Name Spelling Correction Affidavit in Uttarakhand
The affidavit that says two spellings belong to one person: when an office asks for it, what it must say, a sample format, the cost and what it cannot do.
A name spelling correction affidavit is a sworn statement that a name written two ways in your papers belongs to one and the same person, and which spelling is correct. It is made on stamp paper before a notary. It supports a correction at the office that holds the record. It does not correct any record by itself.

A name spelling correction affidavit is the paper a bank, a school or an employer most often asks for when two of your documents spell your name differently. This page explains the name spelling correction affidavit in Uttarakhand: when it is asked for, what it must say, a sample format and its limits.
It is not the affidavit used for a new name. For that, see name change affidavit in Uttarakhand. To find which office corrects your record, read name spelling correction in Uttarakhand.
What Is a Name Spelling Correction Affidavit?
It is a written statement, sworn before a notary public, in which you declare three things: that your name appears in two forms in your papers, that both forms refer to you, and which of them is correct.
People call it by several names: a name correction affidavit, a name discrepancy affidavit, or a "one and the same person" affidavit. They are the same kind of paper.
- It is your own statement on oath. No office issues it to you.
- It is typed on non-judicial stamp paper and signed in front of the notary.
- It names the two papers that differ, so a reader can check them.
- It does not say you have changed your name, because you have not.
How Does It Differ From a Name Change Affidavit?
| Point | Spelling correction affidavit | Name change affidavit |
|---|---|---|
| What happened | A record misspelt your true name | You have taken a new name |
| What it says | Both spellings are one person, and this one is correct | I was known by one name and shall be known by another |
| Names the records | Yes, the two that differ | Usually not |
| What follows | A correction at the issuing office | A newspaper notice and a Gazette |
| Records to update | The wrong one only | All of them |
Typists near the courts keep both formats and often hand over whichever is open on the screen. Read the numbered lines before you sign. If the draft says "I have changed my name", it is the wrong affidavit for a correction, and it will confuse the office you give it to.
When Is the Affidavit Needed, and When Is It Not?
It is often asked for by
- A bank, where the account name and an identity paper differ slightly
- A school or college, with its own correction form
- A board or university, along with its application
- An employer's establishment section, for the service record
- A tehsil or a local body, for a state record
It is usually not needed for
- An Aadhaar correction, which is made on a supporting document from UIDAI's list
- A PAN correction, made on the correction application with proof
- A voter ID correction through Form 8
- A passport, which has its own document list and declaration formats
- A birth certificate correction on the Registrar's own form
So ask the office first. If it has its own format, use that. A general affidavit does not replace an office's prescribed form. And if the office will correct the record on a supporting document alone, you have saved the cost of an affidavit.
What Must the Affidavit Contain?
| Part | What to write | Common error |
|---|---|---|
| Your name | Your correct name, as on your trusted record | The misspelt form used at the top |
| Father's or husband's name | In full | Initials only |
| Age and address | As on your identity proof | A different address from your papers |
| The correct spelling | With the record that carries it, and its number | A spelling that is on no record |
| The wrong spelling | Exactly as the record shows it, with that record and its number | The mistake "tidied" while typing |
| How it arose | One plain line, such as a clerical error | A long story, or blame |
| One and the same person | That both spellings refer to you | Left out |
| The request | That the record be read or corrected in the right spelling | A claim that the name has been changed |
| Verification, place, date, signature | That the contents are true, signed before the notary | Signed at home, or undated |
The line that names the two records does the real work. An office reads the affidavit with those two papers beside it. If a record number or a spelling in the affidavit does not match the paper, the affidavit is of no use.
What Is the Format for Your Own Name?
This sample shows the usual layout. It is not an official form, and a notary may word it a little differently. The name, address and numbers are made up.

Change lines 1 and 2 to name your own two records. Sign in the correct spelling, as on the identity proof you show the notary.
What Is the Format for a Parent's Name in Your Papers?
A marksheet or a passport often carries a parent's name with a wrong letter. The person whose record it is makes the affidavit. This is also only the usual layout, with made-up names.
AFFIDAVIT
I, Rohit Bisht, son of Shri Trilok Singh Bisht, aged 22 years, resident of Village Kasardevi, Post Office Kasardevi, District Almora, Uttarakhand 263601, do hereby solemnly affirm and declare as under:
1. That the correct name of my father is Trilok Singh Bisht, as recorded in his Aadhaar card and his own certificates.

- Attach a self-attested copy of the parent's identity proof, to show the right spelling.
- Some offices also ask for an affidavit from the parent. Ask before you make either.
- For a child under 18, the parent makes the affidavit about the child's record.
How Do You Make the Affidavit?
Step 1: Ask the office what it wants
Find out if an affidavit is needed at all, and if the office has its own format.
Step 2: Put the two records side by side
The one with the right spelling and the one with the wrong. Note the number on each.
Step 3: Type the draft
Copy each spelling from its record, letter by letter, the mistake included.
Step 4: Read it aloud
Check that the correct and the wrong spellings have not been swapped, and that nothing says the name was changed.
Step 5: Print it on stamp paper
Non-judicial stamp paper. The vendor or the notary will tell you the current value.
Step 6: Sign before the notary
Carry your identity proof. The notary signs, seals and enters it in the register.
Step 7: Keep copies and submit it with proof
Give the office the affidavit with self-attested copies of both records, and keep a scan.
Where Is It Made in Uttarakhand, and What Does It Cost?
At the district court of your own district, or at a tehsil where a notary sits. Stamp vendors and typists are usually found in the same place.
| Item | Typical cost |
|---|---|
| Stamp paper and notary together | Rs 100 to Rs 500 |
| Typing and printing | A small amount |
| Our drafting help | Quoted per case |
The charge differs from one notary to another, so ask before the work starts. Our associate office is at the Dehradun District Court on Haridwar Road, near Prince Chowk. For other districts, we draft on WhatsApp and you sign before a notary near you.
One affidavit covers one difference. If three records carry three different mistakes, list each in the same affidavit, with its own record and number, or ask the office if it wants a separate one for each.
What Can This Affidavit Not Do?
It cannot
- Correct any record by itself. Only the office that holds the record can
- Stand in for the supporting document an office asks for
- Stand in for a Gazette where an office asks for a Gazette
- Support a spelling that is on none of your papers
- Bind an office that does not accept affidavits for this purpose
It can
- Put on oath that two spellings are one person
- Support an office's own correction form
- Explain a small difference that is not worth correcting, such as an old passbook
- Be shown years later with the two records it names
An affidavit is your word. Its weight comes from the records it points to. A shop that sells it as a "name correction certificate" is misdescribing it.
What If the Office Wants a Gazette Notification?
Then the paper is a public notice in Part IV of the Gazette of India, which states the wrongly written name and the actual name. The Department of Publication's list for it names an undertaking, not a notarised affidavit.
Step 1: Get the office's requirement in writing
Confirm that it wants a Gazette notification, and which spelling it treats as correct.
Step 2: Sign the undertaking
It shows the correct and the wrong name, your father's or husband's name and your address.
Step 3: Publish the newspaper notice
In a daily local leading newspaper, showing the details of the case. Keep the original newspaper.
Step 4: Type the public notice form
Two typed copies of the specimen, signed by you and two witnesses.
Step 5: Prepare the CD, photographs and supporting papers
The same text on a CD in MS Word, two self-attested photographs with identity proof, a certificate that both copies match, and an attested copy of the connected document.
Step 6: Pay the public notice charge on BharatKosh
Commonly Rs 1,400. Confirm it before you pay. It is not refunded.
Step 7: Send the file yourself, then take the Gazette to the office
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. When it is published, apply to the office that holds the wrong record.
The full route is on our public notice for wrongly written name page.
Can the Uttarakhand Gazette Be Used in Its Place?
The Government Press at Roorkee sets its own practice. Ask the office that wants a Gazette which one it accepts.
Step 1: Ask the office which Gazette it accepts
A board or office outside the state may want the Gazette of India.
Step 2: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private public notice for a wrongly written name is being accepted.
Step 3: Ask what declaration it wants
An undertaking or an affidavit, in which language, and the fee.
Step 4: Submit as directed and keep copies
Keep a photocopy of every page.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do People Make With This Affidavit?
| Mistake | What to do |
|---|---|
| Using a name change affidavit for a correction | Use the format that says both spellings are one person |
| Making an affidavit the office never asked for | Ask the office first |
| Not naming the two records | Give each record and its number |
| The wrong spelling "tidied" by the typist | Copy it from the record, mistake and all |
| The correct and wrong spellings swapped | Read it aloud before signing |
| A "correct" spelling that is on no record | That is a change of name |
| Treating the affidavit as the correction | Take it to the office that holds the record |
| Signed at home, then taken for a seal | Sign in front of the notary |
| A general affidavit where the office has its own form | Use the office's form |
A typical case we handle: a pensioner in Almora had an old bank passbook with one letter of his surname different from his Aadhaar. The branch asked for an affidavit. A typist gave him a name change affidavit, and the branch sent him back because he had not changed his name. He made a correction affidavit naming the passbook and the Aadhaar, and the branch updated the account. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
A typist and a notary at any court can make this affidavit from the samples on this page. For Aadhaar, PAN and the voter ID you usually need no affidavit at all. We are useful when several records carry different mistakes, or when an office has asked for a Gazette public notice. Our charge is quoted per case.

Name Spelling Correction Affidavit: The Short Version
This affidavit says two spellings in your papers are one person, and which is correct. It names both records, gives the wrong spelling exactly as written, and never says the name was changed. Ask the office first if it needs one, because Aadhaar, PAN and the voter ID are usually corrected without it.
It costs Rs 100 to Rs 500 and supports a correction. It does not make one. To have yours drafted or checked, send the two papers on WhatsApp 9540005064, and we will reply in working hours.
