Janani Suraksha Yojana Name Mismatch in Uttarakhand
For expectant and new mothers whose mother and child card, hospital record, Aadhaar and bank account do not carry one name, and whose cash assistance after delivery is held: which papers are read, who corrects each, and why it should be settled before the delivery, not after.
A Janani Suraksha Yojana name mismatch is settled during the pregnancy: the mother's own bank account in her Aadhaar name, and the mother and child card written in the same name by the health worker. Give those papers at admission, so the hospital's record follows them. No Gazette is needed.

A woman who delivers her baby in a Government hospital is due a sum of money for it, paid into her bank account. It is not large, and it matters in the weeks after a birth. A Janani Suraksha Yojana name mismatch keeps it from coming, and is found when the mother has least time to chase it. This page explains Janani Suraksha Yojana name mismatch for mothers in Uttarakhand.
We do not handle this scheme, and it needs no Gazette. What the scheme is, that it covers Uttarakhand, and how it is paid are from the health ministry's and the National Health Mission's own descriptions. We give no amount. For the maternity benefit paid in instalments, read PM Matru Vandana Yojana name mismatch.
What Is the Scheme, in Brief?
- What it is. Cash assistance to a mother who delivers in a Government or accredited health facility.
- Why. To bring births into health facilities, where mother and child are safer.
- Who runs it. The health department, under the National Health Mission.
- Who helps. The village health worker, who registers the pregnancy and goes with the mother.
- How it is paid. Into the mother's bank account.
- What the mother holds. A mother and child card, made when the pregnancy is registered.
The sum differs between villages and towns and is revised from time to time. The health worker knows the present figure.
What Does the Ministry Say?
From the health ministry's annual report chapter on the scheme and the National Health Mission's page for it.
| Point | What the ministry says |
|---|---|
| The scheme | A safe motherhood scheme under the National Health Mission, launched on 12 April 2005, to reduce the deaths of mothers and newborns by promoting delivery in health facilities |
| What it joins | Cash assistance with care at delivery and after it |
| The health worker | The accredited social health activist is named as the link between the Government and pregnant women |
| Uttarakhand | Listed among the states given special dispensation for low rates of delivery in health facilities |
| Payment | Made through direct benefit transfer, into bank accounts |
| Home delivery | A smaller sum is described for poor pregnant women who deliver at home |
Guides add the papers a mother carries: the mother and child card, an identity paper such as Aadhaar, and her bank passbook. Three papers, one woman, and usually three different hands that wrote her name.
Where Is the Mother's Name Written?
| Record | Written by | Who corrects it |
|---|---|---|
| The mother and child card | The health worker, at registration | The health worker, or the health centre |
| The health worker's register and the tracking record | The health worker and the nurse | The same |
| The hospital's delivery register and discharge paper | The hospital, at admission | The hospital |
| The bank account | The bank | The branch, on Aadhaar |
| Aadhaar | The identity authority | An Aadhaar centre |
The payment is sent to the bank account, in the name the health system holds. If that name and the account's name are not the same, the bank does not credit it.
What Kinds of Mismatch Are There?
A married name on the card
- The health worker in the husband's village writes the name the village uses
- Aadhaar and the account are still in the maiden name
- The card is written in the Aadhaar name
A loose name at the hospital
- A relative gives the name at admission, in a hurry
- The mother is in labour and reads nothing
- The hospital corrects its register, before discharge
No account of her own
- The account given is the husband's, or a joint one in his name first
- Or her own account is old, dormant or not linked to Aadhaar
- An account in her own name, opened during the pregnancy
The first is the commonest among young mothers. She has been married a year or two, and her papers have not caught up with what the village calls her. The scheme pays the woman on the papers.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The card has a married name, Aadhaar a maiden one | The health worker writes the card in the Aadhaar name | No |
| The card has a home name | The same | No |
| The hospital's record has the name wrong | The hospital, on Aadhaar, before discharge | No |
| The bank account is in the husband's name | An account in the mother's own name | No |
| The account's name differs from Aadhaar | The branch, on Aadhaar | No |
| The account is not linked to Aadhaar, or is dormant | The branch | No |
| Aadhaar itself is wrong | Use it as it is for now. Correct it after the delivery | No |
| The mother changed a name by choice earlier, and the papers never followed | The earlier Gazette, or a fresh one later | Rarely |
No row here says a plain yes. A pregnancy lasts nine months, and none of that time should be spent on a Gazette for this.
How Do You Put It Right?
This is the general method. Do the first four steps in the early months.
Step 1: Read the mother's name on her Aadhaar, letter by letter
That is the name the payment system will look for.
Step 2: Open or check a bank account in her own name
The same name as Aadhaar, and linked to it. Not her husband's account, or a joint one led by him.
Step 3: Read the mother and child card against both
The name the health worker wrote when the pregnancy was registered.
Step 4: Ask the health worker to correct the card and her register
During pregnancy, at any visit. It is a minute's work then.
Step 5: Carry Aadhaar, the card and the passbook to the hospital
And give them at admission, so that the hospital's register is written from them.
Step 6: Read the discharge paper before leaving
The mother's name, the date and the baby's sex.
Step 7: If the payment does not come, ask where it stopped
The health worker or the hospital's accounts section can see the reason.
Step 8: Correct the paper that stopped it, and ask for the payment to be sent again
The bank account at the branch, or the hospital's entry.
What About the Mother and Child Card?
- The card is made when the pregnancy is registered, by the health worker or the nurse at the health centre.
- It records the mother's name, her husband's name, her age, and every check-up and dose after that.
- The same details go into the health worker's register, and into the system that tracks mothers and children.
- A health worker writes what she is told, in the village's way of naming its daughters-in-law.
- Show her Aadhaar at registration, and ask her to copy the name from it.
- A card already written otherwise can be corrected by her, at any visit. Ask her to correct her register as well.
The card is used for years after the birth, for the child's vaccines. Read vaccination certificate name mismatch.
What About the Hospital's Record?
- A woman in labour is admitted by whoever brought her: a husband, a mother-in-law, a neighbour.
- That person gives her name at the counter, and the clerk writes it in the delivery register.
- The discharge paper and the hospital's claim for her payment are made from that register.
- So hand over her Aadhaar and her card at admission, and say that the name is to be copied from them.
- Read the discharge paper before leaving. A slip is corrected by the hospital there and then.
- After discharge, the same correction means a trip back, with a newborn.
Read hospital discharge summary and medical records name correction.
Why Does the Mother Need Her Own Account?
- The ministry says the assistance is paid by direct benefit transfer, into a bank account.
- It is the mother's money, and goes to an account in her name.
- Many young wives have no account, or one opened in their parents' village in a maiden name.
- Either open a fresh account near her present home, on Aadhaar, or bring the old one up to date.
- See that the account is linked to Aadhaar, and that it has been used lately. A dormant account holds a credit.
- Give the health worker the account number and the bank's code from the passbook itself.
Read name change in bank records and Jan Dhan account name mismatch.
What If the Delivery Is Away From Home?
- Many women go to their parents' home for a first delivery, or to a town with a better hospital.
- The pregnancy was registered by one health worker, in one block. The baby is born under another.
- The hospital there does not know her. It has only what is said at its counter.
- Her card and her Aadhaar are what tie the two ends together. Carry both.
- Her parents' family may give her maiden name. Her card may carry her married one. The hospital should follow her Aadhaar.
- Tell her own health worker of the delivery soon after, with the discharge paper.
We could not confirm how a payment is handled between two blocks or districts. Ask the hospital's accounts section before discharge.
What About the Baby's Birth Certificate?
- A hospital reports each birth to the Registrar, from its own delivery register.
- So the mother's name on the baby's birth certificate is the name the hospital wrote at admission.
- A home name or a loose spelling there becomes the child's first official record of its mother.
- That is one more reason to have the hospital's entry right before leaving.
- The father's name is taken the same way. Give his as on his own Aadhaar.
- A certificate already issued with a slip is corrected by the Registrar, on the parents' identity papers.
Read name change in birth certificate, mother’s name correction in a birth certificate and name change for a newborn.
Where Does a Gazette Not Come In?
Anywhere in a pregnancy's paperwork. The scheme pays on the papers a woman holds today. The steps are set out only so that the first two can be read.
Step 1: Confirm that it is a change of name, and not a married surname or a home name
Those are settled by the health worker, the bank and a marriage certificate.
Step 2: Do not start a Gazette during a pregnancy for this
The sum is small and the time is short. Use the papers as they are.
Step 3: Fix the exact new name
The spelling that will go on Aadhaar and the bank account.
Step 4: Make the affidavit and publish the newspaper notice
Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update Aadhaar and the bank
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054.
See name change after marriage, for taking a married surname afterwards.
What If a Name Was Changed Earlier?
Occasionally a mother did change her name by choice before, and some papers never followed. If a Gazette was published then, a copy of it settles the matter. If not, these points apply, after the delivery.
Step 1: Remember who pays
The health department, into a bank account, by a central payment system.
Step 2: Remember what that system reads
Aadhaar, and the bank's record.
Step 3: Prefer the Gazette of India
In the rare case that one is needed at all.
Step 4: Contact the Roorkee press only if a state notice suits another purpose
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 5: Download the notice
Search egazette.gov.in and save the PDF.

Which Mistakes Do Families Make?
| Mistake | What to do |
|---|---|
| Giving the husband's bank account | The mother's own account |
| Registering in a married name that Aadhaar does not carry | Register in the Aadhaar name |
| Changing Aadhaar in the last months | Leave it until after the delivery |
| Going to the hospital without Aadhaar and the card | Keep them in the hospital bag |
| Letting a relative say the name at admission | Hand over the papers |
| Leaving without reading the discharge paper | Read it at the bed |
| Waiting months for a payment in silence | Ask where it stopped |
| Paying anyone to release the payment | Never. Ask the health centre instead |
| Asking for a Gazette | None is needed |
A typical case we handle: a young mother in Pauri delivered her first child at the district hospital. Her card read "Pooja Rawat", her married name. Her Aadhaar and her account read "Pooja Negi". The payment came back from the bank. Her father-in-law asked us for a Gazette so that she could "become Rawat". She did not need one for this, and it would have taken weeks. The health worker corrected the card and the entry to her Aadhaar name, and the payment was sent again. She took her married surname on Aadhaar months later, on her marriage certificate. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
For everything under this scheme. The health worker, the hospital and the bank settle the names. If anyone says a Gazette is needed for it, ask the health centre before spending anything.

That the scheme is a safe motherhood scheme under the National Health Mission launched on 12 April 2005, that it joins cash assistance with care at and after delivery, that the accredited social health activist is its link with pregnant women, that Uttarakhand is among the states given special dispensation, that payment is by direct benefit transfer, and that a smaller sum is described for a home delivery, are from the health ministry's annual report chapter and the National Health Mission's page. The papers a mother carries are from guides. We gave no amount. We could not confirm how a payment is handled where registration and delivery are in different blocks. How a card and a hospital register are written is general knowledge. The health worker and the hospital are the authority.

Names Under the Scheme: The Short Version
The assistance is paid into the mother's own bank account, in the name the health system holds for her. Early in the pregnancy, see that she has an account in her Aadhaar name, linked to Aadhaar. Have the health worker write the mother and child card in the same name. Take Aadhaar, the card and the passbook to the hospital, and read the discharge paper before leaving. Leave any change of surname until after the delivery.
