Every paper that goes in the Gazette file, who makes it, what it must show and the small mistakes that send a file back.

Quick answer

The documents required for gazette name change in Uttarakhand are a notarised affidavit, the full original newspaper page, the typed form in two copies, a soft copy of the notice, a signed undertaking, self-attested photo identity proof, two passport size photographs, the BharatKosh fee receipt and a request letter. Supporting papers must not be older than one year.

A Gazette name change file from Uttarakhand laid out with the affidavit, newspaper page, typed form, photographs and BharatKosh receipt
A Central Gazette file has nine papers: affidavit, full newspaper page, typed form in two copies, soft copy, undertaking, identity copy, two photographs, BharatKosh receipt and request letter.

Most Gazette files that come back are returned for a missing or wrong paper, not for the name itself. This page lists the documents required for gazette name change in Uttarakhand, one by one, for adults, children and marriage cases. Use it as a checklist, so the documents required for gazette name change in Uttarakhand are complete before you pay the fee.

This page covers the papers only. For the steps, the fee and the time, read our guide to gazette name change in Uttarakhand. For the address the file goes to, see the gazette office in Uttarakhand page.

Which Documents Are Required for a Gazette Name Change?

This is the Central Gazette file, the one most people from Uttarakhand send. Nine papers, each with a job.

The Department of Publication publishes this list in its change of name guidelines, and it can revise the list. Open the current guidelines on deptpub.gov.in before you post the file. The form itself and the guidelines are free.

Papers are never returned. Whatever you send to the Gazette office stays there. Scan every page before you seal the envelope.

Which Documents Does an Adult Need?

If you are 18 or above and applying for yourself, the nine papers above are the whole file. Here is what each one is.

1. The affidavit

A sworn statement on non-judicial stamp paper with your old name, your new name, your father's or husband's name, your address and the reason. A notary public signs and seals it.

2. The newspaper page

The complete page of the daily newspaper where your notice was printed, with the name of the paper and the date at the top.

3. The prescribed form

The Department's set form for the notice. It is typed, not handwritten, and made in two copies. You sign it in your old name. Two witnesses sign it and write their names and full addresses.

4. The soft copy

The same notice text as a digital file, in the format the current guidelines ask for. The guidelines have long asked for a typed MS Word file on a CD. In the soft copy the old name is typed where the signature would be, and the witness part is left out.

5. The undertaking

A short signed statement that the printed form and the soft copy carry the same words, and that the details you have given are true.

6. Photo identity proof

A photocopy of one photo identity document in your old name, signed by you across the copy.

7. Photographs

Two recent passport size photographs, signed by you.

8. The fee receipt

The printed receipt of the Government fee paid on BharatKosh.

9. The request letter

A short covering letter to the Controller of Publications asking for the notice to be published, with a list of the papers enclosed.

Which Documents Are Needed for a Child Under 18?

A child cannot apply. The parents or the legal guardian apply for the child, and the file grows by a few papers.

Papers about the child

  • The child's birth certificate
  • A school record or bonafide certificate, if the child is in school
  • The child's Aadhaar, if issued
  • Two passport size photographs of the child

Papers from the parents or guardian

  • An affidavit signed by both parents, or by the legal guardian
  • Photo identity and address proof of both parents or the guardian
  • The form, signed by the parent or guardian for the child
  • A custody or guardianship order, where only one parent applies

The newspaper notice, the soft copy, the undertaking, the fee receipt and the request letter are needed as for an adult. The Central fee for a minor is commonly Rs 1,700.

One parent signing alone is the usual problem. If the parents are separated, or one parent has died, add the paper that explains it: the custody order, the decree, or the death certificate. If custody is in dispute, that is a matter for a lawyer and the court first.

Which Extra Documents Do Marriage, Divorce and Other Cases Need?

Add these to the main file. They are supporting proof for the reason you have given.

Extra supporting documents for special name change cases
Your caseAdd this paperNote
Surname change after marriageRegistered marriage certificateFirst check if you need a Gazette at all. The certificate alone is often enough.
Back to a maiden name after divorceCertified copy of the decree of divorceThe affidavit says you are taking back the name used before marriage.
After adoptionRegistered adoption deed or the court orderThe adoptive parents apply for a minor.
Indian living abroadPassport copy and papers attested as the guidelines askThe Central fee is Rs 3,500. Read the guidelines for applicants abroad before you sign anything.
Central Government employeeThe deed under the office procedureA separate route under the Ministry of Home Affairs Office Memorandum of 12 March 1987. Ask your administration section.

A change of religion and a change of gender have their own papers and their own wording. Call us before you prepare anything for those, so the file is made in the right form the first time.

What Must the Name Change Affidavit Show?

Every other paper copies from the affidavit, so it is checked first.

  • Your old name, exactly as on your identity proof
  • Your new name, with the spelling, spacing and initials you want for life
  • Your father's name, or your husband's name
  • Your full address with pin code
  • The reason for the change, in one plain line
  • A line that you will be known by the new name from now on
  • Your signature, and the notary's signature, seal and date

It is typed on non-judicial stamp paper. Notaries sit at every district court in Uttarakhand, so this paper is made in your own district.

Read it aloud, letter by letter, before the notary signs. "Aarav" in the affidavit and "Arav" in the newspaper is enough to send a file back.

What Must the Newspaper Page Show?

  • The name of the newspaper and the date, printed at the top of the page
  • Your notice, with the old name, the new name, your father's or husband's name and your address
  • The same spelling as the affidavit
  • The whole page, not a cutting and not a photocopy

The guidelines for a simple adult case ask for one daily local leading newspaper. We place the notice in two papers, one Hindi and one English, and send both pages. Buy two or three copies of the paper on the day, because you will want one for your own folder.

An online or e-paper printout is not the original page. Send the printed newspaper.

Which Identity Proof and Photographs Are Accepted?

Identity proof

One Government photo identity document in your old name. Any one of these is commonly used:

  • Aadhaar
  • PAN card
  • Voter ID
  • Passport
  • Driving licence

Aadhaar is the usual choice, because it also shows your address. The address on the form should match it.

How to self-attest a copy

  1. Take a clear photocopy. Nothing should be cut off at the edges.
  2. Write "self-attested" on the copy.
  3. Sign below it in your old name, the way you sign today.
  4. Write the date.

Photographs

Two recent passport size photographs with a plain background. Do not staple through the face. Sign them as the guidelines ask.

Witnesses

The witnesses are adults who know you. Each writes a name and full address on the form and signs. Keep a self-attested copy of each witness's identity proof with the file.

What Is the One-Year Rule, and What Else Should You Know?

The one-year rule

Supporting documents must not be older than one year. If your affidavit or newspaper notice was made long ago and never filed, make fresh ones.

Nothing comes back

Documents once submitted are never returned. Send photocopies where a copy is asked for, and keep scans of the originals you must send.

Typed, not handwritten

The form is typed. A handwritten form is a common reason for return.

Sign in the old name

Until the Gazette is printed, your legal name is the old one. Every paper is signed in it.

One spelling everywhere

The affidavit, the newspaper, the form and the soft copy must match letter for letter.

The applicant sends it

The file goes in from you, in person or by post or courier. An agent cannot submit it.

How Do You Put the Central Gazette File Together?

Arrange the papers in this order, so the person who opens the envelope finds everything at once.

Step 1: Put the request letter on top

It lists what is enclosed, so it works as the index of the file.

Step 2: Add the fee receipt

Place the BharatKosh receipt right behind the letter.

Step 3: Add the two copies of the form

Check that both copies are signed by you and by the witnesses.

Step 4: Add the affidavit and the newspaper page

Fold the newspaper page neatly. Do not cut it.

Step 5: Add the undertaking, the identity copies and the photographs

Put the photographs in a small cover so they do not get lost.

Step 6: Add the soft copy

Label it with your old name and your phone number.

Step 7: Compare the spelling one last time

Read the new name in the affidavit, the newspaper, the form and the soft copy. Seal the envelope only when all four match.

Which Documents Does the Uttarakhand Gazette Ask For?

The Government Press at Roorkee sets its own paper list, and it has not published one for private name change notices that we can quote. So the honest answer is to ask first.

Step 1: Get the paper list from the press

Take the contact from govtpress.uk.gov.in and ask which papers are needed and in how many copies.

Step 2: Prepare the two papers every route needs

The notarised affidavit and the newspaper notice are the base of any name change, state or Central.

Step 3: Add identity and address proof

Keep self-attested copies of your photo identity and your Uttarakhand address proof ready.

Step 4: Pay the fee and keep the receipt

Pay the way the press directs and place the original receipt in the file.

Step 5: Keep a full photocopy

Copy every page before you hand in or post the file.

If you have not yet chosen a route, read Uttarakhand Gazette vs Gazette of India first. The papers you make depend on that choice.

Which Papers Do You Not Need?

People are often told to bring these. For a plain name change through the Central Gazette, they are not asked for.

  • A court order. A name change is an office process, not a court case.
  • A lawyer's letter. A notary attests the affidavit. No advocate's certificate is needed.
  • A domicile or residence certificate of Uttarakhand. Any Indian citizen can apply to the Central Gazette.
  • A police verification. None is asked for.
  • Your original Aadhaar, PAN or passport. Send self-attested copies. Never post an original identity document.
  • An income certificate or a caste certificate. They have no part in a Gazette file.
  • A paid "Gazette certificate". There is no such paper. The published Gazette page is the proof, and it is free to download.

And if your case is a spelling mistake in a birth certificate, you need none of the papers on this page. That is a Registrar correction, done by the office that issued the certificate.

A typical case we handle: a mother in Haridwar sent a file for her daughter with the affidavit signed by her alone. The father worked in another city. The file was not accepted until an affidavit signed by both parents was added. This is an illustrative example of the type of case we handle, not a named client review.

What Do You Send Us to Start?

Much less than the full list. From these we prepare the rest.

  • A clear scan of one photo identity proof in your old name
  • An address proof that shows your Uttarakhand address. The same Aadhaar will do
  • Two recent passport size photographs
  • Your new name, typed by you, exactly as you want it
  • For a child: the birth certificate and both parents' identity proof
  • For a marriage or divorce case: the certificate or the decree

We draft the affidavit, book the newspaper notice, type the form and the soft copy, and check the file against the current guidelines. You sign and send it. Our charge is quoted per case and shown apart from the Government fee.

When you do not need us

If you are an adult with a plain case, and you are at ease with typing a form, the free guidelines and this checklist are enough to make the file yourself.

Which Areas of Uttarakhand Do We Cover?

All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The document list is the same in every district. Only the notary and the newspaper change.
