Health Insurance Policy Name Correction in Uttarakhand
When the name of an insured person on a health policy or its card does not match the identity proof shown at a hospital desk: how the insurer corrects it, what each kind of policy needs, and what to do if it is found on the day of admission.
A health insurance policy name correction is made by the insurer, as an endorsement on the policy, at the policyholder's request with identity proof. A spelling needs only that proof. A married name needs the marriage certificate, and a change by choice a Gazette notification. Do it while everyone is well, not at the admission desk.

A health policy is bought once and renewed by habit, and few people read the names on it until someone is ill. A health insurance policy name correction asked for from a hospital corridor is slow and anxious. This page explains health insurance policy name correction for families in Uttarakhand, and how to get it done in a quiet week.
We do not sell insurance or deal with insurers. We prepare the Gazette notification, where an insured person has changed a name by choice. This page is general guidance, and we did not read any insurer's own procedure. For life insurance, read name change in LIC policy. For hospital papers, read hospital discharge summary and medical records name correction.
Which Names Does a Health Policy Carry?
| Name | Who that is | Where it comes from |
|---|---|---|
| The proposer or policyholder | The person who bought the policy and pays for it | The proposal form and that person's identity proof |
| Each insured member | The people covered: self, spouse, children, parents | What the proposer typed for each, with age and relationship |
| The nominee | The person to be paid if the policyholder dies with a claim due | What the proposer typed |
| The name on each health card | The insured member the card is for | The policy record |
- The policy schedule is the page that lists these. It is the document to read.
- The proposer's own identity is verified when the policy is bought. The other members' names are usually taken on the proposer's word.
- That is why the proposer's name is seldom wrong, and a wife's, a child's or a parent's often is.
- A date of birth matters as much as a name here, since the premium and the cover turn on age.
Where Is the Name on a Health Policy Read?
- At the hospital's insurance desk, on admission. The health card is read against the patient's photo identity proof.
- By the insurer or its claims administrator, when the hospital asks for approval of cashless treatment.
- On the discharge papers and bills, which the hospital makes in the name it registered the patient under.
- At a reimbursement claim, where the bills, the policy and the bank account are read together.
- At renewal, at a move to another insurer, and when cover is increased.
- Against the insurer's own identity record of the policyholder, which the regulator requires it to keep.
So three papers meet at a claim: the policy, the patient's identity proof and the hospital's record. All three have to describe one person.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| A member's name is misspelt on the policy | An endorsement, on that member's identity proof | No |
| A wife is entered in her maiden name and her Aadhaar now has a married one | An endorsement, on the marriage certificate and her Aadhaar | No |
| A child was entered as "baby of" the mother, and now has a name | An endorsement, on the birth certificate | No |
| A parent's name on the policy differs from an old identity card | Decide which is right, correct the other | Rarely |
| A member has changed a name by choice | An endorsement, on the Gazette notification and updated identity proof | Yes |
| The policy is right, and the hospital registered the patient wrongly | Ask the hospital to correct its record before discharge | No |
| The policy is an employer's group policy | The employer's personnel office asks the insurer | As the reason requires |
Most rows are a matter of one email and one attachment. The trouble is only ever in the timing.
How Do You Ask for a Correction?
This is the general method. Each insurer has its own form and channel.
Step 1: Read the policy schedule, member by member
Name, date of birth, gender and relationship, for everyone covered.
Step 2: Put each member's Aadhaar beside it
Mark every difference, however small.
Step 3: Decide which paper is right
Where Aadhaar is the wrong one, correct Aadhaar first.
Step 4: Write to the insurer for an endorsement
Through its customer portal, its app, a branch or your agent. Give the policy number and the member concerned.
Step 5: Attach the proof
The member's identity proof, and the marriage certificate, birth certificate or Gazette notification as the case needs.
Step 6: Ask for a written endorsement and fresh health cards
An endorsement is a dated page that becomes part of the policy.
Step 7: Check the corrected schedule and each card
And the insurer's app, which hospitals sometimes read.
Step 8: Check again at renewal
A correction occasionally fails to carry into the renewed policy.
What Proof Goes With Each Kind of Change?
A correction
- The entry never matched the person's papers
- Proof: the member's Aadhaar or other photo identity
- For a date of birth: the birth certificate or class 10 certificate
- Insurers treat a spelling as a small matter. A date of birth may change the premium
A change of name
- The entry was right, and the person now has another name
- For marriage: the marriage certificate, with identity proof in the new name
- For a change by choice: the Gazette notification, with identity proof in the new name
- For a child named after birth: the birth certificate
An insurer may ask for an affidavit as well. It will not usually accept one in place of the papers above for a real change of name. Ask for its list in writing.
What About Family Policies, Newborns and Parents?
- A family policy covers several people under one sum. Each member is named separately, and each name is corrected on that member's own papers.
- A newborn is often added in the first weeks, before a name is chosen. Send the birth certificate with the name as soon as it is issued.
- A wife added after marriage should be entered in the name on her Aadhaar as it stands that day. If she changes it later, send the certificate then.
- Elderly parents are the commonest trouble. A father entered from memory as "R. S. Negi" whose Aadhaar reads "Ram Singh" is two people to a hospital desk.
- A parent whose own papers disagree with each other has to settle that first. The policy can match only one of them.
- The nominee should be named as on the nominee's own identity proof.
Read name change in birth certificate and name change in Aadhaar card.
What About an Employer's Group Policy?
- The employer is the policyholder. Employees and their dependants are members, on a list the employer sends the insurer.
- A member cannot usually ask the insurer directly. The request goes through the employer's personnel office or its insurance portal.
- The list is built from the employer's own records. Where your name is wrong there, the policy copies it. Correct the employer's record first.
- Dependants are entered by the employee, often in a hurry in a short enrolment window. Read what you typed.
- On leaving the job, the cover ends. A correction then is of use only for a claim already made.
Read name change in EPFO and UAN records, for the employer's other records.
What If It Is Found at the Admission Desk?
- Treatment comes first. Admit the patient. A cashless approval can follow, and a claim can always be made by reimbursement.
- Register the patient at the hospital in the name on the identity proof, not a short form.
- Tell the insurance desk at once what differs between the card and the identity proof.
- Call the insurer's helpline and ask for an urgent endorsement, sending the proof from your phone.
- Give the desk whatever links the two names: the marriage certificate, an old identity card, the policy schedule showing the relationship and date of birth.
- If cashless is refused, pay and claim. Keep every original bill, the discharge summary and the refusal in writing.
A small spelling difference is often passed where the date of birth, the relationship and the photograph all fit. A different surname is harder. Nobody can promise the outcome on the day, which is the whole case for doing it earlier.
What About Reimbursement Claims and the Bank Account?
- A reimbursement is paid to the policyholder's bank account. The account's name is matched with the policy.
- The hospital's bills and discharge summary have to be in the patient's name as the policy has it. Ask the hospital to correct its papers before you leave.
- Where the patient is a member and the policyholder someone else, the claim form names both. Fill each as on that person's identity proof.
- Where the policyholder has died with a claim pending, the nominee claims, and the nominee's name is read against the nomination.
- Send the correction request and the claim separately, and quote one in the other.
Read name change in bank records and nominee name mismatch in a claim.
What If the Insurer Does Not Act?
- Write to the insurer's grievance officer, with the policy number, your request and its date.
- Keep the complaint number and the reply.
- Use the insurance regulator's complaint portal if the insurer does not answer in the time it is given.
- Go to the Insurance Ombudsman for a claim refused on a name difference, within the time and the money limits that office works to.
- A consumer commission is the further remedy. See a lawyer for that.
A claim refused only because of a spelling, where identity is otherwise plain, is the kind of refusal these bodies exist to look at. Put the identity papers, the policy and the hospital record together in one set.
How Does the Central Gazette Route Work?
Use this where an insured person has changed a name by choice. It does nothing for a spelling, a married name or a child named after birth.
Step 1: Confirm that it is a change of name
A spelling is corrected on identity proof.
Step 2: Fix the exact new name
The spelling that will go on Aadhaar and the policy.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary. For a minor, the parents apply.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult and Rs 1,700 for a minor. It is not refunded.
Step 8: Send the file yourself, then update Aadhaar and ask for the endorsement
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Give the insurer the published notification with the updated identity proof.
Keep a copy of the notification with the policy. Old hospital records and old claims stay in the earlier name, and it links them. See gazette name change in Uttarakhand.
Would an Insurer Accept the Uttarakhand Gazette?
An insurer follows the identity proof, and its offices are usually outside the state. Aadhaar comes first. That points to the Gazette of India.
Step 1: Ask the Aadhaar centre which Gazette it accepts
The insurer follows Aadhaar.
Step 2: Ask the insurer, in writing
Where it wants the notification itself.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Families Make?
| Mistake | What to do |
|---|---|
| Typing family members' names from memory | Copy each from that person's Aadhaar |
| Never reading the policy schedule | Read it the day it arrives, and at every renewal |
| Leaving a child as "baby of" | Send the birth certificate once the name is registered |
| Finding the mismatch at admission | Check in a quiet week |
| Registering the patient at the hospital in a short form | Use the name on the identity proof |
| Getting a Gazette for a spelling | An endorsement on identity proof is enough |
| Asking the insurer directly about a group policy | Go through the employer |
| Throwing away originals after a cashless refusal | Keep every bill for a reimbursement claim |
| Assuming a correction carried into the renewal | Check the renewed schedule |
A typical case we handle: a man in Dehradun had covered his mother on a family policy as "Kamla Devi". Her Aadhaar read "Kamla Rawat". When she was admitted for a hip operation the desk held the cashless request. His sister rang us asking how fast a Gazette could be had. We told her none was needed, and none would come in time anyway. He sent the insurer his mother's Aadhaar and her old ration card showing both forms, and the endorsement came through on the second day of her stay. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
For a spelling, a married name, a child named after birth or a parent's short form. Each is an endorsement on ordinary papers. We are useful only where an insured person has changed a name by choice, and Aadhaar needs a Gazette before the insurer can follow.

All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. Insurers are dealt with online or through an agent. The Gazette file, where one is needed, is prepared on WhatsApp.

Health Insurance Policy Name Correction: The Short Version
Read the policy schedule against each member's Aadhaar, now. A wrong name is corrected by the insurer as an endorsement, on identity proof. A married name needs the marriage certificate, a child's name the birth certificate, and a change by choice a Gazette notification. An employer's policy is corrected through the employer. If it is found at admission, treat first, and claim by reimbursement if cashless is refused.
