Clinical Establishment Registration Name Mismatch in Uttarakhand: Clinics, Nursing Homes and Labs
For doctors and owners of clinics, nursing homes, hospitals and diagnostic labs in Uttarakhand, where the owner's or a doctor's name on the registration under the Clinical Establishments Act differs from Aadhaar, PAN or the council registration: who registers, who corrects each record, and why it matters at renewal and empanelment.
A clinical establishment name mismatch is fixed at the council register and Aadhaar first, then at the district registering authority. The owner registers the clinic, and staff details are read against each professional's council registration. Correct any council entry that differs, then ask the authority to update the certificate. No Gazette is needed for a spelling.

Every clinic, nursing home and lab in Uttarakhand must be registered under the Clinical Establishments Act, which the state adopted. The certificate names the owner, and the application lists the doctors and staff. Health schemes and the High Court have both pressed for registration. A clinical establishment name mismatch is a name there differing from Aadhaar or a council's register. This page explains clinical establishment name mismatch.
We do not register clinics, and this subject needs no Gazette in the ordinary case. The rules are from the Act and the Union health ministry's guidance. For a doctor's own registration, read medical council registration.
What Is the Act, in Brief?
- The law. The Clinical Establishments (Registration and Regulation) Act, 2010, adopted by Uttarakhand.
- Who. Hospitals, nursing homes, maternity homes, clinics, dispensaries and diagnostic establishments.
- Registration. Provisional first, then permanent once standards are met.
- The authority. A district registering authority in each district.
- The state's rules. The Uttarakhand rules of 2015.
The certificate names the owner and the establishment. The application names its professionals.
What Do the Act and Guidance Say?
From the Act as published and the Union health ministry's questions and answers on it.
| Point | What is said |
|---|---|
| Uttarakhand | Among the states that adopted the Act by a resolution of the assembly |
| Who registers | The owner of the clinical establishment is responsible |
| The authority | A district registering authority, chaired by the District Collector, with the Chief Medical Officer as convener |
| Two steps | Provisional registration, then permanent registration |
| Validity | Permanent registration described as valid for five years from issue |
| Appeals | To the State Council for clinical establishments |
The second row tells you whose name leads. The owner's name should match the owner's own papers.
Where Are the Names Written?
| Record | Kept by | Where it is corrected |
|---|---|---|
| The registration certificate | The district registering authority | That authority |
| Doctors' council registrations | The medical councils | Each council |
| Nurses' and paramedics' registrations | Their councils | Each council |
| Owner's Aadhaar and PAN | Their own offices | Their own offices |
| Trade licence and other licences | Local bodies and departments | Each issuer |
Councils are the root for professionals. The authority's certificate follows.
What Kinds of Mismatch Are There?
The owner's name
- An initial on the certificate, the full name on PAN
- Or a married name
- The authority updates it
A doctor's name
- The council register in one form, Aadhaar in another
- Staff details read against it
- The council corrects it
The establishment's name
- The signboard, trade licence and certificate differ
- Not a person at all
- Kept the same everywhere
The second is common for doctors who registered with a council years before Aadhaar.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The owner's name differs | The district registering authority | No |
| A doctor's council entry differs | The medical council | No |
| A nurse's registration differs | The nursing council | No |
| A technician's registration differs | The paramedical council | No |
| The pharmacy's pharmacist differs | The pharmacy council | No |
| The trade licence differs | The local body | No |
| The owner's Aadhaar is wrong | An Aadhaar centre | No |
| A doctor changed a name by choice | Gazette, then the council and the authority | Yes |
A Gazette appears once in eight rows. The councils and the authority settle the rest.
How Do You Put It Right?
This is the general method. Ask the Chief Medical Officer's office for its forms.
Step 1: Read the owner's name on the registration certificate
The owner is responsible for registering.
Step 2: Read each doctor's name as the council registered it
The staff details given at registration are read against it.
Step 3: Put Aadhaar and PAN beside them
For the owner and each professional.
Step 4: Correct the council registration where it differs
Each council corrects its own register.
Step 5: Apply to the district registering authority to update the certificate
With the corrected papers.
Step 6: Update staff details when people join or leave
The establishment discloses its staff.
Step 7: Keep the establishment's own name the same everywhere
Trade licence, signboard, bills and the registration.
Step 8: Renew in the same names
Permanent registration has a validity period.
What About the Owner?
- The owner is responsible for registering the establishment.
- The owner may be a doctor, a firm, a company or a trust.
- An individual owner's name should match Aadhaar and PAN.
- A firm's or company's name should match its registration.
- A change of owner should be told to the authority.
- Ask the authority how a change of ownership is recorded.
Read company records and partnership firms.
What About Doctors and Staff?
- Reports say establishments disclose the number and qualifications of their doctors, nurses and technicians.
- Each professional's name is read against their council registration.
- A doctor registered years ago may carry an initial or a maiden name in the council's register.
- The council corrects its own register, on proof.
- Update the establishment's staff details after the correction.
- Keep a list of staff with their council numbers, in their registered names.
Read medical council, nursing council and paramedical council.
What About the Establishment's Own Name?
- The establishment has its own name, separate from the owner's.
- It appears on the registration, signboard, bills, prescriptions and trade licence.
- A clinic called one thing on its board and another on its certificate invites questions.
- Keep one name everywhere.
- A change of the establishment's name should be told to the authority.
- Patients' reports and bills carry it too.
What About Renewal and Empanelment?
- Permanent registration has a validity period and is renewed.
- Renewal reads the same names.
- Health schemes commonly ask for the establishment's registration for empanelment.
- A news report in 2019 found few empanelled hospitals in Dehradun with permanent registration.
- The High Court has pressed for unregistered clinics to be acted against.
- Keep registration current and in matching names.
Read Ayushman card, on the scheme's patient side.
What About Other Licences?
- A clinic may also need a trade licence, fire safety clearance, biomedical waste authorisation and others.
- A pharmacy inside it needs a drug licence with a registered pharmacist.
- A diagnostic centre may need further registrations.
- Each names the owner or the establishment.
- Keep every name consistent.
- Ask each issuer how to correct a name.
Read pharmacy council and drug licence and FSSAI licence, if a canteen is run.
Where Does a Gazette Come In?
Not for an initial or a spelling. Only where a person has changed a name by choice.
Step 1: Confirm that a name has in fact changed
A short form on a certificate is not a change of name.
Step 2: Remember the councils
A doctor's registered name is changed by the council, which may ask for proof.
Step 3: Fix the exact new name
The spelling that will go on Aadhaar, PAN and the council register.
Step 4: Make the affidavit and publish the newspaper notice
Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update in order
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Then Aadhaar, PAN, the council and the registration.
Which Gazette Should You Use?
Registration is in the district, but Aadhaar, PAN and national registers are central. The Gazette of India serves all.
Step 1: Remember whose registration this is
Issued in the district under a central Act adopted by the state.
Step 2: Remember the councils
State councils, and some national registers.
Step 3: Prefer the Gazette of India
It serves Aadhaar, PAN and national registers as well as the state.
Step 4: Contact the Roorkee press only if a state notice suits you
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 5: Download the notice
Search egazette.gov.in and save the PDF.
For the difference between the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Happen?
| Mistake | What to do |
|---|---|
| A doctor's council entry in an old form | Correct it at the council |
| Staff details never updated | Update them as staff change |
| Different clinic names on board and certificate | One name everywhere |
| A change of owner not told | Tell the authority |
| Staying on provisional registration | Move to permanent |
| Letting registration lapse | Renew in time |
| Paying an agent to "manage" registration | Apply to the authority |
| Applying for empanelment with a mismatch | Correct first |
| Thinking an initial needs a Gazette | It does not |
A typical case we handle: a woman doctor in Haldwani ran a small maternity clinic. She had registered with the medical council as "Dr. Rekha Pant" before marriage. Her Aadhaar and PAN now read "Rekha Joshi", and the clinic's certificate used her married name. At renewal the staff details were questioned. She asked us whether a Gazette was needed. It was not. She asked the council to record her married name with the marriage certificate, and updated the clinic's staff details. This is an illustrative example of the type of case we handle, not a named client review.

That Uttarakhand adopted the Act, that the owner is responsible, the district registering authority's make-up, the two steps of registration, the five-year validity and appeals, are from the Act and the Union health ministry's guidance. The state's 2015 rules and staff disclosure are from reports and law summaries. The 2019 report on empanelled hospitals is from a newspaper. How names are corrected is general guidance. The district registering authority and the councils are the authority.
