Public Notice Proforma Format for a Wrongly Written Name
The Gazette form for a name correction: the specimen, how to pick the right words, filled samples for each case and the rules for typing and signing.
The public notice proforma is the typed Gazette form for a wrongly written name. It states who you are, whose name was wrongly written, how it was written, in which record, and the actual name. You type it in two copies, sign it, add two witnesses and send the same text on a CD in MS Word.

The public notice proforma is one paragraph long, but the specimen packs seven choices of person and six kinds of record into it, and most errors come from leaving the wrong words in. This page shows how to fill the public notice proforma for each case, with filled samples and the rules for typing and signing.
For when this notice applies and the full list of papers, read public notice for wrongly written name in Uttarakhand. If you want a new name, you need the other form: see gazette name change proforma format.
What Is the Public Notice Proforma?
A proforma is a set form. The Department of Publication gives a specimen for a public notice in the Gazette of India, Part IV. For a wrongly written name, the text you type on that specimen is what the Gazette prints.
- It is used for a correction: a name that was always the same, written wrongly in a record.
- It is not used for a new name. That has a separate specimen.
- It is free to download from the Department's website.
- It is submitted computer typed, in two copies, with a CD in MS Word.
The form does not ask for a reason, and it does not say anyone has changed a name. It makes one statement: this name was wrongly written here, and the actual name is this.
What Does the Public Notice Specimen Look Like?
This is the layout of the Department's specimen. It holds every option at once, separated by slashes. You keep the options that fit and delete the rest.
It is for general information that I [NAME], son / daughter / wife of Shri / Smt [NAME], residing at [ADDRESS], declare that name of mine / my father / my mother / my minor son / my minor daughter / my wife / my husband has been wrongly written as [WRONG NAME] in my minor daughter's / my minor son's, namely [CHILD'S NAME], aged [AGE] years, in his / her educational documents / service book / driving licence / passport / PAN card / Aadhaar card. The actual name of mine / my father / my mother / my minor son / my minor daughter / my wife / my husband is / are [CORRECT NAME] respectively, which may be amended accordingly.
It is certified that I have complied with other legal requirements in this connection.
(Signature of applicant)
Witness No. 1: signature, full name, address, mobile
Witness No. 2: signature, full name, address, mobile
Read as it stands, the specimen makes no sense. It is a menu. The part about "my minor daughter's / my minor son's, namely ..., aged ... years" is kept only when the wrong record belongs to your minor child. For your own records, that part comes out.
The Department can revise its specimen. Download the current one before you type, and treat the samples below as a guide to choosing words.
Which Words Do You Keep From the Specimen?
| Choice | Options in the specimen | How to choose |
|---|---|---|
| 1. Who you are | Son of, daughter of, wife of | Keep one, with your father's or husband's name |
| 2. Whose name is wrong | Mine, my father, my mother, my minor son, my minor daughter, my wife, my husband | Keep the person whose name the record has wrongly |
| 3. Whose record, and which | Your own, or your minor child's: educational documents, service book, driving licence, passport, PAN card, Aadhaar card | Keep the record that carries the wrong name |
Choice 2 appears twice in the form: once where the wrong name is given, and again where the actual name is given. Keep the same person in both places.
What Do Filled Public Notices Look Like for Each Case?
All names and addresses below are made up. Each sample ends with the certificate line, the signature and the two witnesses, which are left out here to save space.
Your own name, in your educational documents
It is for general information that I, SUNIL KUMAR BISHT, son of Shri DEVENDRA SINGH BISHT, residing at 5, Subhash Nagar, Haldwani, District Nainital, Uttarakhand 263139, declare that name of mine has been wrongly written as SUNIL KUMAR BIST in my educational documents. The actual name of mine is SUNIL KUMAR BISHT, which may be amended accordingly.

Your husband's name, in your Aadhaar card
It is for general information that I, KAVITA NEGI, wife of Shri ANKIT NEGI, residing at 22, Kusumkhera, Haldwani, District Nainital, Uttarakhand 263139, declare that name of my husband has been wrongly written as ANKIT NAGI in my Aadhaar card. The actual name of my husband is ANKIT NEGI, which may be amended accordingly.
Your own name, in your service book
It is for general information that I, HARISH CHANDRA JOSHI, son of Shri GOPAL DUTT JOSHI, residing at 31, Dharanaula, Almora, Uttarakhand 263601, declare that name of mine has been wrongly written as HARISH JOSHI in my service book. The actual name of mine is HARISH CHANDRA JOSHI, which may be amended accordingly.
Before you file for an Aadhaar or a passport error, check that office's own correction route. Many such errors are corrected there with a supporting document and no Gazette.
How Is the Form Filled for a Minor Child's Records?
When the wrong record belongs to your minor child, the parent makes the notice, and the part naming the child and the child's age stays in.
The child's own name, in the child's educational documents
It is for general information that I, DEEPAK JOSHI, son of Shri HARISH CHANDRA JOSHI, residing at Village Kasardevi, District Almora, Uttarakhand 263601, declare that name of my minor daughter has been wrongly written as ANANYA JOSI in my minor daughter's, namely ANANYA JOSHI, aged 9 years, educational documents. The actual name of my minor daughter is ANANYA JOSHI, which may be amended accordingly.
The parent's name, in the child's educational documents
It is for general information that I, DEEPAK JOSHI, son of Shri HARISH CHANDRA JOSHI, residing at Village Kasardevi, District Almora, Uttarakhand 263601, declare that name of mine has been wrongly written as DIPAK JOSHI in my minor daughter's, namely ANANYA JOSHI, aged 9 years, educational documents. The actual name of mine is DEEPAK JOSHI, which may be amended accordingly.
- The parent signs. The child does not.
- Give the child's age in years on the day you type the form.
- Attach an attested copy of the child's record and of the proof of the correct name, such as the birth certificate.
If the error is in the child's birth certificate itself, this form is not the route. The Registrar who issued the certificate corrects it.
Can One Public Notice Cover More Than One Name?
The specimen is worded to allow it. It speaks of "name" for several persons and ends with "is / are ... respectively". So a notice can state, for example, that both the father's and the mother's names were wrongly written in the same record, and give both actual names in the same order.
It is for general information that I, ROHIT BISHT, son of Shri TRILOK SINGH BISHT, residing at Village Kasardevi, District Almora, Uttarakhand 263601, declare that names of my father and my mother have been wrongly written as TIRLOK SINGH BIST and KAMLA DEVI in my educational documents. The actual names of my father and my mother are TRILOK SINGH BISHT and KAMALA BISHT respectively, which may be amended accordingly.
- Keep the wrong names and the actual names in the same order, so "respectively" reads correctly.
- Attach proof for each name.
- Ask the Department, before you pay, if your wording attracts any additional charge.
Where the errors are in different records held by different offices, ask each office what it wants. One notice that names every record may serve, or the Department may ask for separate ones.
What Do You Type in Each Part of the Public Notice?
| Part | What to type | The usual error |
|---|---|---|
| Your name | Your own correct name, as on your identity proof | Typing the wrong spelling here too |
| Son of, daughter of, wife of | One of the three, with the full name | Leaving all three in |
| Address | House, street or village, post office, district, state, pin code | An address that differs from your identity proof |
| Whose name | Mine, my father, my mother, my minor son, my minor daughter, my wife or my husband | A different person in the second half of the form |
| Wrongly written as | The wrong name, copied exactly from the record, error included | "Tidying" the wrong name while typing |
| Child's name and age | Only where the record is your minor child's | Leaving it in for your own record |
| The record | The one that carries the wrong name | Leaving all six records in |
| Actual name | The correct name, exactly as on the trusted proof | A spelling that is on no proof |
| Signature | Yours, by hand | Typed only, on the paper copy |
| Witnesses | Two, each with signature, full name, address and mobile | One witness, or no address |
The "wrongly written as" line trips people most. It feels wrong to type a mistake on purpose. But the notice has to match the record letter for letter, or the office holding that record cannot connect the two.
What Are the Typing and Signing Rules?
Typing
- The form must be computer typed
- Type it on plain paper, complete in all respects
- Make two copies of the same text
- Remove every slash and every option that does not apply
- No pen corrections on the printed page
Signing
- You sign both copies in your name
- Two witnesses sign both copies
- Each witness gives full name, address and mobile number
- For a child's record, the parent signs
- Photographs and the identity copy are self-attested
These rules are from the Department's guidelines for a public notice. The guidelines also say the Controller of Publications may reject printing matter that is incomplete, vague, unlawful or misleading. A form with leftover options reads as vague.
How Do You Prepare the CD and the Certificate?
| Point | Printed form | Soft copy |
|---|---|---|
| Medium | Paper, two copies | A CD, with the file in MS Word |
| Signature | Signed by hand | Your name typed in place of the signature |
| Witness part | Included | Left out |
| Text of the notice | The same, letter for letter | |
With the CD goes a certificate, signed by you, declaring that the contents of the hard copy and the soft copy are similar. The Department gives a format for this certificate on its website.
How Should the Undertaking and the Newspaper Notice Be Worded?
Both must say the same thing as the form. These samples are our own illustrations, with made-up names. They are not official texts.
Undertaking
I, Rohit Bisht, son of Shri Trilok Singh Bisht, residing at Village Kasardevi, Post Office Kasardevi, District Almora, Uttarakhand 263601, undertake that the name of my father has been wrongly written as Tirlok Singh Bist in my educational documents, and that the correct name of my father is Trilok Singh Bisht. I declare that the facts stated above are true and correct to the best of my knowledge and belief.
Place, date and signature
The guidelines ask for an undertaking signed by the applicant, showing the correct and the wrong name with full details of the father's or husband's name and the residential address.
Newspaper notice
It is for general information that I, Rohit Bisht, son of Trilok Singh Bisht, resident of Village Kasardevi, District Almora, Uttarakhand 263601, declare that the name of my father has been wrongly written as Tirlok Singh Bist in my educational documents. The actual name of my father is Trilok Singh Bisht.
The guidelines ask for the matter to be published in one daily local leading newspaper, showing the details of the case, and for the original newspaper to be sent. Do not let the newspaper desk reword it as a change of name.
How Do You Fill the Public Notice Proforma, Step by Step?
Step 1: Download the current specimen
Take the public notice guideline and specimen from the Department of Publication's website.
Step 2: Keep both proofs open
Have the record with the wrong name and the proof with the correct name next to you.
Step 3: Make the three choices
Decide who you are, whose name is wrong, and whose record it is and which one.
Step 4: Type the notice and delete the rest
Type your details, the wrong name exactly as in the record and the actual name exactly as in the proof. Remove every unused option.
Step 5: Read it as a sentence
Read the finished notice aloud. If it does not read as plain English, an option is still left in.
Step 6: Print two copies, sign and get two witnesses
Sign both copies. Each witness adds a signature, full name, address and mobile number.
Step 7: Make the CD and sign the certificate
Save the text in MS Word without the witness part, with your name typed in place of the signature, and sign the certificate that both copies match.
Then put the form with the undertaking, the original newspaper, the photographs, the identity copy, the attested connected document and the request letter with the fee. The applicant sends the file, in person or by post.
Is the Same Format Used for the Uttarakhand Gazette?
Do not assume so. The specimen on this page is for the Gazette of India. The Government Press at Roorkee follows its own practice.
Step 1: Ask the office which Gazette it accepts
The board, employer or office holding the wrong record may want the Gazette of India.
Step 2: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private public notice is being accepted.
Step 3: Ask for the wording and the language
Ask if the press has a set format, and if the notice is to be in Hindi, English or both.
Step 4: Ask about copies, signatures and the soft copy
Note how many copies are wanted, who signs and in what form the soft copy goes.
Step 5: Type the notice to match your proofs
Use the same wrong name and actual name as in your records, and keep a copy of what you submit.
Which Mistakes in the Public Notice Proforma Cause Trouble?
| Mistake | What it causes |
|---|---|
| Unused options and slashes left in | The notice reads as vague and may be rejected |
| The wrong name is not typed as the record shows it | The office holding the record cannot match the notice to it |
| The actual name is on no proof | The correction is refused later |
| The person changes midway, father in one half and mine in the other | The notice states something untrue |
| The child's name and age left in for an adult's own record | A confused notice |
| The change of name specimen used | The Gazette says you changed your name |
| Handwritten, or one copy | The file is returned |
| One witness, or witnesses with no address or mobile | The file is returned |
| The CD carries different words, or the witness part | The file is returned |
| No attested copy of the connected document | The file is incomplete |
A typical case we handle: a father in Rudraprayag typed a public notice for his son's school record and left "my minor son / my minor daughter" and all six records in the sentence, as the specimen shows them. Read aloud, it claimed nothing clear. We retyped it with one person, one record and the two names, and he sent it again. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
The specimen is free, and the form is one paragraph. If your case is one name in one record, you can type it yourself with this page. First make sure you need a Gazette at all, because the office that issued the record can often correct it on its own form. Our charge, where we prepare the file, is quoted per case.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The form is the same in each. Only the address you type in it changes.
