Hospital Discharge Summary and Medical Records Name Correction in Uttarakhand
When a hospital's papers carry a wrong name: which papers matter, how to ask the hospital to correct them, what it can and cannot change, and why to do it before a claim or a certificate depends on it.
A hospital discharge summary name correction is made by the hospital that issued it. Apply in writing to its medical records section with your identity proof, and ask for a corrected summary or a certificate that the papers relate to you. Do it early, as hospitals keep records for a limited time. No Gazette is needed.

A hospital discharge summary name correction is usually asked for when an insurer returns a claim, or when a birth or death certificate comes out with the hospital's mistake in it. The fix lies with the hospital, and it is quick if you ask soon. This page explains hospital discharge summary name correction and the correction of other medical records in Uttarakhand.
We do not deal with hospitals, and medical records need no Gazette. Our work is the Gazette for names. For the health account that links your records, read name change in ABHA health ID.
Which Hospital Papers Carry Your Name?
A hospital makes many papers for one patient, and they all take the name from the first one.
| Paper | What it is | Who reads it later |
|---|---|---|
| Registration slip or card | Made at the counter, with a hospital number | Every department of the hospital |
| Admission file | The in-patient record | The hospital, and an insurer on request |
| Discharge summary | The account of the stay, signed by the doctor | Insurers, employers, other doctors |
| Reports and scans | Laboratory and imaging results | Other doctors, insurers, visa medical boards |
| Bills and receipts | The charges, in the patient's name | Insurers, employers, the tax office |
| Birth or death report | Sent by the hospital to the Registrar | The Registrar, who issues the certificate from it |
| Medical certificates | Fitness, sickness, disability and the like | Employers, schools, licensing offices |
The registration slip is the root. Whatever is typed there is printed on everything that follows. A correction at the registration desk on the first day saves a dozen later.
How Does a Wrong Name Get on Hospital Papers?
- Typed from hearing. The counter is busy, and the clerk types what the attendant says.
- A home name. The person who brings the patient gives the name the family uses.
- An emergency. The patient cannot speak, and nobody present knows the full name.
- A newborn. The baby's file is opened as "Baby of" the mother, and the mother's name is taken from the admission slip.
- An old registration. A returning patient is found in the system under an earlier name or spelling, and the old entry is used again.
- A married woman. Her insurance card is in one surname and her identity proof in another, and the desk picks one.
None of this is anyone's fault in the moment. It becomes a problem only when the papers leave the hospital and meet a record that says something else.
Where Does a Wrong Name on Medical Papers Matter?
Money
- A health insurance claim, cashless or by reimbursement
- Reimbursement from an employer or a Government health scheme
- Treatment under a health cover scheme, where the card is matched at admission
- An accident claim, which rests on the medical record
- A tax deduction for medical expenses
Certificates and records
- A birth certificate, made from the hospital's report
- A death certificate, made the same way
- A disability certificate, which starts from medical papers
- Sick leave and fitness certificates given to an employer
- Medical papers for a visa or a job abroad
The two that cannot wait are the birth report and the death report. Once the Registrar has issued a certificate from the hospital's entry, you have a second office to deal with.
Which Case Is Yours?
| The situation | Example | Route |
|---|---|---|
| You are still admitted, or it is the day of discharge | The wristband has a wrong spelling | Tell the ward and the registration desk at once |
| The discharge summary has a wrong letter | Bist for Bisht | A written request to the medical records section, with identity proof |
| A home name or a short form is on all the papers | Guddu for Rajesh Singh | The same, with identity proof. Expect a certificate that both are one person |
| The papers are in a maiden name and the policy in a married name | Sunita Bisht and Sunita Rawat | The marriage certificate links them. Correct the hospital record if the insurer insists |
| The mother's name is wrong on a newborn's papers | A surname she never used | Correct it before the birth report goes to the Registrar |
| The name on a death report is wrong | A home name for the person who died | Ask the hospital at once, then the Registrar if the certificate is issued |
| The papers are part of a police case | An accident or injury report | The hospital will not alter them. See the section below |
Every row is for the hospital. None is for a Gazette.
How Do You Get Hospital Papers Corrected?
Each hospital has its own practice. This is the general method, and it works at Government and private hospitals alike.
Step 1: Collect the papers and note every place the name appears
The registration number, the admission number, the summary, the reports and the bills.
Step 2: Find out who deals with corrections
Usually the medical records section. In a Government hospital, the office of the Medical Superintendent. Ask at the enquiry desk.
Step 3: Apply in writing
Give the patient's name as recorded and as it should read, the hospital number, and the dates of the stay. Sign as the patient, or as the close relative.
Step 4: Attach identity proof
The patient's Aadhaar or other identity paper. If a relative applies, the relative's identity proof and a paper showing the relationship.
Step 5: Say exactly what you need
A corrected discharge summary, corrected bills, or a certificate that the papers relate to the person named in the identity proof. Ask what your insurer or office will accept before you choose.
Step 6: Take a dated receiving
A stamped copy of your application.
Step 7: Collect the corrected papers, and read them
Check the name, age, the dates and the hospital number on every page.
Step 8: Ask for the registration itself to be corrected
So that the next visit does not print the old name again.
What Can a Hospital Change, and What Not?
It can usually
- Correct the patient's name, age, address and relation's name in its registration
- Issue a corrected discharge summary, or the same summary with a correction signed by the doctor or the records officer
- Reissue bills and receipts in the corrected name
- Give a certificate that the papers in one name relate to the person in the other
It will not, and should not
- Rewrite clinical notes, findings or dates
- Put a different person's name on a patient's record
- Alter papers already sent to the police or a court
- Backdate anything
A correction of identity details is routine. A change to what happened medically is not a correction at all, and a hospital that offered it would be doing you no favour. An insurer that finds altered clinical papers can refuse the claim.
What Is Your Right to Your Medical Records?
The professional conduct regulations for doctors, made in 2002 and published by the National Medical Commission, say two things that matter here.
| Point | What the regulations say |
|---|---|
| A request for records | Where a patient or an authorised attendant asks for medical records, the request is to be acknowledged and the documents issued within 72 hours |
| How long records are kept | A physician is to maintain the records of indoor patients for three years from the start of treatment |
- So you are entitled to copies of your records, and a hospital should not make you wait for weeks.
- And a correction should be asked for early. After some years, the file that proves the mistake may no longer exist.
- Many hospitals keep records longer than this, and some kinds for much longer. Ask yours.
Where a hospital will not respond, write to its head. For a Government hospital, the Chief Medical Officer of the district is the next step. For a private one, its own grievance officer, and then the district health authority.
What About Birth and Death Reports?
When a birth or a death takes place in a hospital, the hospital reports it to the Registrar of births and deaths, and the certificate is made from that report.
A birth
- Give the mother's and the father's names from their Aadhaar at admission, in full
- Before discharge, ask to see the birth report, and read both parents' names
- If a name is wrong, ask the hospital to correct it before the report is sent
- If the certificate is already issued, the Registrar corrects it, with the hospital's corrected paper as proof
A death
- Give the hospital the identity paper of the person who has died
- Read the name, age and the relation's name on the hospital's papers before you leave
- Ask for a correction at once if a home name or a short form has been used
- If the certificate is already issued, the Registrar corrects it on proof
See mother's name correction in a birth certificate and name correction in a death certificate.
What Do Insurers and Employers Ask For?
An insurer, or the agency that handles claims for it, compares the hospital papers with the policy and with the patient's identity proof.
- At admission. Give the desk the insurance card and the identity proof together, and ask that the registration follow the policy name.
- Before discharge. Read the draft summary and the final bill. It is far easier to correct them before they are signed off.
- After a query. Ask the insurer in writing which paper it wants: a corrected summary, or a certificate from the hospital that both names are the same patient.
- Where the policy is the wrong one. Ask the insurer for an endorsement correcting the policy. The hospital papers may be right.
- For reimbursement from an employer or a Government scheme, the same applies. Ask the claims section what it accepts.
If a claim is refused only for a difference in the name after you have supplied the hospital's correction, use the insurer's grievance channel, and then the insurance ombudsman.
What About Medico-Legal Papers?
After an accident, an assault or an unnatural death, the hospital makes a medico-legal report, and a post-mortem report where there is a death. These go to the police and form part of a case.
- A hospital will not alter such a paper on a private request, and it should not.
- Where the name in it is wrong, tell the investigating officer in writing, with identity proof, so that the correct name is taken on record in the case.
- The doctor may give a clarification where the police or a court asks for one.
- For a claim or a case that rests on these papers, take a lawyer's advice.
See name wrong in an FIR, which follows the same idea.
What About Old Records in an Earlier Name?
Here the hospital made no mistake. Your reports from some years ago are in your maiden name, or in the name you had before a Gazette, and your identity papers have moved on.
- Old medical papers are not reissued in a new name. They record who you were when you were treated.
- Keep the linking paper with them: the marriage certificate, or the Gazette notification.
- At the hospitals you still use, ask for your registration to be updated to your present name, with your identity proof, so that new papers match.
- Tell your doctor that earlier records exist in the other name, so nothing in your history is missed.
This is the only place on this page where a Gazette appears, and only because you changed your name for other reasons. See name change after marriage in Uttarakhand.
How Does the Central Gazette Route Come In?
Not for the hospital's mistake. Only where you yourself are taking a new name, after which hospitals, like every other office, update their registration on your new identity proof.
Step 1: Confirm that you are changing your own name
A wrong spelling on a hospital paper is not a change of name.
Step 2: Fix the exact new name
The same spelling you will use on every paper.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and an undertaking showing the old and the new name.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update your records
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar, then your insurer and the hospitals you use.
Does the Uttarakhand Gazette Matter Here?
Not to a hospital. The question is only which Gazette Aadhaar, your insurer and your other records accept.
Step 1: Ask the Aadhaar centre which Gazette it accepts
Hospitals will follow your identity proof.
Step 2: Think about your insurer, PAN and the passport
If one Gazette has to serve them all, the Gazette of India is the safer choice.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.
To weigh the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Do Patients and Families Make?
| Mistake | What to do |
|---|---|
| Giving a home name at the registration desk | Hand over the identity paper, and read the slip |
| Not reading the discharge summary before leaving | Check the name, age and dates at the ward |
| Correcting a paper by hand | Never. Ask the hospital for a corrected one |
| Waiting years to ask | Ask at once. Files are kept for a limited time |
| Letting a birth or death report go out with a wrong name | Read the report before it is sent |
| Paying for an affidavit or a Gazette for a hospital's slip | The hospital corrects its own record |
| Asking for clinical details to be changed | Only identity details are corrected |
| Making an oral request | Apply in writing and keep a receiving |
| Correcting the summary and leaving the registration | Have the registration itself corrected |
A typical case we handle: a man in Kashipur had surgery at a private hospital, and his cashless claim was queried because the discharge summary gave his name as "Pappu Singh", the name his brother had given at the emergency counter, while his policy and Aadhaar said "Pradeep Singh". He asked us for an affidavit. We told him to go back to the hospital. Its records section reissued the summary and the bill in his full name on his Aadhaar, and the claim was settled. This is an illustrative example of the type of case we handle, not a named client review.
What is a lawyer's job, not ours
Medico-legal papers, a claim refused after the hospital has corrected its record, and any dispute with a hospital over what its record says.
When you do not need us
For hospital papers, always. The hospital corrects its own record on your identity proof, at no charge or a small copying charge. We are useful only where you yourself are taking a new name and need a Gazette.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. A hospital's record is corrected at that hospital. For your identity papers, send photos on WhatsApp and we will tell you which one to correct.
People Also Ask About Names on Medical Records
How can I correct my name in a hospital discharge summary?
Apply in writing to the hospital's medical records section with your identity proof.
Will a wrong name on a discharge summary affect my insurance claim?
It can be queried. A corrected summary or a certificate from the hospital usually settles it.
Can I correct the name on a medical report myself?
No. A paper altered by hand is treated as tampered.
How long does a hospital keep medical records?
The regulations for doctors speak of three years for in-patients. Many hospitals keep them longer.
Can I get copies of my medical records?
Yes. The regulations say documents are to be issued within 72 hours of a request.
Is an affidavit or a gazette needed to correct hospital records?
No. The hospital corrects its own record on identity proof.
Did the NMC change the 72-hour rule for medical records?
In August 2023 the NMC issued new professional conduct regulations under which a request for records was to be met within five working days instead of the 72 hours in the 2002 regulations, as reported. Those 2023 regulations were then put on hold.
Are the NMC's 2023 professional conduct regulations in force?
By a notification of 23 August 2023, the NMC's ethics board said the 2023 regulations would not be operative until a further Gazette notification, as reported. We could not find a later notification bringing them back, which is why this page goes by the 2002 regulations.
From when are the three years for keeping records counted?
The 2002 regulations count from the start of treatment. The 2023 regulations, now on hold, would have counted three years from the last contact with the patient, as reported.
Did the 2023 regulations say anything about emergencies?
As reported, they said that in a medical emergency efforts should be made to make the records available at the earliest. The 2022 draft had proposed the same day.
Will hospital records have to be digital?
The 2023 regulations, now on hold, proposed fully digitised records within three years of their publication, as reported. Many hospitals already keep digital records. Ask yours for a printed copy with its seal.
Do doctors have to keep outpatient records too?
The NMC's 2022 draft proposed keeping records of outpatients and inpatients for three years. The 2023 text, as reported, spoke of inpatients. Either way, ask early for any correction.
