Name Change in Pension and Family Pension Records in Uttarakhand
When the name on a pension payment order does not match Aadhaar or the bank, or a spouse's name is wrong for family pension: who corrects it, on what papers, and why it should be done in the pensioner's lifetime.
A name change in pension and family pension records is made by the office that sanctioned the pension, which issues a corrected pension payment order. The order is drawn from the service record. The pension department has said there is no separate procedure, and that the 1987 procedure for employees may be followed for a family pensioner too.

A name change in pension and family pension records is most often a wife's name. It was written one way in a service book forty years ago, and reads another way on her Aadhaar today, and nobody notices until she has to claim. This page explains name change in pension and family pension records for pensioners in Uttarakhand, civil and defence.
We do not deal with pension offices. We prepare the Gazette notification and the papers that go with it, which pension offices ask for where a name has in fact changed. For serving employees, read name change for government employees.
What Is the Pension Order, and Where Do Its Names Come From?
A pension payment order is the document that authorises a pension. It names the pensioner, and it names the spouse who will receive family pension after the pensioner, usually with a joint photograph.
- It is issued by the pension sanctioning authority and sent to the bank or treasury that pays.
- The names on it are taken from the service record, and from the family details the employee gave before retiring.
- The pension department has put it plainly: the pension order is issued on the basis of the service book.
- The paying bank holds a copy and pays strictly on it.
That is the root of most trouble. A service book opened decades ago recorded a wife as she was called at home, or by her maiden name, or with a spelling a clerk chose. The pension order copies it. Her Aadhaar and bank account, made much later, say something else.
What Has the Pension Department Said?
The Department of Pension and Pensioners' Welfare has issued a clarification on change of name in a pension order, which other departments have circulated. These are its points.
| Point | What the clarification says |
|---|---|
| A separate procedure | None is prescribed in the central pension rules for change of name or surname of an employee or spouse in the pension order after retirement |
| The basis of the order | The pension order is issued on the basis of the service record, and the service book is a matter for the personnel department |
| What may be followed | The personnel department's memorandum of 12 March 1987 on change of name may be followed for a family pensioner also |
| A discrepancy in the papers | The ministry concerned may sort it out directly with the family pensioner, ensuring the request meets that memorandum's conditions |
The 1987 memorandum is the one that asks a serving employee for a deed, a notice in a prominent local newspaper and a notice in the Gazette of India for a change of name. It has a lighter route for a woman's surname on marriage. So for central civil pensioners, the same papers come back at the pension stage.
See name change for government employees for that procedure in full.
Who Corrects a Pension Record?
| Pensioner | Where the request goes | What comes back |
|---|---|---|
| Central civil pensioner | The head of the office from which the pensioner retired, then its pay and accounts office | An amended or corrigendum pension order |
| Defence pensioner | The record office of the service, usually through the district's soldier welfare office | A corrigendum pension order from the defence pension authority |
| State Government pensioner | The department last served, and the treasury that pays | A corrected order under the state's procedure |
| Railway, postal and telecom pensioners | The division or circle that sanctioned the pension | A revised order |
| Provident fund pensioner | The provident fund organisation's regional office | A corrected pension record |
| Bank and public sector pensioners | The employer's pension cell | As its rules provide |
The bank that pays the pension cannot change a name on the pension order. It pays on what it is sent. Go to the office that sanctioned the pension, and the bank will follow the corrected order.
Which Case Is Yours?
| The situation | Example | Route | Gazette? |
|---|---|---|---|
| A spelling slip in the pensioner's own name | Order: Trilok Sing. Aadhaar: Trilok Singh | A request to the sanctioning office with identity proof | Usually no |
| The spouse's name is a home name or a short form | Order: Guddi Devi. Aadhaar: Kamla Devi | A request with her identity proof and a declaration that both are one person | Ask the office |
| The spouse's surname differs | Order: Kamla. Aadhaar: Kamla Rawat | A request with her identity proof and the marriage certificate | Usually no |
| The spouse's name is not on the order at all | Married after the service book was opened | A request to add her, with proof of marriage | No |
| The pensioner has changed a name by choice | A new name after retirement | The 1987 procedure: deed or affidavit, newspaper and Gazette | Yes |
| The family pensioner's name has changed | A widow who has changed her name | The same procedure, as the clarification says | Yes |
| The pensioner has died and the spouse's name is wrong | Found when she claims | Her own application, with heavier proof | Often asked for |
Each office decides what it needs. Where the difference is a clerk's spelling of forty years ago, many accept identity papers and a declaration. Where the two names are quite different, expect to be asked for more.
How Do You Apply?
Step 1: Take out the pension order and read every name
The pensioner's name, the spouse's name, the dates of birth and the joint photograph.
Step 2: Compare each with Aadhaar, PAN and the bank passbook
For the pensioner and for the spouse. Write down every difference.
Step 3: Find the sanctioning office
It is named on the pension order. For a defence pensioner, the record office.
Step 4: Ask that office for its form and its list of papers
In writing. Offices differ, and the list depends on how different the two names are.
Step 5: Apply in the pensioner's own hand, while the pensioner can
State the name as recorded and as it should read, and ask for a corrected pension order.
Step 6: Attach the proof
Identity papers of the person whose name is to be corrected, the marriage certificate, a declaration that both names are one person, and for a changed name the Gazette notification and newspaper notice.
Step 7: Keep a dated receiving, and follow up
Pension offices are slow. Note the reference number and write again after a reasonable time.
Step 8: Give the corrected order to the bank
Check that the bank's pension record, and the joint account if there is one, now carry the same names.
Why Does the Spouse's Name Matter So Much?
While the pensioner lives, a wrong spelling of the spouse's name harms nobody. The day the pensioner dies, it decides how soon she is paid.
- Family pension is started by the bank on the pension order, for the spouse named in it.
- The bank compares that name with her Aadhaar, her PAN and her bank account. Where they differ, it holds the payment and sends her to the sanctioning office.
- She is then newly widowed, often elderly, sometimes unable to travel, and has to prove to an office that the woman named in a forty-year-old service book is herself.
- The one person who could have settled it with a signature is no longer there.
Before you apply, settle which form of her name is right, and bring her own papers to it. See name change after marriage in Uttarakhand and short name and full name difference in documents.
What Is the Route for Defence Pensioners?
Uttarakhand is home to many ex-servicemen and their families, and this is the group most affected. A soldier's wife was often recorded at marriage by a home name, in a unit far from home.
- The family details were recorded through the unit's orders while the soldier served, and the pension order was drawn from them.
- After retirement, the request goes to the record office of the regiment or corps, usually through the district's soldier welfare office, which helps with the application.
- The record office amends the service documents, and the defence pension authority then issues a corrigendum pension order.
- Guides for veterans commonly list a personal application, the pension order, and the Aadhaar and PAN of both spouses. For larger differences they report affidavits and newspaper notices being asked for.
- Guides also report that the defence ministry has moved to a uniform procedure for such corrections. Ask the record office or the soldier welfare office for the current list.
A serving soldier can have a spouse's name corrected far more simply, through the unit. If a family member is still in service, tell him to check his family details now.
The health scheme card and the canteen card are issued on the same service details, so they follow the corrected record.
What If the Pensioner Has Already Died?
The spouse can still apply, and offices do correct such records. It takes longer and needs more.
- Collect what links her to the name on the order. The marriage certificate or old proof of marriage, the joint photograph on the pension order, old ration cards, the children's certificates naming both parents, and any paper from the service years.
- Apply to the sanctioning office, or the record office for a defence family, with the death certificate and her identity papers.
- Give a sworn declaration that the woman named in the pension order and the woman named in the identity papers are one and the same.
- Be ready for more. Where the names are far apart, an office may ask for newspaper notices or a Gazette notification, in line with the 1987 procedure.
- Ask for help. The district's soldier welfare office for a defence family, and the pensioners' grievance portal for a central civil pensioner.
Check the death certificate as well. If it gives the pensioner's name differently from the pension order, that is a second mismatch. See name correction in a death certificate and legal heir certificate.
What About the Bank and the Life Certificate?
- The pension account. Its name should match the pension order. A joint account with the spouse makes the move to family pension smoother, where the scheme allows one. Ask the bank.
- The spouse's own identity papers. Aadhaar, PAN and her account should carry one name, the same as the corrected pension order.
- The yearly life certificate. The digital life certificate is given through Aadhaar. A pensioner whose pension record and Aadhaar differ can find it failing, which stops the pension until it is sorted out.
- Income tax. Pension is reported against PAN. A name on PAN that differs from the pension record causes its own queries.
The order of work is Aadhaar and PAN first, then the pension order, then the bank. See name change in Aadhaar card and name change in PAN card.
What About State Government Pensioners?
- A pensioner of the Government of Uttarakhand is paid through the state's treasuries, under the state's own pension rules.
- The request goes to the department last served, or to the treasury that pays, which will say where a correction is made.
- We could not confirm the state's own procedure for a change of name in a pension order. Ask the treasury officer in writing.
- The papers are likely to be the same in kind: identity proof, the marriage certificate, a declaration, and a Gazette notification for a changed name.
A family pensioner should carry the pension order, the death certificate and her identity papers to the treasury, and ask for the list before preparing anything.
How Does the Central Gazette Route Work?
Use this where a pensioner or a family pensioner has changed a name, or where the sanctioning office asks for a Gazette notification. For central and defence pensioners, the Gazette of India is the one named in the 1987 procedure.
Step 1: Get the office's requirement in writing
A declaration and identity papers may be all it asks for.
Step 2: Fix the name to be carried, and list the other form as on the pension order
Letter by letter.
Step 3: Make the affidavit
State the name as in the pension order, the name as it should be, the husband's or father's name and the address. Sign before a notary. Ask us how an applicant who cannot sign completes the papers.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper, and spares for the office.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult, with Rs 250 more for each alias. It is not refunded.
Step 8: Send the file, then apply to the pension office
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Give the published Gazette to the sanctioning office with the newspaper and the application.
We prepare this file on WhatsApp and by courier, so an elderly pensioner or a widow does not have to travel.
Would the Uttarakhand Gazette Do?
For a central or defence pension, choose the Gazette of India. For a state pension, ask the treasury which it accepts.
Step 1: Ask the sanctioning office or the treasury which Gazette it accepts
In writing.
Step 2: Think about Aadhaar, PAN and the bank
The Gazette of India serves them all.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.
To weigh the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Do Pensioners and Families Make?
| Mistake | What to do |
|---|---|
| Never reading the spouse's name on the pension order | Read it today, beside her Aadhaar |
| Leaving the correction for the family to do later | Apply in the pensioner's lifetime |
| Asking the bank to change the name | Apply to the sanctioning office |
| Changing the spouse's Aadhaar to match a wrong pension order | Decide the right name first, then bring both to it |
| Paying for a Gazette before asking the office | Get its requirement in writing |
| A serving soldier ignoring his family details | Correct them through the unit, while it is easy |
| Paying an agent near the pension office | The welfare office and the pension office help free |
| Throwing away old family papers | Keep every paper that names both spouses |
| Correcting the order and not telling the bank | Give the bank the corrected order |
A typical case we handle: a retired havildar in Pauri found that his pension order gave his wife's name as "Guddi Devi", the name used at home when he married, while her Aadhaar, PAN and bank account said "Kamla Devi". He applied through the district's soldier welfare office. The record office asked for newspaper notices and a public paper linking the two names. We prepared a Gazette notice stating both, joined by alias, and he submitted it. A corrigendum pension order came in her right name. This is an illustrative example of the type of case we handle, not a named client review.
What is a lawyer's job, not ours
A family pension refused, a dispute between claimants, and a pension office that will not act on complete papers.
When you do not need us
For a spelling slip, or a surname the marriage certificate explains, you need the sanctioning office and the identity papers. We are useful where the office asks for a Gazette notification, which it commonly does when two names are quite different or a name has been changed.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. Pensioners in the hill districts need not travel to us. We work on WhatsApp and by courier, and a family member can send the papers.
People Also Ask About Names in Pension Records
How do I correct my wife's name in my pension order?
Apply to the office that sanctioned the pension, with her identity papers and proof of marriage.
Can the bank correct a name in the PPO?
No. The bank pays on the order. The sanctioning office corrects it.
Is a gazette needed to change a name in pension records?
For a changed name, the 1987 procedure applies. For a spelling, identity papers usually serve.
Can a widow correct her name after her husband's death?
Yes, on her own application, with heavier proof.
How do defence pensioners correct a spouse's name?
Through the record office, usually via the district's soldier welfare office.
What is a corrigendum PPO?
An amended pension order issued after a correction.
When must a pensioner give the life certificate?
Every year in November, for the pension to continue. Pensioners aged 80 and above were allowed to give it from October in 2025. If the name in the pension record and on Aadhaar differ, fix it before November.
Can a pensioner give the life certificate by face?
Yes. Aadhaar-based face authentication lets a pensioner submit the digital life certificate without a fingerprint device. It checks the person against Aadhaar, so the pension record should carry the same name.
What is the Digital Life Certificate campaign?
A nationwide drive run by the Department of Pension and Pensioners' Welfare every year from 1 to 30 November. Campaign 4.0 in 2025 aimed for two crore digital life certificates.
Where can a pensioner get help with the life certificate?
The 2025 campaign was run with 19 pension disbursing banks and the Ministry of Electronics and Information Technology, with camps across districts. Banks and India Post Payments Bank help pensioners on the spot.
Why does a name mismatch matter for the digital life certificate?
The digital life certificate is verified through Aadhaar, by fingerprint or face. A pension record in one name and Aadhaar in another is a common reason for trouble, so correct the name with the sanctioning office first.
Does the life certificate campaign cover provident fund pensioners?
The 2025 campaign guidelines were sent to the Central Provident Fund Commissioner of EPFO among others, so provident fund pensioners can use the same campaign. Their name is corrected with the EPFO regional office.

The clarification that there is no separate procedure, that the pension order rests on the service record and that the 1987 memorandum may be followed for a family pensioner is from the pension department's advice as circulated by another department. The route and papers for defence pensioners are as veterans' guides report them. We could not confirm the state's procedure. Each sanctioning office decides what it needs.

Pension and Family Pension Records: The Short Version
A pension order takes its names from the service record, and only the office that sanctioned the pension can amend it. The pension department has said there is no separate procedure, and that the 1987 procedure for employees may be followed for a family pensioner. A spelling usually rests on identity papers. A changed name needs the newspaper and the Gazette.
