Adding a Surname When Records Show Only a First Name in Uttarakhand
When your papers say only "Rakesh" or "Rakesh Kumar" and a passport, a visa or a job form asks for a surname: how to add one, once, on every record.
Adding a surname to a single name in Uttarakhand is a change of name. If your certificates, Aadhaar and other papers show only a first name, you add a surname by an affidavit, a newspaper notice and a Gazette notification. The Gazette of India charge is commonly Rs 1,100 for an adult and Rs 1,700 for a minor.

Many people in Uttarakhand were enrolled in school by one name, with the family surname left off. Years later a form asks for a last name, and there is none on record. This page explains adding a surname to a single name: when it is needed, how it is done and what adding a surname to a single name changes on your papers.
For every kind of surname change, read surname change in Uttarakhand. For the Gazette steps in full, see gazette name change in Uttarakhand.
What Does a Single Name on Record Mean?
A single name on record means your documents carry a given name and no family name. It takes a few forms.
| On your papers | What is missing | What people usually want |
|---|---|---|
| Rakesh | Any second word at all | Rakesh Negi |
| Rakesh Kumar | A family surname. Kumar is a middle name | Rakesh Kumar Negi, or Rakesh Negi |
| Sunita Kumari or Sunita Devi | A family surname | Sunita Rawat |
| Mohan Singh | A family surname, where Singh is a middle name in that family | Mohan Singh Bisht |
A single name is a full and valid legal name. Nothing is wrong with your papers, and no rule says you must have a surname. The trouble is practical: many forms and systems are built for two names and do not know what to do with one.
Adding a surname that has never been on your records is not a correction. It is a change of name. The Department of Publication's guideline for a change of name covers the addition of a surname.
When Do You Need a Surname, and When Do You Not?
A surname helps when
- You are applying for a passport, or yours carries one name and you plan to travel
- A visa form, a foreign university or an airline asks for a first name and a last name
- An employer's system will not take a blank last name
- Your father, your children and you appear on papers with no name in common
- Two people in your town share your single name and records get mixed
You can leave things as they are when
- Your single name is the same on every paper you own
- You do not plan to travel or work abroad
- No office has asked you for a surname
- Only one form wants a last name, and it accepts your single name there
Some countries and airlines have raised difficulty with a passport that carries a single name. The rules differ from place to place and they change, so check the rule for your own destination before you decide. If your only reason is one form, ask that office first how it wants a single name entered. That costs nothing.
Is It an Addition or a Correction? How Do You Check?
If the surname is on some of your papers already, your case may be a correction or an alias case, and the route is different.
Step 1: Lay out every paper with your name
Birth certificate, school certificates, Aadhaar, PAN, passport, voter ID and bank passbook, oldest first.
Step 2: Write down the name on each
Note which carry the surname and which do not.
Step 3: Look at the oldest record
If your birth certificate or first school record has the surname and later papers dropped it, the later papers may be the ones in error.
Step 4: Decide which case you have
Surname on no paper: an addition. Surname on the oldest paper, missing later: a correction of the later ones. Surname on some papers and not others: one name with an alias.
Step 5: Ask the office concerned
For a correction, ask the office that left the surname out what it needs to put it back.
| Your papers show | Your case | Route |
|---|---|---|
| The surname on none of them | Addition of a surname | Gazette change of name |
| The surname on the oldest record, dropped by one later record | Correction | The office that dropped it. A public notice if it asks |
| The surname on some and not on others, with no clear error | Two forms of one name | Gazette change of name with an alias |
For the difference, read name correction vs name change.
Which Surname Can You Add, and in What Form?
An adult chooses his or her own name. Most people add the family surname their father uses. Some add the mother's surname. The Gazette office does not ask why.
The harder choice is the full form. Take a man recorded as "Rakesh Kumar" whose family name is Negi.
| New name | What it does | Think about |
|---|---|---|
| Rakesh Kumar Negi | Keeps everything and adds the surname | Closest to your old papers. Easy for offices to follow |
| Rakesh Negi | Drops Kumar and adds the surname | Shorter. It is an addition and a deletion in one notice |
| Rakesh K. Negi | Shortens Kumar to an initial | Initials cause mismatches. Best avoided |
- Write the name the way you will sign and type it for life.
- Spell the surname the way your father's papers spell it, so the family reads as one.
- Decide which word is the given name and which is the surname. A passport prints them in separate boxes.
- Do not use the new form anywhere until the Gazette is published and Aadhaar is updated.
How Does the Gazette Notice Read?
The notice follows the Department's change of name specimen. These are illustrations, with made-up names and addresses.
One name to a name with a surname
I hitherto known as RAKESH, son of Shri GOVIND SINGH NEGI, residing at 7, Mall Road, Almora, Uttarakhand 263601, have changed my name and shall hereafter be known as RAKESH NEGI.
Keeping the middle name and adding the surname
I hitherto known as RAKESH KUMAR, son of Shri GOVIND SINGH NEGI, residing at 7, Mall Road, Almora, Uttarakhand 263601, have changed my name and shall hereafter be known as RAKESH KUMAR NEGI.
Where some papers already carry the surname
I hitherto known as RAKESH KUMAR alias RAKESH KUMAR NEGI, son of Shri GOVIND SINGH NEGI, residing at 7, Mall Road, Almora, Uttarakhand 263601, have changed my name and shall hereafter be known as RAKESH KUMAR NEGI.
The form is signed in the old name and carries two witnesses. The father's name in the notice is written as his own papers show it, surname and all. An alias adds Rs 250 on the Department's list of additional charges. See alias name in a gazette notification.
How Do You Add a Surname Through the Gazette of India?
The notice goes in Part IV of the Gazette of India. A person who needs a surname for a passport or a visa should use this Gazette, because it is accepted across India and abroad.
Step 1: Fix the exact new name
The full form, the spelling and the order. Write it down.
Step 2: Make the affidavit
State the old name, the new name, your father's name and your address, and sign before a notary.
Step 3: Publish the newspaper notice
Print the change in a daily local leading newspaper. Keep the original newspaper.
Step 4: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and an undertaking showing the old and the new name.
Step 5: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 6: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 7: Send the file yourself
In person or by post, with a request letter, to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054.
Step 8: Download the Gazette
Check Part IV on egazette.gov.in each week and download the PDF.
Can You Use the Uttarakhand Gazette to Add a Surname?
The Government Press at Roorkee sets its own practice. Most people who add a surname do it for a passport or work outside the state, and for them the state Gazette is the wrong choice.
Step 1: Ask why you need the surname
For a passport, a visa or a central job, use the Gazette of India.
Step 2: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 3: Ask for the papers, the format and the fee
Note what the press wants and how the fee is paid.
Step 4: Publish the notice and submit the file
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.
To weigh the two, read Uttarakhand Gazette vs Gazette of India.
How Is a Surname Added for a Child?
The best time to add a surname is before it is missing from many papers. For a child, that is early.
- At birth registration. Give the child's full name, with the surname, when the name is entered in the birth record.
- At school admission. Check the admission form. The name the school writes on day one is the name that reaches the board certificate.
- After the school has recorded a single name. A Gazette change of name for a minor, applied for by the parents, at a charge of commonly Rs 1,700.
- Before board registration. Schools and boards grow strict about names once a child is registered for board examinations. Ask the school what its board allows.
For a child under 18, both parents sign the affidavit, or the legal guardian does. Where only one parent applies, a custody or guardianship order is needed. At 18, the young person can add a surname alone, as an adult.
What Happens to Your Records Afterwards?
The Gazette is supporting proof. It does not correct any record by itself. You apply to each office, in this order.
| Order | Record | Note |
|---|---|---|
| 1 | Aadhaar | At an Aadhaar Seva Kendra, with the Gazette and proof of the old name |
| 2 | PAN | Correction application. The PAN number stays the same |
| 3 | Bank accounts | Once Aadhaar and PAN agree |
| 4 | Passport | A re-issue. Check that the given name and the surname go in the right boxes |
| 5 | Voter ID, driving licence, employer | Each at its own office |
| 6 | Degrees and marksheets | They stay in the single name and remain valid. Keep the Gazette with them |
UIDAI says the address in the Gazette should match your Aadhaar, so give your Aadhaar address in the notice. If you already hold a visa or a ticket in the single name, finish that trip before you change the passport, or ask the airline and the embassy first.
What Does Adding a Surname Cost, and How Long Does It Take?
| Item | Typical cost | Note |
|---|---|---|
| Gazette charge, adult | Rs 1,100 | Paid on BharatKosh. Revised from time to time |
| Gazette charge, minor | Rs 1,700 | For a child under 18 |
| Each alias | Rs 250 more | Where some papers already carry the surname |
| Affidavit | Rs 100 to Rs 500 | Stamp paper and notary |
| Newspaper notice | Rs 800 to Rs 4,000 for each paper | Depends on the paper and the edition |
| Our assistance | Quoted per case | Shown as a separate line |
If you do everything yourself, the total is usually Rs 2,500 to Rs 6,000. The Gazette commonly takes 3 to 7 weeks from the affidavit to the published notice. The passport re-issue comes after Aadhaar, so if you are adding a surname for travel, start at least two to three months before you need the passport.
Which Mistakes Do People Make When Adding a Surname?
| Mistake | What to do |
|---|---|
| Typing the surname into a passport form with no record behind it | Add it through the Gazette first, then apply |
| Adding the surname in Aadhaar alone and nowhere else | Update every record, in order |
| Filing an addition when the oldest record already has the surname | That may be a correction. Check your papers first |
| Using an initial in the new name | Write every word in full |
| Spelling the surname differently from the father's papers | Copy it from his identity proof |
| Choosing the state Gazette for a passport case | Use the Gazette of India |
| Starting a few days before travel | Allow two to three months |
| Leaving a child's single name until the board certificate is printed | Do it before board registration |
A typical case we handle: a young man from Bageshwar had "Rakesh Kumar" on every certificate and a job offer abroad. His employer's form would not take a blank last name. He added his family surname through the Gazette of India, updated Aadhaar and PAN, and had his passport reissued with the given name and the surname in separate boxes. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
If your single name is the same everywhere and no office is asking for a surname, you need do nothing. If the surname was only dropped by one office, that office can usually put it back. And if your case is a plain adult addition and you are at ease with forms, the Department's guideline and this site are enough to file it yourself. Our charge, where we prepare the file, is quoted per case.
