Name Change in Disability Certificate and UDID Card in Uttarakhand
When the name on a disability certificate or a UDID card is wrong or out of date: who issued it, how the record is updated, and why it has to match Aadhaar for a pension or a concession.
A name change in disability certificate and UDID card records is made through the UDID portal and the medical authority that issued them, usually the Chief Medical Officer's office. Correct Aadhaar first, because the record is matched with it. No Gazette is needed for a spelling correction. A new name follows the Gazette used for Aadhaar.

A name change in disability certificate records matters because benefits are paid against it. A pension, a travel concession or a reserved seat is given to the person named on the certificate, and one wrong letter can hold it up. This page explains name change in disability certificate and UDID card records in Uttarakhand: who issues them, how they are updated, and where to get help.
We do not handle these certificates, and for most people no Gazette is involved. Our work is the Gazette for names. Because the record follows Aadhaar, start with name change in Aadhaar card in Uttarakhand if your name is changing.
What Are the Disability Certificate and the UDID Card?
A disability certificate states the kind and the percentage of a person's disability. It is issued under the Rights of Persons with Disabilities Act, 2016 by a medical authority, after assessment by a specialist or a medical board.
The UDID card is the Unique Disability Identity card. It comes from a national project of the Department of Empowerment of Persons with Disabilities, which keeps a single database of persons with disabilities.
- Since June 2021, disability certificates and UDID cards are issued through the project's online portal.
- The application goes to the Chief Medical Officer's office or the notified medical authority of the district.
- After verification and assessment, the certificate and the card are issued electronically, and can be downloaded.
- The card carries a number, the person's name, a photograph and the details of the disability.
So the two are one record in practice. The name on the certificate, on the card and in the database is the same entry, and it is the medical authority that controls it.
Where Does the Name Come From?
- From the application. The name typed when the person, a family member or a help centre filled the form.
- From Aadhaar. The Department has made the Aadhaar number mandatory for applying, and has issued a procedure for an identity check of applicants on the portal.
- From an old certificate. Where a handwritten certificate was brought onto the system, the name was typed from it.
That gives the usual causes of a wrong name. The form was filled by someone else in a hurry. The old certificate was hard to read. Or Aadhaar itself carried a different form of the name on the day.
It also tells you the order of work. Where Aadhaar is the paper that is wrong, correct Aadhaar first. The disability record will be compared with it.
Where Is the Name Checked?
| Benefit or use | What the name is compared with |
|---|---|
| Disability pension from the state | Aadhaar and the bank account the pension is paid into |
| Travel concession on trains and buses | An identity proof shown with the card |
| Reserved seats and posts | The application form, the school certificates and identity proof |
| Scholarships | The student's school record, Aadhaar and bank account |
| Tax relief | PAN and the certificate, for the person or the family member claiming |
| Aids and appliances under schemes | Aadhaar and the card |
Payments now go straight into bank accounts after matching with Aadhaar. A difference between the certificate and Aadhaar is the commonest reason a pension is held. It is worth putting right before you apply for a benefit, not after it stops.
Which Case Is Yours?
| The situation | Example | Route |
|---|---|---|
| A wrong letter on the certificate and the card | Bist for Bisht | An update request through the portal or the medical authority, with Aadhaar |
| A short form or a home name | Guddu for Rajesh Singh | The same, with Aadhaar and a school certificate |
| Aadhaar itself is wrong | Both carry the same mistake | Correct Aadhaar first, then the disability record |
| The father's or guardian's name is wrong | A letter wrong in the father's name | An update request, with a paper showing the right name |
| A surname after marriage | Certificate in the maiden name, Aadhaar in the married name | An update request, with the marriage certificate |
| A new name | A name changed by Gazette | Update Aadhaar with the Gazette, then the disability record |
| An old paper certificate in a different spelling | A handwritten certificate from years ago | Apply on the portal in the name as on Aadhaar |
Only one row involves a Gazette, and there the Gazette is for Aadhaar. The disability record then follows.
How Do You Get the Name Corrected?
The Department describes the portal as allowing online renewal and update of information by or on behalf of persons with disabilities. We could not confirm the exact screens for a name, so this is the general method. The Chief Medical Officer's office will tell you its own steps.
Step 1: Compare the certificate, the card and Aadhaar
Write down the name on each, with the father's or guardian's name and the date of birth.
Step 2: Correct Aadhaar first, if it is the wrong one
At an Aadhaar centre. Wait for the updated Aadhaar.
Step 3: Sign in to the UDID portal
With the UDID number or enrolment number and the details the portal asks for.
Step 4: Use the option to update your information
Enter the name exactly as on Aadhaar, and upload the papers asked for.
Step 5: Follow it up with the medical authority
The request is dealt with by the Chief Medical Officer's office or the medical authority that issued the certificate. Visit with the originals if asked.
Step 6: Keep the reference
The application number from the portal, or a dated receiving from the office.
Step 7: Download the corrected certificate and card
Read the name, the guardian's name, the date of birth and the details of the disability.
Step 8: Give the corrected card to the offices that pay you
The social welfare office for a pension, the school or college, the employer.
A correction of the name does not need a fresh medical assessment by itself. If the office says it does, ask why, and ask the District Social Welfare Officer for help.
What About an Old Paper Certificate?
Many people hold a disability certificate written by hand at a district hospital years ago. It may carry a home name, a spelling no other paper has, or no surname.
- An old certificate is brought onto the national system by applying for a UDID card. That is the moment to get the name right.
- Apply in the name as on Aadhaar, and upload the old certificate with it.
- If the office points to the difference in names, explain that the old certificate was written from what was spoken, and show Aadhaar and any school certificate.
- Where it asks for a sworn statement that both names are the same person, give one. See name spelling correction affidavit.
Keep the old certificate even after the new one is issued. It shows how long the disability has been on record, which some benefits ask about.
Why Is Renewal the Time to Check?
- Some certificates are issued for a fixed period, where the condition may change, and have to be renewed. The Department has a procedure for renewal requests through the portal.
- At renewal, the record is opened again in any case. Ask for the name to be corrected in the same request.
- Do not let a certificate expire while a correction is pending. Benefits tied to it may stop.
- A permanent certificate is not renewed, so nothing will prompt a check. Look at it now.
The Department also has procedures for a card that is lost, and for cases where the kind or percentage of disability has changed. Those are separate from a correction of the name.
What About a Child, or an Adult Who Cannot Apply?
A child
- A parent applies on the child's behalf
- The child's name follows the birth certificate and the child's Aadhaar. Put those right first
- Check the parent's name on the card as well. It is compared with the parent's own papers
- Give the school the same name, so scholarships match
An adult who cannot manage the application
- The portal allows an update on behalf of the person
- A family member or guardian can go to the medical authority with the person's papers
- Where a legal guardian has been appointed, carry that order
- The District Social Welfare Officer can assist
For a child's Aadhaar, see Baal Aadhaar child name correction.
Where Can You Get Help?
- The Chief Medical Officer's office, or the medical authority named on your certificate. It issued the record and corrects it.
- The District Social Welfare Officer. The portal lists this office for assistance, and it also deals with disability pensions.
- The portal's own helpdesk, through the contact details on the UDID portal.
- The State Commissioner for Persons with Disabilities, where an office will not act. The law gives this office the power to look into complaints.
All of these help free of charge. A person with a disability should not have to pay an agent, or travel twice, for a spelling to be put right. If you are asked to, write to the District Social Welfare Officer.
What Should You Not Do?
- Do not apply for a second UDID card to get the name right. One person has one record, and the Department has warned about fraudulent cards and certificates.
- Do not alter a printed certificate or card. It carries a code that is checked against the database.
- Do not use a card in a name your Aadhaar no longer carries without asking for it to be updated.
- Do not pay for a Gazette or an affidavit on a shop's word. Ask the medical authority what it needs.
How Does the Central Gazette Route Come In?
Only where the person has taken a new name. Aadhaar asks for a Gazette for a major change, and the disability record then follows Aadhaar.
Step 1: Confirm that the change is one Aadhaar treats as major
A spelling correction or a surname after marriage usually is not.
Step 2: Fix the exact new name
The same spelling you will use on every paper.
Step 3: Make the affidavit
State the old name, the new name, the father's or husband's name and the Aadhaar address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in the old name with two witnesses, and an undertaking showing the old and the new name.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file, then update Aadhaar and the disability record
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar, and then ask for the disability record to be updated.
Where the person cannot sign, or cannot act for himself or herself, ask before starting. Who may sign the papers is a question to settle first.
Does the Uttarakhand Gazette Matter Here?
Not to the UDID portal. The question is only which Gazette Aadhaar and your other records accept.
Step 1: Ask the Aadhaar centre which Gazette it accepts
The disability record will follow Aadhaar.
Step 2: Think about the other records
If one Gazette has to serve them all, the Gazette of India is the safer choice.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Families Make?
| Mistake | What to do |
|---|---|
| Letting a help centre type the name from hearing | Hand over Aadhaar, and read the form before it is submitted |
| Asking for the certificate to be changed while Aadhaar is wrong | Correct Aadhaar first |
| Applying for a second card | Ask for the existing record to be updated |
| Waiting until the pension stops | Check the name before you apply for a benefit |
| Paying an agent for a correction | The medical authority and the welfare office help free |
| Paying for a Gazette to fix a spelling | An identity paper settles a spelling |
| Throwing away the old paper certificate | Keep it. It shows how long the disability has been on record |
| Forgetting the parent's or guardian's name on the card | Check it against that person's own papers |
| Letting a temporary certificate expire | Renew in time, and correct the name in the same request |
A typical case we handle: a father in Almora called us because his son's disability pension had not started. The UDID card, made from an old handwritten certificate, gave the boy's home name, and his Aadhaar and bank account had his school name. The father thought a Gazette was needed. It was not. He asked the Chief Medical Officer's office to update the record to the name on Aadhaar, with the school certificate, and the pension application then went through. This is an illustrative example of the type of case we handle, not a named client review.
