Name Change in Ayushman Card in Uttarakhand

When the name on your Ayushman or golden card does not match Aadhaar, and a hospital's desk cannot verify you for cashless treatment: where the name comes from, how the identity check is done again, and what families, new brides and pensioners should correct first.

Quick answer

A name change in Ayushman card records is made by doing the identity check again against Aadhaar, after the wrong record is corrected. Correct Aadhaar at an Aadhaar centre, or the family's ration card at the supply office, then repeat the check on the app or at a hospital help desk. No Gazette is needed for a spelling.

A family in Uttarakhand at a hospital help desk comparing the name on an Ayushman golden card with Aadhaar and the ration card
The Ayushman card name follows Aadhaar, so correct Aadhaar or the family list first and then repeat the identity check for a fresh card.

The card is shown at a hospital's desk on the day of admission, and that is usually when a wrong name is found. A name change in Ayushman card records is not an edit anyone types in. It follows Aadhaar. This page explains name change in Ayushman card records for families in Uttarakhand, under all three of the state's health cover schemes.

We do not make or correct these cards, and this subject almost never needs a Gazette. The schemes are as the state health authority's site describes them, and the identity check as the national authority's published answers describe it. We did not test the app. For the health record account, which is a different thing, read name change in ABHA health ID.

Which Schemes Issue the Card in Uttarakhand?

From the state health authority's own site. All three are run by that authority.

The three health cover schemes run by the state health authority of Uttarakhand
SchemeWho it is forWhat the family list is read from
The central scheme (Ayushman Bharat, PM-JAY)Families on the national listThe national list of entitled families
The state's scheme (Atal Ayushman Uttarakhand Yojana)The state's remaining families. The authority's site says it uses the same computer system and packages as the central schemeThe state's own family data. Guides say the ration card is the usual basis
The employees' scheme (State Government Health Scheme)State Government employees, pensioners and their dependantsThe employee's or pensioner's own record

People call all three cards "Ayushman card" or "golden card". The cure for a wrong name depends on which list your family was read from. So find that out first.

Where Does the Name on the Card Come From?

  1. A source list. The scheme holds a record of the family, with each member's name as that list had it.
  2. The search. The member is found in the list, on the app or the beneficiary portal, or by an operator at a centre or a hospital.
  3. The identity check. The member verifies with Aadhaar, by a one-time code, a fingerprint, an iris scan or a face scan.
  4. The comparison. The name and other details from Aadhaar are compared with the list, and a match score is given.
  5. The card. Issued on approval, and downloaded or printed.

So two names meet at the fourth step: the one a list wrote years ago, and the one on Aadhaar today. The card is made from the meeting of the two. Nobody at a counter types the name freely.

In Which Three Places Can a Name Differ?

The source list against Aadhaar

  • The list has a home name, an old spelling or a maiden name
  • The identity check gives a low match and is sent for approval
  • Cured by approval, or by correcting the list

The card against today's Aadhaar

  • The card was made years ago, and Aadhaar has been corrected or changed since
  • The hospital's desk sees two names
  • Cured by doing the identity check again

Aadhaar itself

  • Aadhaar is the one that is wrong, and the list was right
  • Every scheme now copies the wrong name
  • Cured at an Aadhaar centre, and then the check is repeated

Lay the card, Aadhaar and the ration card side by side. Two of the three usually agree, and the third is the one to correct.

How Do You Put It Right?

This is the general method. Screens and menus change, so follow what the app or the help desk shows now.

Step 1: Find out which scheme your card is under

The central scheme, the state's scheme for all families, or the scheme for Government employees and pensioners.

Step 2: Open your family's record on the app or the beneficiary portal

Or ask the help desk at an empanelled hospital to show it. Note the name the record holds.

Step 3: Put Aadhaar beside it

And the ration card, or the service record for an employee's card.

Step 4: Correct Aadhaar first, if it is the odd one

At an Aadhaar centre, with the mobile number linked.

Step 5: Correct the source list, if that is the odd one

The ration card at the supply office, or the dependants' details through the employee's office.

Step 6: Do the identity check again

On the app, the portal, at a common service centre or at a hospital help desk.

Step 7: Wait for approval where the check is sent up

A low match is not a refusal. It is looked at by the state's health authority or its agency.

Step 8: Download the fresh card and read it

Then show it at the hospital with Aadhaar.

What Is the Identity Check, and the Match Score?

From the national health authority's published answers for its helpline.

  • The authority has an app for making the card, and a beneficiary portal. A member can log in as a beneficiary, or an operator can do it for the member.
  • The member authenticates with Aadhaar and a mobile number.
  • The check gives a match score. The answers say that above 80 per cent the request is approved automatically and the card can be downloaded.
  • Below that, the request is sent to the state's health authority or its support agency, which looks at it and decides.
  • The answers say a beneficiary need not pay anything for the card.

A low score is the system being careful, not a refusal. "Kamla Devi" in a list and "Kamala Bisht" on Aadhaar is one woman to her village and two to a machine. An officer who sees her ration card and her marriage certificate can approve what the machine could not.

What About Brides, Babies and the Family List?

  • A new bride is on her parents' family list, in her maiden name, and not on her husband's. She is added to her husband's family through the record the scheme reads, usually the ration card.
  • Add her in the name on her Aadhaar that day. If she takes a married surname later, Aadhaar changes first and the check is repeated.
  • A newborn has no Aadhaar at first. Ask the help desk how a child is added. The birth certificate and the family's ration card are the usual papers.
  • A child's name written one way at birth and another at school should be settled before Aadhaar is made, since the card will copy Aadhaar.
  • A member who has died stays on the list until someone reports it. That does not harm the others' cover.
  • A family that has split into two households may still be one family on an old ration card.

Read name change in ration card, name change after marriage and name change for a newborn.

What About Employees, Pensioners and Dependants?

  • The state health authority's site says this scheme covers State Government employees and pensioners, with those of public undertakings, autonomous and local bodies, aided institutions and universities.
  • Guides say it has run since January 2021, on a monthly contribution, and covers dependants.
  • The member's name comes from the service or pension record. A dependant's name comes from what the employee declared.
  • So a wife entered in her maiden name in a service book thirty years ago appears that way on her golden card, whatever her Aadhaar says.
  • The cure starts at the employee's own office, or the treasury for a pensioner, where the dependants' details are corrected. The card is then made again.
  • A family pensioner, after the employee's death, should see that her own name on the pension order matches her Aadhaar before asking for a card.

Read name change for government employees and name change in pension and family pension records.

What About the Card for People Aged 70 and Above?

  • The central scheme was widened to give a separate card to everyone aged 70 and above, whatever the family's income.
  • Guides say enrolment for it rests on Aadhaar alone, and that the age is read from Aadhaar.
  • So for an old person the name and the date of birth on Aadhaar decide everything. An Aadhaar that carries only a year of birth, or a wrong one, can put a person on the wrong side of 70.
  • Old people in the hills often have a home name on older papers and no birth certificate. Correct Aadhaar on the best paper there is, and do it before illness makes it urgent.
  • Fingerprints that no longer read are a common block. Ask the centre about a face or an iris scan.

We did not confirm how this card sits with the state's own schemes. Ask the help desk. Read date of birth correction.

What Happens at the Hospital's Desk?

  1. The desk. An empanelled hospital has a help desk for the scheme. Its staff verify the patient before a cashless admission is approved.
  2. What they check. The card, and the patient's identity against Aadhaar, usually by fingerprint or a code.
  3. Where a name differs. The desk can often redo the identity check there, or raise the mismatch for approval.
  4. In an emergency. Treatment should not wait for a card. Ask the desk what it needs, and bring Aadhaar and the ration card as soon as someone can.
  5. The hospital's own file. Give Aadhaar at admission, so that the discharge papers carry the same name as the card and the claim.

The best time to find a wrong name is on a quiet day at home, not in a corridor. Download every family member's card once and read it. Read hospital records name correction and health insurance policy name correction.

What If the Record Is Not Found?

  • The national authority's answers say "no beneficiary found" means the Aadhaar entered is not in the scheme's database. That is not, in itself, a name mismatch.
  • Search again by another route the app offers, such as the family or ration card details, and under the right scheme.
  • A member who is in the list under a badly different name may simply not be recognised. The help desk can look through the family's record line by line.
  • A family that has no ration card, or whose ration card is in another state, should ask the help desk how the state's scheme takes it on.
  • A complaint that stays unanswered goes to the scheme's helpline and its grievance service.

Do not pay anyone who offers to "add your name" for a fee. The card is free, and the list is corrected at its own office.

Where Does a Gazette Come In?

Almost never for this card. It comes in where you have changed your name by choice and Aadhaar needs a Gazette for that. The card then simply follows the new Aadhaar.

Step 1: Confirm that it is a change of name, and not a spelling

A spelling is corrected at the Aadhaar centre or in the source list.

Step 2: Do not wait for it if treatment is near

A Gazette commonly takes 3 to 7 weeks. Use the papers you have for the hospital.

Step 3: Fix the exact new name

The spelling that will go on Aadhaar, the ration card and the card.

Step 4: Make the affidavit and publish the newspaper notice

Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.

Step 5: Type the form and sign the undertaking

Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.

Step 6: Prepare the CD, photographs and certificate

The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.

Step 7: Pay the charge on BharatKosh

Commonly Rs 1,100 for an adult. It is not refunded.

Step 8: Send the file, then update Aadhaar and repeat the identity check

Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054.

See gazette name change in Uttarakhand.

Does the Uttarakhand Gazette Matter Here?

Not to the scheme, which follows Aadhaar. The question is only which Gazette Aadhaar accepts.

Step 1: Ask the Aadhaar centre which Gazette it accepts

The card follows Aadhaar.

Step 2: Think about the ration card, the bank and PAN

The Gazette of India serves them all.

Step 3: Contact the Roorkee press

Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.

Step 4: Ask for the papers, the format and the fee, and submit

Keep photocopies of everything.

Step 5: Download the notice

Search gazettes.uk.gov.in and save the PDF.

To weigh the two, read Uttarakhand Gazette vs Gazette of India.

Which Mistakes Do Families Make?

Common mistakes with a name on an Ayushman card
MistakeWhat to do
Reading the card for the first time at admissionDownload and read each member's card now
Asking someone to "edit" the name on the cardThe card follows Aadhaar. Repeat the identity check
Repeating the check before correcting AadhaarAadhaar first, where it is the wrong one
Taking a pending approval for a refusalFollow it up with the help desk
Leaving a bride off the ration cardAdd her, in her Aadhaar name
Confusing the card with ABHAThey are separate records
Paying for the card, or to be "added"The card is free
Paying for a Gazette to fix a spellingAadhaar and the list are corrected at their own offices
A pensioner asking the hospital to correct a dependant's nameThe pensioner's own record comes first

A typical case we handle: a woman from a village near Almora was to be admitted in Haldwani for an operation. Her card, made years earlier from the ration card, read "Kamla Devi". Her Aadhaar had since been updated to "Kamala Bisht". Her son rang us from the hospital asking for a Gazette. None was needed. The help desk did the identity check again with her Aadhaar, the request went for approval with her ration card and marriage certificate, and the fresh card came in her Aadhaar name. This is an illustrative example of the type of case we handle, not a named client review.

When you do not need us

For this card, almost always. The Aadhaar centre, the supply office, the employee's office and the hospital's help desk each do their own part. We are useful only where a person has changed a name by choice and Aadhaar needs a Gazette first.

Which Areas of Uttarakhand Do We Cover?

All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The card is made and corrected on the scheme's own system. For the papers behind it, send photos on WhatsApp and we will tell you which one to correct.

People Also Ask About a Name on the Ayushman Card

Can I edit the name on the Ayushman card myself?

No. The name follows Aadhaar and the scheme's list.

The match between the list and Aadhaar was low, so an officer has to look.

Is a gazette needed to change the name on an Ayushman card?

No. Only where the holder has changed a name by choice.

Is there a fee for the Ayushman card?

No. The national authority says the card is free.

Is the golden card of Government employees the same?

Guides say only Aadhaar, with Aadhaar-based eKYC. Keep the name and date of birth on Aadhaar up to date, since the card is made from it.

Where do I apply for the card for people aged 70 and above?

Through the Ayushman App or the beneficiary portal at beneficiary.nha.gov.in. Help is also given at common service centres and hospital help desks.

What is the Ayushman card helpline number?

Who Wrote and Reviewed This Page?

This page is written by the Documentation Desk Team of UttarakhandNameChange.com, the team that prepares Gazette files for people across Uttarakhand.

It is reviewed by Advocate Vipin Chauhan.

The three schemes and who they cover are from the state health authority's own site. The identity check, the match score, the meaning of "no beneficiary found" and the fee are from the national health authority's published answers for its helpline. That the state's scheme reads families from the ration card, the start of the employees' scheme and the card for people aged 70 and above are as guides describe them. We did not test the app or the portal, and their screens change. What to correct first is general guidance. The scheme's help desk and the state health authority are the authority.

The card's name comes from a family list, checked against Aadhaar. Nobody edits it by hand.
