Uttarakhand Paramedical Council Registration Name Mismatch
For laboratory, imaging and other paramedical workers whose name on the diploma, the registration certificate and their identity papers do not agree: where each name comes from, what to settle before registering, and how a correction differs from a change of name on the register.
An Uttarakhand Paramedical Council registration name mismatch is best cured before registering, by bringing Aadhaar to the name on the diploma or degree and the class 10 certificate. After registration, the council corrects its own slip on application. A change of name on the register is asked for separately, with a marriage certificate or a Gazette notification.

Technicians in laboratories, imaging rooms and operation theatres are the largest group of trained health workers after nurses. A hospital or a recruitment board asks each for a registration certificate. An Uttarakhand Paramedical Council registration name mismatch can cost a job offer. This page explains Uttarakhand Paramedical Council registration name mismatch for paramedical workers in the state.
We prepare the Gazette notification for a change of name. We did not read the state's Paramedical Council Act or find the council's own procedure for a correction or a change of name, so this page describes what councils commonly ask for. For other professions, read nursing council, pharmacy council and medical council registration name mismatch.
Which Records Does a Paramedical Worker's Name Sit In?
| Record | Kept by | What it is for |
|---|---|---|
| The diploma or degree, its marksheets and the training certificate | The institute, and the body that examined | The qualification to register |
| The register | The state council | The entry that makes a person a registered paramedical worker |
| The registration certificate and renewals | The state council | Proof of the entry |
| The employer's record | A hospital, a laboratory, a diagnostic centre or the Government | Appointment and pay |
| Reports and records the worker signs | The laboratory or the hospital | They carry the name, and often the registration number |
| A central register | The national commission, under the central Act | A countrywide record of these professions |
The state register is the root. The certificate and the renewals copy it, and an employer reads it at appointment.
Which Laws Apply?
| Law | What we could confirm |
|---|---|
| The state's Paramedical Council Act, 2009 | It is listed on the Government's code of laws as a state Act of July 2009, under the medical education department. Guides describe the council as registering paramedical workers and processing renewals |
| The National Commission for Allied and Healthcare Professions Act, 2021 | It provides for a national commission, a central register, a state allied and healthcare council and a state register. Its index covers registration, renewal, removal and restoration of a name in the state register |
| The rule on practice, under the central Act | Summaries say no person is to practise as a qualified allied and healthcare practitioner unless enrolled in a state register or the national register |
We did not read the text of either Act, and found in neither's summary a procedure for correcting or changing a registered name. That is left to the council's rules and practice, so the council's office is the place to ask.
Where Does the Name Come From?
- The class 10 certificate fixes the name and the date of birth.
- The institute admits the student in that name and enters it in its register.
- The examining body enrols the student from the institute's record, and issues the marksheets and the diploma or degree.
- The registration application is filled by the worker, with the qualification, marksheets, training certificate, identity proof and address proof.
- The council enters the register from that application and issues the certificate.
- The employer copies the certificate into its own record.
Many students take these courses straight after class 12, at private institutes, and fill each form in a hurry. A slip made at the institute's admission desk can pass through all six steps. Read name change in education documents.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| Not yet registered, and Aadhaar differs from the diploma | Correct Aadhaar to the diploma, then register | No |
| The diploma misspells the name against class 10 | A correction by the body that issued it, before registering | No |
| The certificate has a typing slip against the application | An application to the council to correct its slip | No |
| A woman worker wants her married name on the register | An application for a change of name, with the marriage certificate and identity proof in the new name | Ask the council |
| A worker has changed a name by choice | An application for a change of name, with the Gazette notification | Yes |
| The employer's record names the worker differently from the register | A correction by the employer, on the registration certificate | No |
| The registration has lapsed | Renewal. Not a name problem | No |
Most rows need a form and patience. Only one needs a Gazette.
How Do You Put the Papers in Order Before Registering?
Step 1: Lay out class 10, class 12 and the diploma or degree
With every marksheet and the internship or training certificate. Write the name and the father's name from each.
Step 2: Mark any paper that differs from class 10
Have it corrected by the body that issued it.
Step 3: Correct Aadhaar to the name on the qualification
The council takes identity proof.
Step 4: If your name has changed, complete the Gazette
Before the registration form, and ask the council how it will enter you.
Step 5: Read the council's current registration instructions
For its list of papers and its fee.
Step 6: Fill the form in the name on the qualification, in full
With the father's name as it reads there.
Step 7: Check the certificate the day you receive it
A slip pointed out at once is the council's to correct.
Step 8: Give your employer the name as on the certificate
Letter by letter, with the registration number.
How Is a Name Corrected or Changed After Registration?
We could not find this council's procedure. Councils of other professions treat these as two services, and that is a safe way to think about it.
A correction
- The register or the certificate does not match what you applied in
- You are asking the council to put right its own entry
- The proof is your diploma, your class 10 certificate and identity proof
- No Gazette is involved
A change of name
- The register is right for the name you had, and you now have another
- For marriage, councils commonly ask for the marriage certificate and identity proof in the new name
- For a change by choice, expect to be asked for a Gazette notification, with an affidavit
- The registration number ordinarily stays, and a fresh certificate is issued
Ask the council's office for its form, fee and list before you prepare anything. Where it asks for a Gazette notification, that is the paper we prepare. Where a marriage certificate is enough, you do not need us.
What About the Diploma Itself?
- Paramedical courses are run by many kinds of institute: medical colleges, universities, and private institutes recognised by the council.
- The diploma or degree is issued by whoever examined: a university, a state faculty or the council itself, as the course provides.
- Guides list results among the council's own services. Where the council examined you, the same office may hold both your marks record and the register.
- A slip on the diploma is corrected by its issuer, on the class 10 certificate, through the institute.
- An institute that has closed, or lost its recognition, makes this harder. Ask the council where its records went.
- Before joining any course, check that the institute and the course are recognised. A name correctly spelt on an unrecognised diploma registers nobody.
Read polytechnic diploma certificate name correction and migration certificate name correction.
What About Renewal?
- Guides say the council processes renewals of registration, under its own rules.
- The central Act's index also has provisions on renewal, removal and restoration of a name in the state register.
- A renewal is made in the name on the register. It is a natural moment to ask for a correction or a change, with the papers.
- Where identity proof is now in a different name from the register, expect a question.
- A worker returning from work in another state, or after a break, should check that the registration is still live.
- Keep every renewal receipt and certificate.
The period, the fee and the due date are for the council to tell you. We did not confirm them.
What About Employers, and the Reports You Sign?
- A hospital, a laboratory or a diagnostic centre asks for the registration certificate at appointment, and reads it against the diploma and identity papers.
- Government recruitment for technician posts verifies the same papers, more strictly.
- A laboratory's own licence or accreditation lists its technical staff by name and qualification.
- A technician's name goes on test reports, imaging records and theatre registers. Courts and insurers read these.
- When the register is corrected or the name changed, tell the employer in writing, with the fresh certificate.
- A worker who leaves a laboratory should see that its staff list no longer carries the name.
Read name change in private job and employer records and name mismatch at document verification.
What Is the Central Law, and What May Change?
- Parliament passed the National Commission for Allied and Healthcare Professions Act in 2021, to regulate these professions across the country.
- It provides for a national commission and a central register, and for each state to have its own allied and healthcare council and state register.
- A High Court, reading the Act, has called it a comprehensive law on the education, registration and licensing of these professionals.
- We could not confirm how far the state has set up its council under this Act, or how existing registrations with the state's earlier council are being carried over.
- Where registers are being moved from one body to another, names are re-entered. That is exactly when a slip is added.
Whatever body keeps the register, check your entry when it is carried over, and keep the old certificate. The principle is unlikely to change: the full name, as on the qualification.
What About a Qualification From Another State?
- Many paramedical workers in the state trained elsewhere, and many trained here work elsewhere.
- Ask the council what it requires of a qualification from outside the state, and whether it asks the other body to confirm it.
- A slip on another state's diploma or registration is corrected in that state, before you apply here.
- A woman who married and moved in the same year often has a diploma in one name and an Aadhaar in another. Decide which name this registration is to carry, and take the marriage certificate.
- Under the central Act a central register is meant to make movement between states easier. Ask the council how that works now.
How Does the Central Gazette Route Work?
Use this where a paramedical worker or a student has changed a name by choice. It does nothing for a slip on a certificate, which the issuing body corrects.
Step 1: Ask the council what it requires for a change of name
And which Gazette it accepts.
Step 2: Fix the exact new name
The spelling that will go on the register, Aadhaar and your employer's record.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then apply to the council
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar and PAN, then apply to the council, and then tell your employer.
A worker in Government service follows the service procedure for a change of name as well. See name change for government employees.
Would the Council Accept the Uttarakhand Gazette?
A state council may. A worker also needs a central register, Aadhaar and another state's council, on a later move, to read the same paper. That points to the Gazette of India.
Step 1: Ask the council which Gazette it accepts
In writing.
Step 2: Think about a central register, Aadhaar and work in other states
The Gazette of India serves everywhere.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Paramedical Workers Make?
| Mistake | What to do |
|---|---|
| Registering on a diploma that misspells the name | Have its issuer correct it first |
| Registering in the Aadhaar name where it differs from the diploma | Bring Aadhaar to the diploma |
| Asking for a "correction" when the name has in fact changed | Apply for a change of name, with the right proof |
| Getting a Gazette for a typing slip on the certificate | The council corrects its own slip |
| Changing the register and not telling the employer | Give the employer the fresh certificate |
| Letting the registration lapse | Renew in time |
| Not checking the entry when registers are moved to a new body | Read it, and keep the old certificate |
| Joining a course without checking recognition | Ask the council first |
| Not reading the certificate on receipt | Check it that day |
A typical case we handle: a laboratory technician in Rudrapur was offered a post at a hospital. His diploma read "Vikas Kumar". His registration certificate read "Vikash Kumar", and his Aadhaar matched the diploma. The hospital held the offer, and he asked us for a Gazette. He did not need one, since his name had never changed. The council had typed an extra letter. He applied to it with his diploma and class 10 certificate for a corrected certificate, and gave the hospital the receipt meanwhile. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us

That the state has a Paramedical Council Act of 2009 is from its entry on the Government's code of laws. We did not read its text. What the central Act of 2021 provides is from its published index, summaries and a High Court's description of it. That the state council registers workers, processes renewals and lists results is from a guide. We found nothing published by the council on correcting or changing a name, and could not confirm how the central Act is being applied in the state. What councils commonly ask for is general guidance. The council's office is the authority.

A Paramedical Worker's Registered Name: The Short Version
The register is written from the diploma or degree, in the full name. Bring Aadhaar to that name, and have the diploma itself corrected if it is wrong, before registering. A slip on the certificate is corrected by the council. A change of name is asked for separately, usually on a marriage certificate for a married name and a Gazette notification for a change by choice. Then give your employer the fresh certificate.
Renew in time, and check your entry if registers are moved to a new body. To find out if your case needs a Gazette, message us on WhatsApp 9540005064, and we will reply in working hours.
