A proforma is a set form. For a name change, it is the form given by the Department of Publication for a notice in Part IV of the Gazette of India. People also call it the specimen, the deed form or the Gazette form.
It is short, usually four or five lines. What makes it matter is this: the Gazette prints these lines as they are. The affidavit and the newspaper notice support your file. The proforma is the notice itself.

The form is the same. Two things change: you write "wife of" with your husband's name, and the old name is the name on your papers before the change.

A child under 18 does not sign. The father, the mother or the legal guardian makes the statement for the child.

The sample is made up. The affidavit behind a minor's form is signed by both parents, or by the guardian. Where only one parent applies, a custody or guardianship order goes in the file. The Central fee for a minor is commonly Rs 1,700.

Address tips for Uttarakhand
- For a village address, write the village, the post office, the tehsil and the district.
- Write "Uttarakhand", not "Uttaranchal", even if an old paper says Uttaranchal.
- If your place was renamed by the Government, use the name that is on your current Aadhaar.
- Check the pin code on the India Post website. One wrong digit puts you in another district.
What Are the Typing and Signing Rules?
Typing

- No cutting, overwriting or correction fluid

Signing
- Sign both copies by hand, in the old name

- Keep a self-attested identity copy of each witness
If you find a mistake after printing, do not correct it with a pen. Fix the file, print again and sign again. A fresh page costs a few rupees. A returned file costs weeks.
How Is the Soft Copy Different From the Printed Proforma?
The Gazette office asks for the notice text as a digital file too, so the press does not have to retype it. The words are the same. Three things differ.

How Do You Fill the Central Gazette Proforma?
Step 1: Download the current specimen
Take the adult or the minor specimen from deptpub.gov.in. It is free.
Step 2: Keep your affidavit open next to it
Copy the names and the address from the affidavit, so the two match.
Step 3: Type the old name, the relation and the address
Keep only the relation that applies: son of, daughter of or wife of.
Step 4: Type the new name
Check the spelling, the spacing and the initials one letter at a time.
Step 5: Print two copies and sign in the old name
Sign both copies by hand.
Step 6: Get two witnesses to sign
Each witness writes a name and a full address and signs both copies.
Step 7: Make the soft copy from the same file
Type the old name in place of the signature, remove the witness part and save the file in the format the guidelines ask for.

The form on this page is the Central one. The Government Press at Roorkee has its own practice, and it has not published a format for private name change notices that we can quote. So ask for it.

The specimen and the guidelines are published by the Department of Publication on deptpub.gov.in. They are free to download. There is one specimen for adults and one for minors.

A wrong spelling that gets printed is the costly one. A Gazette cannot be unpublished. The fix is a corrigendum, applied for within six months, and you pay the fee again if the error came from your own form.
