CGHS Card Name Correction in Uttarakhand
For central Government employees, pensioners and their families whose name on a health scheme card does not match the service record, the pension order or Aadhaar: where the name comes from, who corrects it, and why a dependant's name deserves as much care as your own.
A CGHS card name correction is asked for through the employee's own office, or by a pensioner at the Additional Director's office of the city. The card copies the service record or the pension payment order, so correct that record first where it is the one that is wrong. A slip on the card needs no Gazette.

The card is shown at a dispensary counter or a hospital's admission desk, usually when someone in the family is unwell. A CGHS card name correction is far easier on an ordinary day than on that one. This page explains CGHS card name correction for central Government employees and pensioners living in Uttarakhand.
We do not deal with the scheme's offices. We prepare the Gazette notification, where a name has in fact been changed. How a card is applied for is as guides to the scheme and a recent office memorandum describe it. We could not confirm a separate form for a correction. For the state's own employees, read name change for government employees.
What Is the Scheme, and Who Issues the Card?
- What it is. The central Government's health scheme for its serving employees, its pensioners and their dependent family members, in the cities the scheme covers.
- In this state. The scheme's published answers list Dehradun among its cities.
- The dispensary. Each beneficiary is attached to a wellness centre, where the doctor is seen and medicines are issued.
- Who issues the card. The office of the Additional Director of the scheme for the city. Guides say cards are handed over through the wellness centre.
- Who gets a card. The main cardholder and each dependant, each with a card of their own under one family record.
The scheme is run by the health ministry, but a serving employee reaches it through the employee's own office. That is the thing to remember about a correction too.
Where Does the Name on the Card Come From?
- The service record. A serving employee's name, date of birth and family details as the office holds them.
- The pension payment order. For a pensioner, the order names the pensioner and the spouse entitled to family pension.
- The application. It lists the applicant and each family member, with the relationship. A recent office memorandum has serving employees applying online.
- The endorsement. A serving employee's office certifies the application and sends it on. A pensioner applies to the Additional Director's office directly.
- The card. Made from the application, for each member.
So a name passes through three hands before it is printed: the record, the applicant and the office that keys it in. A slip can enter at any of the three, and the cure depends on which.
In Which Three Places Can a Name Differ?
The card against the application
- The application was right, and the card was keyed in wrongly
- A slip of the issuing office
- Cured by pointing it out, with a copy of the application
The record behind the application
- The service book or the pension order itself carries an old spelling, an initial or a maiden name
- The card faithfully copies it
- Cured in the record first, through the office or the pension authority
The card against Aadhaar
- The card and the record agree, and Aadhaar reads differently
- Found when records are linked or an identity is checked
- Cured by bringing Aadhaar to the service record, or the other way where the name has in fact changed
For a Government servant the service record is the anchor. Other papers are brought to it, unless the name itself has changed, in which case the record is changed by the service procedure.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The card has a spelling slip, and the record is right | A request to the issuing office, with the application or the record | No |
| The service record itself has the wrong spelling | A correction of the service record through the office, then the card | Ask the office |
| A wife is on the card in her maiden name | Her updated identity papers and the marriage certificate | No |
| A dependant's name differs from the dependant's Aadhaar | Correct whichever is wrong, then the card | No |
| The pension order names the spouse wrongly | A correction of the pension record, then the card | No |
| A spouse or a child is not on the card at all | An addition. Not a correction | No |
| The employee has changed a name by choice | The service procedure, with a Gazette, then the record and the card | Yes |
A Gazette appears once for certain. In the second row it depends on how far the record is from the true name, and the office decides.
How Do You Put It Right?
This is the general method. Ask the wellness centre or the Additional Director's office for the form in use.
Step 1: Look at every card in the family
Each member has a card. Read the name, the relationship and the date of birth on each.
Step 2: Find the paper the card was made from
The application as it was submitted, and behind it the service record or the pension payment order.
Step 3: Put Aadhaar beside them
Yours and each dependant's.
Step 4: Decide where the difference began
In the card only, in the application, in the service or pension record, or in Aadhaar.
Step 5: Correct the root record first
The service record through your office, the pension record through the pension office, Aadhaar at an Aadhaar centre.
Step 6: Apply for the card to be corrected
A serving employee through the office. A pensioner to the Additional Director's office of the city.
Step 7: Attach the proof for each name
The corrected record, identity papers, and a marriage or birth certificate for a dependant.
Step 8: Collect the fresh card and read it
Then check that the dispensary's own screen shows the same.
What Is the Route for Serving Employees?
- A serving employee's dealings with the scheme go through the office. Guides say the application is forwarded by the department to the Additional Director's office.
- A recent office memorandum has serving employees applying for a new card online, with the office verifying it.
- A request to correct a name goes the same way: to the office first, with the proof.
- The office will read the request against the service book. Where the two differ, it will ask which is to change.
- On a transfer to another city of the scheme, guides give different accounts of what happens to the card. Ask the office, and read the card that follows.
- An employee posted outside the scheme's cities is covered by other medical rules. The card is not then in use, and its errors go unnoticed for years.
The memorandum also speaks of linking the scheme with the Government's systems for staff and pensioners. Where records are linked, a difference between them shows at once.
What About Pensioners and Family Pensioners?
- A pensioner applies to the Additional Director's office of the city, with the pension payment order. Guides say an employee can apply with the pension papers before retiring.
- The pension payment order is the pensioner's root paper. A spelling in it travels to the bank, the card and every later claim.
- The order usually names the spouse who will receive family pension. That name is the one she will be held to.
- Many central pensioners settle in Dehradun and the hills after service elsewhere. Their order was issued by an office far away, and a correction is asked for through the pension disbursing bank or the pension authority.
- A family pensioner, after the pensioner's death, has the card made in her own name. Her name on the order, on her Aadhaar and on her bank account should be one.
What About Spouses, Children and Parents?
- Each dependant has a card, in a name taken from what the cardholder wrote on the application.
- A wife is often written as she was known at the time of the first application. Her Aadhaar may have changed since, or may never have matched.
- Guides say a serving employee can have a spouse or a newborn added on the form for addition, endorsed by the department.
- A child's name written at birth and later settled differently at school should be brought to one form before it matters.
- Dependent parents are entered on the cardholder's declaration. Old parents' papers are the most likely in the family to carry a home name or a doubtful date of birth.
- Who counts as a dependant, and up to what age, is set by the scheme's rules. We did not confirm the current limits. Ask the office.
Read name change after marriage, name change for a newborn and date of birth correction.
What Happens at the Dispensary and the Hospital?
- At the wellness centre. The card is read at registration. The staff know regular patients, and a spelling rarely stops anyone.
- At an empanelled hospital. The admission desk has never seen the patient. It reads the card against an identity paper before treating a case as cashless.
- On a referral or a permission. The paper carries the name on the card. The hospital's bill then has to carry the same.
- On a reimbursement claim. The bills, the discharge summary and the card are read together by the office that pays.
- In an emergency. Treatment should not wait for a card. Show what you have and sort the papers afterwards.
Give the card's name, not a home name, at the hospital's desk, so that the discharge papers and the bill come out in it. Read hospital records name correction.
Which Other Health Cards Might the Family Hold?
| Card | Whose it is | Where its name comes from |
|---|---|---|
| The central scheme's card | Central employees, pensioners and dependants | The service record or the pension payment order |
| The state's golden card | State employees, pensioners and dependants | The state service or pension record |
| The ex-servicemen's health card | Retired soldiers and dependants | The discharge and pension papers |
| The Ayushman card | Families under the central and state health cover schemes | A family list, checked against Aadhaar |
| The health record account | Anyone who has made one | Aadhaar |
A household in Dehradun can easily hold three of these. Each is corrected at its own office, and a correction of one does nothing for the others. Read name change in Ayushman card, ECHS card name correction and name change in ABHA health ID.
How Does the Central Gazette Route Work?
Use this where a cardholder or a dependant has in fact changed a name. For a serving employee the Gazette is one step in the service procedure, which the office will set out. It does nothing for a slip on a card.
Step 1: Follow the service procedure first, if you are serving
A Government servant's change of name has its own steps, and the Gazette is one of them.
Step 2: Confirm that it is a change of name, and not a spelling
A spelling is corrected in the record that carries it.
Step 3: Fix the exact new name
The spelling that will go on the service or pension record, Aadhaar and the card.
Step 4: Make the deed or affidavit and publish the newspaper notice
As the service procedure or the publication guideline asks. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded. Confirm the charge that applies to a Government servant.
Step 8: Send the file, then update in order
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Then the service or pension record, Aadhaar, and last the card.
Does the Uttarakhand Gazette Matter Here?
Hardly ever. This is a central scheme resting on central service and pension records. Expect to need the Gazette of India.
Step 1: Ask your office or pension authority which Gazette it accepts
A central office will expect the Gazette of India.
Step 2: Think about Aadhaar, PAN and the bank
The Gazette of India serves them all, including your PAN.
Step 3: Contact the Roorkee press only if a state notice is asked for
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.
To weigh the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Do Cardholders Make?
| Mistake | What to do |
|---|---|
| Reading only your own card | Read every family member's |
| Asking for the card to be corrected when the service record is wrong | The record first, through the office |
| A serving employee going straight to the scheme's office | Go through your own office |
| Writing a wife's name from memory on the application | Copy it from her Aadhaar |
| Leaving a wrong spouse name on the pension order | Correct it while the pensioner can sign |
| Giving a home name at the hospital desk | Give the name on the card |
| Taking the state's golden card for this one | They are separate schemes |
| Getting a Gazette for a typing slip | The issuing office corrects its slip |
| Changing a name privately while in service | Follow the service procedure |
A typical case we handle: a retired central employee settled in Dehradun found, when his wife was referred to a hospital, that her card read "Kamla Rani" while her Aadhaar read "Kamla Sharma". He asked us for a Gazette. Her name had not changed by choice. She had taken his surname on marriage, and his pension order still carried the name he had written in 1988. He had the pension record corrected on the marriage certificate, and the card was then remade. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
For a slip on the card, a spelling in a record, or a married surname. Your office, the pension authority and the scheme's office settle those. We are useful where a cardholder or a dependant has changed a name by choice, and a Gazette notification is asked for.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The card is corrected through your office or the scheme's office for your city. The Gazette file, where one is needed, is prepared on WhatsApp.

That a serving employee applies through the office, a pensioner to the Additional Director's office, and how a spouse or child is added, are as guides to the scheme describe them. That serving employees apply online, and that the scheme is being linked with the Government's staff and pensioner systems, is from a recent office memorandum on issuing cards. That Dehradun is covered is from the scheme's published answers. We could not confirm a separate form or fee for correcting a name, the current rules on dependants, or what happens to a card on transfer. The order of correction is general guidance. Your office and the Additional Director's office are the authority.

A Name on the CGHS Card: The Short Version
The card copies the application, and the application copies the service record or the pension payment order. Read every family member's card. Where the card alone is wrong, the issuing office corrects it. Where the record is wrong, the record is corrected first, through your office or the pension authority. A serving employee goes through the office, and a pensioner to the Additional Director's office of the city.
A slip needs no Gazette. To find out which record to correct, send photos on WhatsApp 9540005064, and we will reply in working hours.
