Uttarakhand Dental Council Registration Name Mismatch
For dentists and dental graduates whose name on the degree, the registration certificate and their identity papers do not agree: where each name comes from, what to settle before registering, and how a correction differs from a change of name on the register.
An Uttarakhand Dental Council registration name mismatch is best cured before registering, by bringing Aadhaar to the name on the dental degree and the class 10 certificate. After registration, the council corrects its own slip on application. A change of name on the register is asked for separately, with a marriage certificate or a Gazette notification.

A dentist's registration number is printed on every prescription and displayed in the clinic, so the name beside it is read by patients, insurers and inspectors alike. An Uttarakhand Dental Council registration name mismatch can hold up a job, a renewal or a clinic's licence. This page explains Uttarakhand Dental Council registration name mismatch for dentists in the state.
We prepare the Gazette notification for a change of name. We could not find the state council's own procedure for a correction or a change of name, so this page describes what the Act says and what councils commonly ask for, and tells you to confirm with the council. For other professions, read medical council, nursing council and pharmacy council registration name mismatch.
Which Records Does a Dentist's Name Sit In?
| Record | Kept by | What it is for |
|---|---|---|
| The dental degree, its marksheets and the internship certificate | The university and the dental college | The qualification to register |
| The state register of dentists | The state dental council | The entry that makes a person a registered dentist |
| The registration certificate and renewals | The state dental council | Proof of the entry |
| The clinic's registrations and licences | The health, pollution control and other offices concerned | They name the dentist in charge |
| National registers | The central dental body and the national health registry | A countrywide record of dentists |
| Service records | A hospital, a college or the Government as employer | Appointment and pay |
The register is the root. The certificate, the renewals and the national records copy it, and the clinic's papers quote it.
What Does the Act Say About the Register?
From the published text of the Dentists Act, 1948.
- A register of dentists is maintained for the state, and the state council has the duty of keeping it.
- The register includes the full name, nationality and residential address of the registered person, the date of first admission to the register, the qualification for registration with the date and the authority that conferred it, and the professional address.
- The state Government may direct that an annual renewal fee be paid to keep a name on the register.
- Where the fee is not paid by the due date, the Registrar removes the name. A name so removed may be restored on payment in the prescribed manner.
- On payment, the Registrar issues a certificate of renewal.
- The Act provides for transfer of registration from one state's register to another's.
- It has separate registers for dental hygienists and dental mechanics.
The Act asks for the full name. We did not find in it a procedure for a registered person to have a name corrected or changed. That is left to the council's rules and practice, which is why the council's office is the place to ask.
Where Does the Name Come From?
- The class 10 certificate fixes the name and the date of birth.
- The entrance examination and the dental college admit the student in that name, and the university enrols the student from the college's record.
- The degree and the internship certificate carry the university's version of the name.
- The registration application is filled by the graduate, with the degree, marksheets, internship certificate, identity proof and address proof.
- The council enters the register from that application and issues the certificate.
- Every later record copies the certificate.
The graduate types the name once, in the registration form. Type it from the degree, with Aadhaar already brought into line, and the chain holds. Read name change in education documents.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| Not yet registered, and Aadhaar differs from the degree | Correct Aadhaar to the degree, then register | No |
| The degree misspells the name against class 10 | A correction by the university, before registering | No |
| The certificate has a typing slip against the application | An application to the council to correct its slip | No |
| A woman dentist wants her married name on the register | An application for a change of name, with the marriage certificate and identity proof in the new name | Ask the council |
| A dentist has changed a name by choice | An application for a change of name, with the Gazette notification | Yes |
| The clinic's registration names the dentist differently from the register | An amendment at the office concerned, on the registration certificate | No |
| The name was removed for an unpaid renewal fee | Restoration, on payment. Not a name problem | No |
Most rows need a form and patience. Only one needs a Gazette.
How Do You Put the Papers in Order Before Registering?
Step 1: Lay out class 10, class 12 and the dental degree
With every marksheet and the internship certificate. Write the name and the father's name from each.
Step 2: Mark any paper that differs from class 10
Have it corrected by the board or university that issued it.
Step 3: Correct Aadhaar to the degree name
The council takes identity proof, and national registers read Aadhaar.
Step 4: If your name has changed, complete the Gazette
Before the registration form, and ask the council how it will enter you.
Step 5: Read the council's current registration instructions
For its list of papers and its fee.
Step 6: Fill the form in the degree name
In full, with the father's name as on the degree.
Step 7: Check the certificate the day you receive it
A slip pointed out at once is the council's to correct.
Step 8: Give your clinic's registrations the name as on the certificate
Letter by letter.
How Is a Name Corrected or Changed After Registration?
We could not find this state council's procedure. Councils of other professions treat these as two services, and that is a safe way to think about it.
A correction
- The register or the certificate does not match what you applied in
- You are asking the council to put right its own entry
- The proof is your degree, your class 10 certificate and identity proof
- No Gazette is involved
A change of name
- The register is right for the name you had, and you now have another
- For marriage, councils commonly ask for the marriage certificate and identity proof in the new name
- For a change by choice, expect to be asked for a Gazette notification, with an affidavit
- The registration number ordinarily stays, and a fresh certificate is issued
Ask the council's office for its form, fee and list before you prepare anything. Where it asks for a Gazette notification, that is the paper we prepare. Where a marriage certificate is enough, you do not need us.
What About Renewal and Restoration?
- Under the Act, where the state Government has so directed, a renewal fee is paid each year to keep a name on the register.
- A name is removed where the fee is not paid by the due date, and can be restored on payment in the prescribed manner.
- A renewal is made in the name on the register. It is a natural moment to ask for a correction or a change, with the papers.
- A dentist returning from years abroad, or from a break in practice, often finds the name removed. Restore it first, and ask for the name to be put right alongside.
- Where identity proof is now in a different name from the register, expect a question.
- Keep every renewal certificate. Together they show an unbroken registration.
The fee, the due date and the state's direction on renewal are for the council to tell you. We did not confirm them.
Which of the Clinic's Papers Follow the Register?
- A dental clinic is set up on the dentist's registration. The offices that register or license the clinic read the dentist's name against the registration certificate.
- A dentist who owns the clinic appears twice: as proprietor, on PAN and the business papers, and as dentist, on the registration. All should be one name.
- A clinic with an X-ray unit, and one that generates clinical waste, has further permissions in the name of the person in charge.
- Insurers and patients read the name and registration number on prescriptions, bills and certificates.
- When the register is corrected or the name changed, each of these is amended in turn, on the fresh certificate.
- A dentist who leaves a clinic should see that its papers no longer carry the name.
We could not confirm which permissions the state requires of a dental clinic, or their forms. Ask the offices directly. Read name change in trade licence and shop establishment and name change in Udyam and GST registration.
What About National Records, and the New Law?
- The Dentists Act provides for a national register, built from the state registers.
- Health professionals are also being enrolled in a national digital registry. A profile there is built on Aadhaar, and is not the same thing as state registration.
- Where Aadhaar and the state register differ, the linking can fail. So the order is the same as everywhere: Aadhaar and the state register in one name first, then the national profile.
- Parliament passed a National Dental Commission Act in 2023, meant to replace the 1948 Act and its councils with a new commission and state bodies.
- That Act came into force on 19 March 2026. The Dentists Act, 1948 stands repealed and the Dental Council of India is dissolved. We could not confirm what this means for an existing registration in this state. Ask the council which rules it applies now.
Whatever body keeps the register, the principle is unlikely to change: the full name, as on the qualification. Read name change in ABHA health ID.
What About Moving in From Another State?
- Many dentists practising in Uttarakhand studied and first registered elsewhere. The Act provides for the transfer of a name from one state's register to another's.
- The new council enters the name from the old council's record. A slip in the old state's certificate is carried over unless it is corrected first.
- A correction of another state's register is asked for in that state.
- A woman who married and moved in the same year often has a degree in one name, an old registration in the same, and an Aadhaar in another. Decide which name the Uttarakhand registration is to carry, and take the marriage certificate.
- A graduate of a foreign dental college has further steps before registration, which this page does not cover.
What About Dental Hygienists and Dental Mechanics?
- The Act has separate registers for dental hygienists and dental mechanics, kept by the same state council.
- Their qualification is a diploma or certificate course, and the name on it decides the registered name in the same way.
- The same two requests apply: a correction of the council's slip, and a change of name with its own proof.
- A hygienist or mechanic employed in a clinic or a hospital is entered in the employer's record in the registered name.
How Does the Central Gazette Route Work?
Use this where a dentist or a student has changed a name by choice. It does nothing for a slip on a certificate, which the issuing body corrects.
Step 1: Ask the council what it requires for a change of name
And which Gazette it accepts.
Step 2: Fix the exact new name
The spelling that will go on the register, Aadhaar and the clinic's papers.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then apply to the council
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar and PAN, then apply to the council, and then update the clinic's papers.
A dentist in Government service follows the service procedure for a change of name as well. See name change for government employees.
Would the Council Accept the Uttarakhand Gazette?
A state council may. A dentist also needs the national registers, Aadhaar and another state's council, on a later move, to read the same paper. That points to the Gazette of India.
Step 1: Ask the council which Gazette it accepts
In writing.
Step 2: Think about Aadhaar and work in other states
The Gazette of India serves everywhere.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Dentists Make?
| Mistake | What to do |
|---|---|
| Registering on a degree that misspells the name | Have the university correct it first |
| Registering in the Aadhaar name where it differs from the degree | Bring Aadhaar to the degree |
| Asking for a "correction" when the name has in fact changed | Apply for a change of name, with the right proof |
| Getting a Gazette for a typing slip on the certificate | The council corrects its own slip |
| Changing the register and forgetting the clinic's papers | Have each amended on the fresh certificate |
| Letting the registration lapse | Pay the renewal fee in time |
| Treating a national profile as the registration | The state register is the registration |
| Carrying an old state's slip into a new state | Correct it at the old council first |
| Not reading the certificate on receipt | Check it that day |
A typical case we handle: a dentist in Haldwani was registered as "Neha Joshi", the name on her degree. After marriage her Aadhaar read "Neha Joshi Pande", and the office registering her new clinic questioned the application. She asked us for a Gazette. We told her to ask the council first, since a marriage certificate is commonly enough for a married name. She chose to keep her registered name, brought the clinic's application to it, and carried the marriage certificate to explain her Aadhaar. This is an illustrative example of the type of case we handle, not a named client review.

What the register contains, and the provisions on renewal, removal, restoration, transfer and the separate registers, are from the published text of the Dentists Act, 1948. We could not find the state council's own procedure, form or fee for a correction or a change of name, and what councils commonly ask for is general guidance. That the National Dental Commission Act came into force on 19 March 2026, repealing the Dentists Act, is from the health ministry's announcement as reported. The notes on a clinic's papers, national records and a move between states are general. The council's office is the authority.

A Dentist's Registered Name: The Short Version
The register is written from the dental degree, in the full name. Bring Aadhaar to that name, and have the degree itself corrected if it is wrong, before registering. A slip on the certificate is corrected by the council. A change of name is asked for separately, usually on a marriage certificate for a married name and a Gazette notification for a change by choice. Then bring the clinic's papers to the same name.
