Name Change in LIC Policy in Uttarakhand

When the name on a life insurance policy does not match your Aadhaar, PAN or bank account: the names a policy carries, where to apply, the proof to take, and why a nominee's name deserves the same care.

Quick answer

A name change in LIC policy records is requested by a written application at the branch that services the policy, with the original policy bond for endorsement. Guides list a marriage certificate or a Gazette notification as the proof of a changed name, and identity papers for a spelling. Confirm the list with your branch.

A policyholder in Uttarakhand reading the names of the life assured and the nominee on an old life insurance policy bond beside an Aadhaar card
A name on a life insurance policy is changed at the servicing branch and endorsed on the original bond, and the nominee is put right by a fresh nomination, while the policyholder is alive.

A life insurance policy runs for twenty or thirty years, and is read closely on only two days: the day it matures and the day a claim is made. A name change in LIC policy records is best done long before either. This page explains name change in LIC policy for policyholders in Uttarakhand, and the nominee's name with it.

We do not deal with insurers. We prepare the Gazette notification, which guides list as proof of a changed name. We could not find the Corporation's own published list of papers for a change of name, so the proof described here is as guides report it. For the bank account a policy pays into, read name change in bank records in Uttarakhand.

Which Names Does a Policy Carry?

The names on a life insurance policy and who each is
NameWho it isWhere it matters
The life assuredThe person whose life is insuredAt a death claim, against the death certificate
The proposer or policyholderThe person who took the policy and pays for it. Often the same person. A parent, for a child's policyFor every request during the policy, and at maturity
The nomineeThe person named to receive the money on the life assured's deathAt a death claim, against the nominee's identity proof
The appointeeAn adult named to receive it for a nominee who is a minorAt a death claim while the nominee is under age
The assigneeA person or a lender to whom the policy has been assignedWhere the policy secures a loan

The policy bond and the proposal form behind it carry all of these. A request to change a name has to say which one, since the proof and the person who signs differ for each.

When Is the Name on a Policy Read?

  1. At maturity or a survival benefit. The money is sent to the policyholder's bank account. The account's name has to agree with the policy's.
  2. At a death claim. The life assured's name is compared with the death certificate, and the nominee's name with the nominee's identity proof and bank account.
  3. At a loan on the policy or a surrender. The policyholder's identity is checked.
  4. When identity details are updated. The insurer links PAN and other identity papers to the policy, and a difference shows.
  5. At an income tax assessment. A maturity payment above the exempt limits is reported against PAN.

A premium is accepted for decades in any spelling. That is why a policy taken in a home name or a maiden name can run its whole term without a question, and then stop at the last step.

Which Case Is Yours?

How Do You Apply at the Servicing Branch?

Step 1: Read the bond, and list every name on it

The life assured, the proposer, the nominee and any appointee, each exactly as printed. Note the policy number.

Step 2: Find the servicing branch

It is the branch named on the premium receipt. Guides say a change of name is made by that branch. If you have moved, ask the nearest branch if it can forward the request or transfer the policy.

Step 3: Ask the branch for its requirement

Its form or the wording it wants in an application, and the proof for your kind of change.

Step 4: Write the application

To the branch manager. Give the policy number, the name as on the bond, the name as it should read, and the reason in a sentence.

Step 5: Attach the proof, self-attested, and carry the originals

Identity papers, and the marriage certificate or Gazette notification for a changed name.

Step 6: Hand in the original policy bond

The change is recorded by an endorsement on the bond. Take a receipt for the bond.

Step 7: File a fresh nomination at the same visit, if it needs one

On the Corporation's notice of change of nomination. It is endorsed on the bond as well.

Step 8: Collect the bond and read the endorsement

Check the spelling, then update the bank details and PAN linked to the policy.

Where you hold several policies, list them all in the application. Policies serviced by different branches need a request at each.

Which Proof Should You Carry?

We could not find the Corporation's own published list. This is what guides report branches asking for. Confirm it with your branch.

Every request

  • The original policy bond
  • A written application to the branch manager
  • Aadhaar and PAN, in original with self-attested copies
  • A recent premium receipt
  • A cancelled cheque of your bank account

As the case needs

  • The marriage certificate, for a surname on marriage
  • The Gazette notification, for a change of name
  • A divorce decree, for a return to a maiden name
  • A sworn declaration that both names are one person, where asked
  • An older paper carrying the bond's form of the name

If the policy bond is lost, that is a separate request for a duplicate, with its own indemnity and notice. Deal with it first, since there is nothing to endorse without a bond.

What Is the Position After Marriage?

  • A policy taken in a maiden name stays valid. A woman does not have to change it.
  • If she has moved her Aadhaar, PAN and bank account to her married name, the policy should follow, or the maturity payment will not match her account.
  • The marriage certificate is the proof guides list. A Gazette notification is not needed for a married surname where the certificate is at hand.
  • Her nomination usually needs changing at the same time, from a parent to her husband or children. That is a fresh nomination.
  • A policy her father took on her life as a child, with himself as proposer, needs his part attended to as well. Ask the branch how the policy now stands.

See name change after marriage in Uttarakhand.

Why Does the Nominee's Name Deserve the Same Care?

The policyholder never sees a death claim. The nominee does, alone, and has to prove to the branch that the name written on the bond thirty years ago is his or hers.

  • A nomination is changed by giving the Corporation a notice in its form, which guides identify as Form 3750, with the policy bond for endorsement.
  • It can be changed any number of times while the policy runs, and the existing nominee's consent is not needed.
  • Write the nominee's name exactly as on the nominee's present Aadhaar, with the relationship and age.
  • Where the nominee is a minor, name an appointee.
  • A wife nominated in her maiden name, a child nominated by a pet name, and a nominee who has since died are the three common faults. Each is cured by a fresh nomination.
  • A policy assigned to a lender has its nomination affected by the assignment. Ask the branch after the loan is cleared.
Take out every policy bond this week and read the nominee's name. If it is not how that person's Aadhaar reads today, file a fresh nomination. It costs almost nothing and saves the family months.

What Else Should Be Brought Into Line?

  • The bank account for payments. Insurers pay maturity and claims by electronic transfer. Register an account whose name matches the policy.
  • PAN. Link it to the policy in the same name.
  • The address and mobile number. So that notices and codes reach you.
  • The date of birth. If it is wrong on the policy, that is a more serious correction than a name, since the premium was calculated on it. Raise it at once.
  • The father's or husband's name, where the proposal recorded it.

The order is the usual one: Aadhaar and PAN first, then the bank, then the policy. See name change in PAN card in Uttarakhand.

What If a Mismatch Is Found at a Claim?

At maturity, the policyholder alive

  • The policyholder can still apply for the correction, as above
  • Or give a bank account in the bond's form of the name, if one exists
  • The branch may accept a declaration with identity proof for a small difference
  • For a different name, it will want the linking paper before paying

At a death claim

  • The bond's name is compared with the death certificate
  • Where the death certificate is the odd paper, have the Registrar correct it
  • Where the bond is the odd one, give papers showing the dead person used both forms
  • The nominee's own changed name is linked by a marriage certificate or a Gazette notification

A Gazette cannot be taken out for a person who has died. So a home name on a dead policyholder's bond is closed by evidence and declarations, which takes longer and rests on the insurer's satisfaction. That is the whole argument for doing it in the policyholder's lifetime.

See name correction in a death certificate and legal heir certificate.

What About Other Insurers, and Complaints?

  • Private life insurers take the same request at any branch or online, on their own form, and issue an endorsement letter. The proof is of the same kind.
  • Health and motor policies are renewed every year, so a name is corrected at the insurer's service desk or at renewal. A health policy's name should match the identity proof shown at a hospital.
  • Postal life insurance is run by the Department of Posts, through its own offices.
  • If a request is not acted on, write to the insurer's grievance officer. The insurance regulator sets times within which servicing requests and complaints are to be dealt with.
  • If that fails, use the regulator's online grievance portal, and then the Insurance Ombudsman for the region, whose service is free.

Keep a dated copy of everything you hand in. A complaint rests on the date of the first request.

How Does the Central Gazette Route Work?

Use this for a change of name by choice, or where a branch asks for a public paper linking a home name on an old bond to your present name. An insurer with branches across the country is used to the Gazette of India.

Step 1: Ask the branch what it will accept

Identity proof, a marriage certificate or a declaration may be enough.

Step 2: Fix the name on your identity papers, and list the other form as on the bond

Letter by letter. Where several policies carry different forms, list them all.

Step 3: Make the affidavit

State every form, your father's or husband's name and your address. Sign before a notary.

Step 4: Publish the newspaper notice

In a daily local leading newspaper. Keep the original newspaper.

Step 5: Type the form and sign the undertaking

Two typed copies of the specimen, signed with two witnesses, and the undertaking.

Step 6: Prepare the CD, photographs and certificate

The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.

Step 7: Pay the charge on BharatKosh

Commonly Rs 1,100 for an adult, with Rs 250 more for each alias. It is not refunded.

Step 8: Send the file yourself, then go to the branch

Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Take a print of the published notification to the servicing branch with the bond and your application.

See alias name in a gazette notification.

Would the Branch Accept the Uttarakhand Gazette?

Ask it. The Gazette of India is the safer choice where the same paper has to serve PAN, the bank and other insurers.

Step 1: Ask the servicing branch which Gazette it accepts

Before you choose.

Step 2: Think about PAN, the bank and your other policies

The Gazette of India serves them all.

Step 3: Contact the Roorkee press

Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.

Step 4: Ask for the papers, the format and the fee, and submit

Keep photocopies of everything.

Step 5: Download the notice

Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Policyholders Make?

Common mistakes with a name in a life insurance policy
MistakeWhat to do
Never reading the bond after buying the policyRead every name on it this week
Leaving it for the maturity dateCorrect it while the policy is running
Correcting the policyholder's name and ignoring the nominee'sFile a fresh nomination at the same visit
Going to any branchGo to, or through, the servicing branch
Handing over the bond without a receiptTake a dated receipt for it
Paying for a Gazette before asking the branchIdentity proof or a marriage certificate often serves
Letting an agent "manage" it informallyApply in writing yourself, and keep a copy
Registering a bank account in a different nameThe account's name should match the policy's
Forgetting other policies at other branchesList them all, and apply at each

A typical case we handle: a shopkeeper in Kashipur held three policies taken in the 1990s as "Pappu Lal", the name his agent knew him by. His Aadhaar, PAN and bank account said "Prem Lal Arya". With the first policy two years from maturity, the branch asked for something that tied the two names together. He had no old paper as Pappu Lal. We prepared a Gazette of India notice stating both, he gave it with the bonds, and all three were endorsed. He filed fresh nominations in his wife's present name on the same day. This is an illustrative example of the type of case we handle, not a named client review.

What is a lawyer's job, not ours

A claim the insurer has repudiated, a dispute between a nominee and heirs, and a complaint before the Ombudsman or a consumer commission.

When you do not need us

For a spelling, identity proof. For a married surname, the marriage certificate. For a nominee, a fresh nomination. We are useful where a name has in fact been changed, or a branch asks for a Gazette notification to link two forms of it.
