Public Notice for Wrongly Written Name in Uttarakhand
When a name was always right but a record wrote it wrongly: what the public notice is, when you need it, the papers, the format and how to apply.
A public notice for a wrongly written name is a Gazette of India notice which states that your name, or a parent's or child's name, was written wrongly in a record and gives the correct name. It is different from a change of name notice. It needs an undertaking, a newspaper notice, the typed form and a fee.

A public notice for wrongly written name is meant for people who never changed their name at all. A clerk, a school or an operator wrote it wrongly, and now two records disagree. This page explains the public notice for wrongly written name for people in Uttarakhand: when it is the right notice, the papers and how to apply.
If you want a new name, this is not your page. Read gazette name change in Uttarakhand. And before either, check if the office that made the mistake can correct it without any Gazette.
What Is a Public Notice for a Wrongly Written Name?
The Gazette of India, Part IV, carries more than one kind of notice from private persons. One is the change of name notice. Another is the public notice. The Department of Publication's guidelines describe the public notice as the one used for correction of names, among other matters.
In a public notice you do not say "I have changed my name". You say that a name was wrongly written in a certain record, and you state the actual name. The name in question can be your own, or the name of your father, mother, husband, wife or minor child as it appears in your papers.
- It is published in the same Gazette, in the same Part IV.
- It has its own guidelines, its own form and its own fee.
- It is downloaded free from egazette.gov.in, like any other Gazette.
Like every Gazette notice, it is supporting proof. It does not correct the record by itself. You still apply to the board, the employer or the office that holds the wrong record.
Public Notice or Change of Name: Which One Is Yours?
Using the wrong notice is the costliest mistake in this area, because the fee is not refunded.
| Point | Public notice | Change of name notice |
|---|---|---|
| What happened | Your name was always the same. A record wrote it wrongly | You are taking a name you did not have before |
| What the notice says | That the name was wrongly written as one thing, and the actual name is another | That you have changed your name and will be known by the new one |
| Whose name | Yours, or your father's, mother's, spouse's or minor child's, in your papers | Your own, or your minor child's |
| Example | Father is Trilok Singh Bisht. The marksheet says Tirlok Singh Bist | Priya wants to be known as Pari |
| Extra paper | An attested copy of the connected document | None of that kind |
| Fee | Its own fee. Confirm the current amount | Commonly Rs 1,100 adult, Rs 1,700 minor |
A simple test
Ask yourself: which name is on my oldest and most trusted record, and has it ever really changed? If the true name has stayed the same and some records got it wrong, your case is a correction. If you are choosing something new, your case is a change of name.
Which Cases Does a Public Notice Cover?
The form names the records where the wrong name may appear: educational documents, a service book, a driving licence, a passport, a PAN card or an Aadhaar card. These are the cases we see from Uttarakhand.
Your own name in a certificate
Your class 10 certificate says Bist, and every other paper says Bisht.
Father's name in your papers
Your marksheet or passport carries your father's name with a wrong letter or a missing surname.
Mother's name in your papers
Your mother's name is written differently in your school record and in her own Aadhaar.
A minor child's name
The school has recorded the child's name wrongly, and the parent makes the notice.
A service book
The name in the service record differs from the name in the educational certificates.
Spouse's name
Your husband's or wife's name is wrongly written in your passport or another record.
The same guidelines also list other kinds of public notice, such as a change of sex and certain cases after re-marriage. Those have their own papers. This page deals only with a wrongly written name.
Can the Issuing Office Fix It Without a Gazette?
Often, yes. A public notice is worth its cost only when an office asks for Gazette proof, or when records conflict and no single paper settles it.
| Where the name is wrong | Who corrects it | Is a Gazette usually needed? |
|---|---|---|
| Birth certificate | The Registrar who issued it, under Section 15 of the Registration of Births and Deaths Act, 1969 | No. It is not a Gazette matter |
| Aadhaar, a typing or transliteration error | UIDAI, with the supporting document its rules list | No |
| Class 10 or 12 certificate | The board, through your school, on its own correction form | Ask the board. Some cases need it as support |
| PAN | The PAN correction application, with proof | Usually not for a plain error |
| Service book | Your establishment section | Ask what it will accept |
| Passport | Passport Seva, as a re-issue with the right papers | Check its list for your case |
So the first step costs nothing: write to the office that holds the wrong record and ask what it needs to correct it. If the answer includes a Gazette notification, come back to this page. If it does not, you do not need a public notice, and you do not need us.
Which Documents Are Required for a Public Notice?
This is the list in the Department of Publication's guidelines for a public notice in the Gazette of India, Part IV.
| Document | What it must be |
|---|---|
| 1. Undertaking | Signed by you, showing the correct and the wrong name, your father's or husband's name and your address |
| 2. Original newspaper | The notice printed in a daily local leading newspaper, showing the details of the case |
| 3. Prescribed form | Computer typed, in two copies, signed by you and two witnesses |
| 4. Soft copy | A CD with the same text in MS Word, without the witness part, with your name typed in place of the signature |
| 5. Photographs and identity proof | Two self-attested passport size photographs and a self-attested photocopy of photo identity proof |
| 6. Certificate | Signed by you, stating that the hard copy and the soft copy carry the same contents |
| 7. Request letter with the fee | Addressed to the Controller of Publications, with the fee receipt |
| 8. Connected document | An attested photocopy of the legal or other document the case rests on |
Item 8 is what sets a public notice apart. It is the paper that shows the error and the paper that shows the truth: for example, the marksheet with the wrong spelling and the identity proof with the right one.
What Is the Format of a Public Notice for a Wrongly Written Name?
The Department gives a specimen. You keep the words that fit your case and remove the rest. This is its layout.
It is for general information that I [NAME], son / daughter / wife of [NAME], residing at [FULL ADDRESS], declare that the name of mine / my father / my mother / my minor son / my minor daughter / my wife / my husband has been wrongly written as [WRONG NAME] in my [educational documents / service book / driving licence / passport / PAN card / Aadhaar card]. The actual name of mine / my father / my mother / my minor son / my minor daughter / my wife / my husband is [CORRECT NAME], which may be amended accordingly.
It is certified that I have complied with other legal requirements in this connection.

Witness 2: signature, full name, address, mobile
A filled sample
It is for general information that I, ROHIT BISHT, son of Shri TRILOK SINGH BISHT, residing at Village Kasardevi, Post Office Kasardevi, District Almora, Uttarakhand 263601, declare that the name of my father has been wrongly written as TIRLOK SINGH BIST in my educational documents. The actual name of my father is TRILOK SINGH BISHT, which may be amended accordingly.
It is certified that I have complied with other legal requirements in this connection.
ROHIT BISHT
[Signature of applicant]
The name and address in the sample are made up. For a minor's record, the specimen also has a place for the child's name and age. Always type your notice on the current specimen from the Department's website, because the Department can revise it.
- The form must be computer typed, in two copies.
- You sign it, and two witnesses sign with their full names, addresses and mobile numbers.
- Type the wrong name exactly as the record shows it, mistake and all.
- Type the correct name exactly as your trusted proof shows it.
What Goes in the Undertaking and the Newspaper Notice?
The undertaking
- The correct name and the wrong name
- Your father's or husband's name in full
- Your residential address
- A statement that the facts are true and correct to the best of your knowledge and belief
- Your signature
The newspaper notice
- Printed in one daily local leading newspaper
- Showing the details of the case: who you are, the wrong name, the record it is in and the correct name
- Sent to the Department as the original newspaper
- Worded as a correction, not as a change of name
An Indian living abroad submits a deed attested by the Indian Embassy or High Commission, as the guidelines say. Read that part of the guidelines before you sign anything from abroad.
Keep the same spelling of every name in the undertaking, the newspaper, the form and the soft copy. In this kind of notice there are two names to get right: the wrong one and the correct one.
What Is the Fee for a Public Notice in the Gazette?
The public notice has its own printing charge, separate from the change of name fee. The guideline on the Department's website still shows Rs 1,400, a figure dated to the 2016 to 2017 financial year. The Department revises its charges, so confirm the current amount with it before you pay. We do not want to print a figure here that may be out of date.
| Item | Typical cost | Paid to |
|---|---|---|
| Gazette charge for a public notice | Confirm the current amount | The Government, on BharatKosh |
| Newspaper notice | Rs 800 to Rs 4,000 | The newspaper |
| Attestation, photographs, CD, printing | A small amount | Local shops and the attesting officer or notary |
| Speed Post to Delhi | A small amount | India Post |
| Our assistance | Quoted per case | Us, shown as a separate line |
- The charge is paid only through BharatKosh, the Non-Tax Receipt Portal.
- It is not refunded.
- The guidelines list small additional charges for some cases, such as an alias. Ask the Department if any applies to your wording.
How Do You Apply for a Public Notice in the Gazette of India?
Step 1: Ask the issuing office what it needs
Confirm that the office holding the wrong record wants a Gazette notification, and which name it treats as correct.
Step 2: Gather the proof of both names
Keep the record with the wrong name and the trusted proof with the correct name. Get photocopies attested.
Step 3: Publish the newspaper notice
Print the details of the case in one daily local leading newspaper and keep the original.
Step 4: Type the form and the undertaking
Type the form in two copies on the specimen. Sign it, with two witnesses. Sign the undertaking.
Step 5: Make the CD and the certificate
Put the same text on a CD in MS Word, without the witness part, with your name typed in place of the signature. Sign the certificate that both copies match.
Step 6: Pay the charge on BharatKosh
Confirm the current amount, pay online and print the receipt.
Step 7: Send the file to the Department
Send it by post, or hand it in yourself, to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054, with a forwarding letter and self-attested photo identity proof.
Step 8: Download the Gazette
On egazette.gov.in, search the weekly Gazette, Part IV, between Saturday dates, open the PDF and find your name. Download the whole Gazette as your copy.
How to pack and post the envelope is on our send the gazette file from Uttarakhand to Delhi page.
Which Rules of the Gazette Office Should You Know?
These come from the Department's own guidelines for a public notice.
Who may submit
The application is taken only when submitted personally or sent by post or courier. The guidelines say no other channel is permissible, and they name agents and advocates.
Public dealing hours
10:00 AM to 1:00 PM and 2:00 PM to 4:00 PM on all working days, if you go in person. Check the Department's website before you travel.
One year, and no return
Documents must not be older than one year. Nothing submitted is returned, neither original nor photocopy.
The office can refuse
The Controller of Publications may reject a request whose printing matter is incomplete, vague, unlawful or misleading.
Printing time
Once a complete application is received, it is sent to the Government of India Press. The Department says it is not responsible for a delay at the press.
The copy
Printed Gazettes are no longer sold. The Gazette has been published electronically since 1 October 2015, and the copy you download needs no certification from the Department.
What Do You Do After the Public Notice Is Published?
- Download the whole Gazette PDF and note the date of the issue and the page number.
- Read your notice. Check the wrong name, the correct name and the address.
- Print the first page of the issue and your page, in a few sets.
- Apply to the office that holds the wrong record, on its own form, with the Gazette as support.
- Keep the Gazette, the newspaper and a copy of the undertaking together for life.
Step 4 is the one people forget. The Gazette office does not write to your board, your employer or the passport office. A class 10 certificate from the Uttarakhand Board of School Education, for example, is corrected only by the board, on its own form and fee, through your school.
How to get the PDF and which pages to print are on our download the gazette copy page. If the notice itself is printed with an error, see gazette corrigendum.
Can a Public Notice Go in the Uttarakhand Gazette?
The Uttarakhand Gazette portal lists Public Notice among its categories. What the Government Press at Roorkee accepts from a private person, and at what charge, is for the press to say.
Step 1: Ask the office which Gazette it accepts
Ask the board, employer or office holding the wrong record if a state Gazette will do, or if it wants the Gazette of India.
Step 2: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in. Ask if a private public notice for a wrongly written name is being accepted.
Step 3: Ask for the format, the papers and the fee
Note the wording the press wants, the supporting papers and how the fee is paid.
Step 4: Submit as directed and keep copies
Hand in or post the file as the press asks, and keep a photocopy of every page.
Step 5: Download the notice from the state portal
Search gazettes.uk.gov.in and save the PDF.
If the wrong record is with a Central office, such as the passport office, or with a board or employer outside the state, use the Gazette of India. See Uttarakhand Gazette vs Gazette of India.
Which Mistakes Should You Avoid With a Public Notice?
| Mistake | How to avoid it |
|---|---|
| Using the change of name form for a correction | Use the public notice specimen |
| Paying for a Gazette when the issuing office would have corrected the record itself | Ask that office first, in writing |
| The "correct" name in the notice is not on any trusted proof | Take the correct name from your strongest document |
| The wrong name is typed differently from the record | Copy it exactly from the record, error included |
| The record is not named in the notice | Say where the wrong name appears |
| No attested copy of the connected document | Attach it. The guidelines ask for it |
| A handwritten form, or one witness | Computer typed, two copies, two witnesses |
| Last year's fee paid | Confirm the current charge before paying |
| Stopping after the Gazette | Apply to the office that holds the record |
A typical case we handle: a young woman in Dehradun found that her class 12 marksheet carried her father's name with a wrong spelling, and her passport file was held up. She had been told to do a "name change". Her own name had never changed, and neither had her father's. A public notice for the wrongly written name was the right paper, followed by the board's own correction. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
If the issuing office corrects the record on its own form, you need no Gazette and no service. If you do need a public notice and your case is one name in one record, the guidelines and this page are enough to file it yourself. We are useful when several records disagree, when the error is in a parent's name across a child's papers, or when you cannot tell if your case is a correction or a change. Our charge is quoted per case.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The file is prepared on WhatsApp and sent by post from your own district.

It is reviewed by Advocate Vipin Chauhan.
The list of documents, the format and the office rules on this page were checked in October 2026 against the Department of Publication's "Guidelines for Notification of Public Notice". That guideline still prints a charge dated 2016 to 2017, so this page does not state a current fee and tells you to confirm it. The filled sample uses a made-up name and address.
