for a vaccination record in the child's own name.
  • Ask the health centre to add or correct the name, on the birth certificate.
  • Keep the card safe. It is the only record of what the child was given, and when.

The programme's records are moving onto a digital system. Ask the health worker how your child is entered in it. Read name change in birth certificate and name change for a newborn.

What About the Mother's Name on It?

  • A mother and child card names the mother first, and the child under her.
  • It is opened during pregnancy, by a health worker, in the name the mother gives.
  • A young mother in her husband's village may give a married name her papers do not yet carry.
  • Benefits for a mother are tied to visits and doses recorded on that card. A name that differs from her Aadhaar and bank account can hold them.
  • Ask the health worker to correct the card and her register to the mother's Aadhaar name.
  • Do it at the first visit after you notice. The card is used for years.

Read PM Matru Vandana Yojana name mismatch and name change after marriage.

What About Old Records, and a New Name?

  • A vaccination record says that a named person was given a dose on a day. It was true then.
  • A person who later marries or changes a name does not have to change old vaccination records.
  • Where they are asked for, the linking paper is carried: a marriage certificate, or a Gazette notice.
  • A new dose is recorded in the name on the identity paper shown that day.
  • So after a change of name, a person may hold records in two names. That is in order, with the link.
  • Where a certificate can still be corrected on a portal, bringing it to the present passport saves explaining.

Read changing your name a second time, on carrying papers in more than one name.

Where Does a Gazette Come In?

Only where the holder has changed a name by choice. The passport then carries the new name, and the notice links older records to it. It has no part in a slip made at a vaccination counter.

Step 1: Confirm that it is a change of name, and not a slip at a vaccination counter

A slip is corrected on the portal or at the centre.

Step 2: Do not start a Gazette for a certificate alone

A vaccination certificate follows your identity paper. Change that, and the certificate follows.

Step 3: Fix the exact new name

The spelling that will go on Aadhaar and the passport.

Step 4: Make the affidavit and publish the newspaper notice

Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.

Step 5: Type the form and sign the undertaking

Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.

Step 6: Prepare the CD, photographs and certificate

The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.

Step 7: Pay the charge on BharatKosh

Commonly Rs 1,100 for an adult. It is not refunded.

Step 8: Send the file yourself, then update the passport and carry the notice

Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Old vaccination records stay in the old name. The notice links them.

Which Gazette Should You Use?

A vaccination certificate is read against the passport. The Gazette of India is the one a passport office recognises.

Step 1: Remember who reads a vaccination certificate

Mostly an airline, a foreign border or an employer abroad.

Step 2: Remember what they read it against

The passport.

Step 3: Prefer the Gazette of India

It is the one a passport office recognises.

Step 4: Contact the Roorkee press only if a state notice suits another purpose

Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.

Step 5: Download the notice

Search egazette.gov.in and save the PDF.

Which Mistakes Do People Make?

Common mistakes with names on a vaccination certificate
MistakeWhat to do
Checking the certificate at the airportCheck it weeks before
Using the one correction in a hurryWrite the name out first, from the passport
Correcting to Aadhaar when the passport differsSettle the two papers first
Changing the phone number used at vaccinationNote it. The record is reached by it
Leaving a child's card as "Baby of"Have the name added
Giving a short name for a travel vaccineCarry the passport to the centre
Editing a printoutNever. The code shows the record's name
Paying someone for a "corrected" certificateThat is forgery, found at the first scan
Asking for a Gazette for a certificateThe portal or the centre corrects it

A typical case we handle: a cook from Tehri had a job waiting on a ship. His employer's agent asked for his vaccination certificate. It read "Bittu", the name his cousin had typed on a shared phone. His passport read "Birendra Singh Rawat". Someone told him to change his name by Gazette. He had changed no name, and it would have taken weeks. He logged in on his cousin's number, corrected the name to match his passport, and added the passport number. He read the fresh certificate twice before sending it. This is an illustrative example of the type of case we handle, not a named client review.

Yes. It generates a uniform QR-based, digitally verifiable e-vaccination certificate, like the Covid certificate. The name on it is the name typed at registration.

Who can be registered on U-WIN?

All pregnant women, and children up to 6 years of age, can be registered for vaccination on the platform. A child first entered as Baby of the mother should have the name added.

Yes. Parents can download the digital certificate using the registered mobile number, which also lets the child be vaccinated anywhere in the country.

Up to 10 people can be registered for vaccination using the same mobile number. That is convenient, and it is also how names get typed loosely by whoever holds the phone.

It may be questioned. Bring the certificate to the passport's name before you travel, while a correction is still open to you.
I cannot log in, since the mobile number used then is gone. What now?+
Ask at a Government vaccination centre or the portal's helpline how a record is reached without that number. We could not confirm the present steps.
My child's immunisation card says only Baby of and the mother's name. Is that a problem?+
Not for the vaccines. For school or travel, the child's own name should be on it. Ask the health centre to add it from the birth certificate.
The mother's name on the child's card differs from her Aadhaar. Does it matter?+
It can, where a benefit for the mother is tied to the child's vaccinations. Ask the health worker to correct it on her Aadhaar.
I need a vaccine certificate for travel to a country that demands one. Whose name goes on it?+
Yours, exactly as on your passport. Carry the passport when you are vaccinated, and read the certificate before you leave the centre.
My name changed after marriage, and my old vaccination records are in my maiden name. Must they be changed?+
No. They are true records of their date. Carry the marriage certificate with them where they are asked for.
Is the vaccination record linked to my health account number?+
It can be. A health account has its own name, taken from Aadhaar. We have a page on correcting that.
Is a printed certificate enough, or must it be the digital one?+
A reader usually scans the code on it. The code holds the name the record has, so correcting the record matters more than the print.
Someone offers to make a corrected certificate for a fee. Should I use him?+
No. A certificate that does not match the record behind its code is a forgery, and is found at the first scan.
Do you correct vaccination certificates?+
No. The portal and the health centres do. We prepare a Gazette where a person's own change of name needs one.

Who Wrote and Reviewed This Page?

This page is written by the Documentation Desk Team of UttarakhandNameChange.com, the team that prepares Gazette files for people across Uttarakhand.

It is reviewed by Advocate Vipin Chauhan.

That the national portal has a raise an issue feature for correcting inadvertent errors, the fields it covered, that two fields at most could be chosen and the certificate edited once, and that a passport number could be added with the name matching the passport, are from the health ministry's announcements of 2021 as news agencies and guides reported them. We did not test the portal, and it may have changed. We could not confirm the present state of the child immunisation programme's digital records, or what any country now asks of travellers. How a child's card is made, the mother's name on it and how old records are treated are general knowledge. The portal, a Government vaccination centre and your health worker are the authority.
