Mother's Name Correction in a Birth Certificate in Uttarakhand
A wrong, shortened or different mother's name in a birth certificate: what the Registrar can correct, what proof to carry, and what to do where her name only changed later.
A mother's name correction in a birth certificate is made by the Registrar who holds the record, under Section 15 of the Registration of Births and Deaths Act, 1969. A clerical mistake is corrected on proof. It is not a Gazette matter. Where her name was right at the birth and changed later, the entry is not an error.

A mother's name correction in a birth certificate usually comes up when a child's passport, school admission or Aadhaar is being made, and her name on the certificate does not match her own papers. This page explains mother's name correction in a birth certificate in Uttarakhand: what the Registrar can do, the proof to carry, and the cases a correction does not cover.
We do not correct birth records, and no Gazette does. The Registrar does it, at little or no cost. For her name in other records, read mother's name change and correction in Uttarakhand.
What Does a Birth Record Say About the Mother?
A birth is entered in a register kept by the Registrar of births for the area where the child was born. The birth certificate is an extract from that register. It carries the child's name, date and place of birth, and the names of the parents.
- The mother's name is entered from the birth report, which a hospital or the family gives soon after the birth.
- It is written as it was given on that day, often by a hospital clerk from the admission slip.
- It records who the mother is. It is not a record of what she is called at any later time.
- Every later paper of the child, the Aadhaar, the school record and the passport, is made from it.
So the question to ask of a birth certificate is a narrow one: was the mother's name written correctly, as it stood on the day of the birth? If it was not, the Registrar can correct it. If it was, there is no error in the register, even where her name is different today.
Which Case Is Yours?
| The situation | Example | What it is | Route |
|---|---|---|---|
| A wrong letter | Sunita written as Sunitha | A clerical error | The Registrar, on proof |
| A short form or a title | Smt. S. Devi | A clerical error | The Registrar, on proof |
| A surname written by assumption | She was Sunita Devi. The clerk wrote Sunita Rawat | An error, if her papers of that time show Devi | The Registrar, on proof |
| Her name was right then and changed later | Maiden name at the birth, married surname now | Not an error in the register | Keep the linking paper. Ask the Registrar what it will note |
| The mother's name is blank | The column was left empty | An omission | The Registrar, on proof |
| A different woman is named | A relative was entered as mother | An error in substance, and a legal question | The Registrar's stricter procedure. Take a lawyer's advice |
The first three rows and the fifth cover most people. For them, the Registrar's office is the whole of it.
What Can the Registrar Correct?
Section 15 of the Act lets the Registrar correct an entry that is erroneous in form or substance. The correction is made by a suitable entry in the margin of the register. The original entry is not altered, and the Registrar signs and dates the marginal entry.
The rules made under the Act set out how. States make their own rules on the pattern of the model rules, which treat two kinds of error differently.
| Point | Clerical or formal error | Error in substance |
|---|---|---|
| What it is | A slip in writing: a wrong letter, a short form, a word left out | The entry states a wrong fact |
| Example | Sunitha for Sunita | A different woman entered as the mother |
| What the rule asks for | The Registrar enquires and, if satisfied, corrects it | A declaration setting out the error and the true facts, made by two credible persons who know the facts |
| Reporting | The Registrar reports the correction to the officer named in the rules | |
A mother's name with a wrong spelling is the first kind. Where the difference is larger, such as a wholly different surname, a Registrar may treat it as the second kind and ask for the declaration by two persons. Ask the Registrar which it is before you collect papers.
We could not confirm the current text, fee or form of Uttarakhand's own rules, so take the table as the general pattern and the Registrar as the authority.
How Do You Apply for the Correction?
Step 1: Find the Registrar who holds the record
The birth certificate names the registering office. It is the local body for the place where the child was born, not where you live now.
Step 2: Work out what the name was at the time of the birth
Look at the mother's papers from then: her own certificates, the marriage certificate, the hospital papers.
Step 3: Ask the Registrar how it treats your case
A clerical error or an error in substance, which form to use, and the fee.
Step 4: Apply in writing
Give the registration number and date, the entry as it stands and as it should read. A parent applies for a child. An adult applies for his or her own record.
Step 5: Attach the proof
The existing birth certificate, the mother's identity proof, and the papers the Registrar asks for.
Step 6: Keep a dated receiving
A copy of your application with the office stamp, or the reference number.
Step 7: Collect the corrected certificate and read it
Check the mother's name, the father's name, the child's name and the date of birth, letter by letter, at the counter.
Step 8: Use it to correct the child's other papers
The Aadhaar, the school record and the passport follow the birth certificate.
Which Proof Should You Carry?
The Registrar decides what it needs. These are the papers commonly asked for, so you can start collecting.
For a clerical error
- A written application by a parent, or by the person if an adult
- The birth certificate that carries the mistake
- The mother's identity proof showing the right spelling
- The hospital's discharge paper or birth report, where it is kept
- The marriage certificate, where it helps to show her name
- The applicant's identity proof
Where the Registrar asks for more
- A declaration by two persons who know the facts, setting out the error and the true facts
- An affidavit by the parents
- A paper of the mother from before or around the birth, such as her own school certificate
- Any other record of the birth, such as an immunisation card
The strongest proof is a paper of the mother made before the child was born. It shows what her name was on that day, which is the only thing the register is meant to record.
What If Her Name Was Right at the Birth and Changed Later?
This is the case most people have without knowing it. The birth certificate shows the mother as she was then. Since then she has taken her husband's surname, or changed her name by Gazette, and her Aadhaar now reads differently.
- The entry in the register was correct when it was made. Section 15 is for an entry that is erroneous.
- So a Registrar may say there is nothing to correct. That is a fair reading of the law, and not obstruction.
- Some offices will add a note or issue a certificate with her present name on proof. Ask yours, in writing, what it can do.
- Where it will not, keep the linking paper with the birth certificate: her marriage certificate, or her Gazette notification.
That pair of papers does the work. A passport office or a school that sees the birth certificate with her earlier name and her marriage certificate can follow that she is the same woman.
For a mother who took a surname at marriage, see name change after marriage in Uttarakhand.
What About Devi, Kumari and a Surname?
A good number of birth certificates in Uttarakhand carry the mother's name with a surname she never used on paper. A clerk assumed the father's surname, and wrote Sunita Rawat for a woman whose every paper says Sunita Devi.
- If her papers of that time and of today say Sunita Devi, the entry is an error, and the Registrar can correct it on her identity proof.
- If she was Kumari Sunita before marriage and Sunita Devi after it, the certificate should show the name she had when the child was born.
- If she herself has used both forms, on different papers, she settles her own name first. See changing Kumari or Devi to a husband's surname.
- Devi after a name is part of the recorded name. A Registrar should write it as her papers have it.
Where Does the Mother's Name on a Birth Certificate Matter?
| Where | Why it is checked |
|---|---|
| A child's Aadhaar | A child is enrolled on the birth certificate and a parent's Aadhaar. The parent's name on both is compared |
| School admission | The school copies the parents' names from the certificate into its register |
| A child's passport | The parents' names are taken from the birth certificate and compared with the parents' own papers |
| Visas and papers abroad | A foreign office often asks for a birth certificate to prove who the parents are |
| Family and succession matters | The certificate is proof of the relationship between mother and child |
Because every later paper is copied from it, the birth certificate is the first one to put right. Correct it before the child's Aadhaar is made and before school admission, and the same mistake does not travel into four more records.
What About an Adult's Own Birth Certificate?
An adult can apply for a correction in his or her own birth record. It is harder with age, because the proof thins out.
- The hospital's papers may no longer exist, and the people who knew the facts may be hard to find.
- The Registrar may ask for the declaration by two persons, and for old papers of the mother.
- For most purposes in India, an adult's class 10 certificate is used for name and date of birth, and the birth certificate is not asked for.
- It matters most for papers abroad, where a birth certificate is asked for to prove parentage.
So before an adult starts this, ask the office that wants the birth certificate what it will accept. A marriage certificate that links the mother's two names, shown with the birth certificate as it stands, is often enough.
Where the birth was never registered at all, that is a late registration, with its own procedure. The Registrar will explain it.
What If the Registrar Refuses?
- Ask for the refusal in writing, with the reason.
- Read the reason. If it says the entry is not an error, look again at the section on a later change. The Registrar may be right.
- If more proof is asked for, collect it. A declaration by two persons is what the rules provide for an error in substance.
- If you believe the refusal is wrong, take it to the authority above the Registrar, or to a lawyer.
A Gazette notification is not the answer to a refusal. It does not alter the birth register, and a Registrar is not bound to act on one.
Where Does a Gazette Come In, If at All?
For the birth record itself, almost never. A Gazette has a part to play only around it, in three situations.
The mother's own change of name
Where she changed her name by Gazette, that notification is the paper that links her earlier and present names.
Another office asks for it
A passport office or an office abroad wants a public notice to explain a difference the Registrar would not alter.
The Registrar itself asks
Rare, but some offices ask for a public notice as supporting proof before correcting a larger difference.
In the second and third, the notice is a public notice stating that the name of your mother has been wrongly written in your record and what it actually is. Use it only where the entry really was wrong, and only after the office has asked for it in writing. See public notice for wrongly written name.
How Does the Central Gazette Route Work?
Use this only where an office has asked for a Gazette notification. The notice is a public notice in Part IV of the Gazette of India.
Step 1: Get the requirement in writing
Confirm which office wants the notification, and which form of the mother's name it treats as correct.
Step 2: Gather proof of both forms
The birth certificate with the wrong entry, and the mother's identity proof with the right one. Get copies attested.
Step 3: Sign the undertaking
The applicant is the person whose record it is, or a parent for a minor child.
Step 4: Publish the newspaper notice
In a daily local leading newspaper, showing the details of the case. Keep the original newspaper.
Step 5: Type the public notice form
Two typed copies of the specimen, keeping "my mother" and naming the birth certificate as the record. Sign with two witnesses.
Step 6: Prepare the CD, photographs and supporting papers
The same text on a CD in MS Word, two self-attested photographs with identity proof, a certificate that both copies match, and an attested copy of the birth certificate.
Step 7: Pay the public notice charge on BharatKosh
Commonly Rs 1,400. Confirm it before you pay. It is not refunded.
Step 8: Send the file yourself, then take the Gazette to the office
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. When it is published, give a printed copy to the office that asked for it.
Can the Uttarakhand Gazette Be Used?
A Registrar of births is a state authority, so a state notice may serve where it is the Registrar who asks. Ask first.
Step 1: Ask the office which Gazette it accepts
A passport office or an office abroad will usually want the Gazette of India.
Step 2: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private public notice for a wrongly written name is being accepted.
Step 3: Ask for the format, the papers and the fee
Say that the name in question is a mother's name in a birth certificate.
Step 4: Submit as directed and keep copies
Keep a photocopy of every page.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do People Make?
| Mistake | What to do |
|---|---|
| Paying for a Gazette to correct a birth certificate | Go to the Registrar. A Gazette does not alter the birth register |
| Going to the office where you live now | Go to the Registrar for the place of birth |
| Calling a later change of name a clerical mistake | Say what happened. Keep the marriage certificate with the birth certificate |
| Asking for a second birth registration | A birth is registered once |
| Making the child's Aadhaar before the certificate is corrected | Correct the birth certificate first |
| Giving the hospital a home name or a short form at the next birth | Give the mother's name in full, as on her Aadhaar |
| An oral request at the counter | Apply in writing and keep a receiving |
| Not reading the corrected certificate | Check every name and the date before you leave |
| Making the mother change her name to match the certificate | The certificate is the paper to correct or explain |
A typical case we handle: a couple in Dehradun were applying for their daughter's first passport and found the mother's name on the birth certificate spelt with one letter different from her Aadhaar. They had been quoted for a Gazette. The hospital's discharge paper had the right spelling. They applied to the Registrar with that paper and her Aadhaar, the entry was corrected, and nothing was published. This is an illustrative example of the type of case we handle, not a named client review.
What is a lawyer's job, not ours
A refusal by the Registrar, a case where a different woman is named as mother, and any dispute about parentage.
When you do not need us
For a mother's name in a birth certificate, you need the Registrar and nothing more. We do not correct birth records, and we will say so if you call. We are useful only where another office has asked in writing for a Gazette public notice.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The birth record itself is always with the Registrar for the place where the child was born.
People Also Ask About a Mother's Name in a Birth Certificate
How can I correct my mother's name in a birth certificate?
Apply to the Registrar of births who issued it, with proof of the right name.
Is a gazette required to correct a mother's name in a birth certificate?
No. The Registrar corrects an erroneous entry under Section 15 of the Act.
Which name of the mother goes in a birth certificate?
Her name as it stood when the child was born.
Can a mother's maiden name in a birth certificate be changed to her married name?
The entry was not an error, so a Registrar may decline. Keep the marriage certificate with it.
Who can apply for the correction?
A parent for a child, or the person concerned if an adult.
Where do I apply?
To the Registrar for the place where the child was born.
Is the mother's Aadhaar needed to register a birth now?
As reported, the 2023 amendment to the Act requires the parents' Aadhaar numbers for birth registration. That is one more reason the mother's name should match her Aadhaar from the start.
Why does the mother's name on a birth certificate matter more since 2023?
For a child born on or after 1 October 2023, the birth certificate is the single document to prove the date and place of birth, so every later record is copied from it.
For which purposes is the birth certificate the single document?
Admission to an educational institution, a driving licence, the voter list, an Aadhaar number, registration of marriage, appointment to a Government job, and any other purpose the Centre decides.
Is the corrected birth record kept in a national database?
The 2023 amendment empowers the Registrar General of India to maintain a national database of registered births and deaths, and states must register births on the Centre's Civil Registration System portal.
Can a single or unwed mother register her child's birth?
Yes. As reported, the 2023 amendment eases registration of children of single parents or unwed mothers, as well as orphaned, adopted and surrogate children.
Must a child's passport match the birth certificate?
In one reported case, a court said that where the birth certificate is produced, the passport entry must conform to it. So correct the mother's name on the certificate before the passport.

Mother's Name in a Birth Certificate: The Short Version
A birth certificate records the mother's name as it stood on the day of the birth. If it was written wrongly, the Registrar for the place of birth corrects it under Section 15, by a note in the margin, on proof such as her identity papers and the hospital's record. No Gazette is involved.
If her name was right then and changed later, the entry is not an error. Keep her marriage certificate or her Gazette with the birth certificate. To check which case is yours, send the papers on WhatsApp 9540005064, and we will reply in working hours.
