Homoeopathic Practitioner Registration Name Mismatch in Uttarakhand
For homoeopathic doctors and graduates whose name on the degree, the registration certificate and their identity papers do not agree: where each name comes from, what to settle before registering, and how a correction differs from a change of name on the register.
A homoeopathic practitioner registration name mismatch is best cured before registering, by bringing Aadhaar to the name on the degree and the class 10 certificate. After registration, the state body corrects its own slip on application. A change of name is asked for separately, with a marriage certificate or a Gazette notification.

Homoeopathic clinics are found in every town of the state, and many are run by a single doctor who is also the owner. The registration is the one paper that entitles that doctor to practise. A homoeopathic practitioner registration name mismatch can hold up a Government post, a clinic's licence or a move to another state. This page explains homoeopathic practitioner registration name mismatch for practitioners in Uttarakhand.
We prepare the Gazette notification for a change of name. We did not find the state body's own procedure for a correction or a change of name, so this page describes what the central Act says and what such bodies commonly ask for. For Ayurveda and Unani, read Ayurvedic and Unani practitioner registration name mismatch.
Which Records Does a Practitioner's Name Sit In?
| Record | Kept by | What it is for |
|---|---|---|
| The degree or diploma, its marksheets and the internship certificate | The university or examining body, and the college | The qualification to register |
| The state register | The state's council or board for homoeopathy | The entry that lets a person practise |
| The registration certificate | The same body | Proof of the entry |
| The national register | A board of the national commission | A countrywide record of licensed practitioners |
| The clinic's papers | The offices that register or license it | They name the practitioner in charge |
| Service records | The state's homoeopathy service, a hospital or a college | Appointment and pay |
The state register is the root. The national register takes its entries from the states, and every other paper quotes the registration.
What Does the Central Act Say?
From the published text and index of the National Commission for Homoeopathy Act, 2020, rules made under it, and the national commission's own page on its registration board.
- A state medical council of homoeopathy is the body set up under the state's own law to regulate practice and registration. The state register is the register kept under that law.
- A board for ethics and registration, under the national commission, keeps a national register of all licensed practitioners of homoeopathy.
- A person who holds a recognised qualification and passes the national exit test has a licence to practise, with the name and qualifications enrolled first in the state register and then in the national one.
- The Act replaced the Homoeopathy Central Council Act of 1973, under which a central register was kept from copies of the state registers.
- Rules of 2021 list, for each practitioner, the name, address, registration number and date, and the register in which the name stands.
- The national board hears appeals from actions taken by a state council.
We found in these no procedure for a practitioner to have a registered name corrected or changed. That is left to the state body's rules and practice. Its office is the place to ask.
Where Does the Name Come From?
- The class 10 certificate fixes the name and the date of birth.
- The entrance examination and the college admit the student in that name, and the university enrols the student from the college's record.
- The degree and the internship certificate carry the university's version of the name.
- The registration application is filled by the graduate, with the degree, marksheets, internship certificate, identity proof and address proof.
- The state body enters the register from that application and issues the certificate.
- The national register takes the entry from the state.
The graduate types the name once, in the registration form. Type it from the degree, with Aadhaar already brought into line, and both registers will agree. Read name change in education documents.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| Not yet registered, and Aadhaar differs from the degree | Correct Aadhaar to the degree, then register | No |
| The degree misspells the name against class 10 | A correction by the university, before registering | No |
| The certificate has a typing slip against the application | An application to the state body to correct its slip | No |
| The national register differs from the state register | The state entry is put right first, and the national one follows | No |
| A woman practitioner wants her married name on the register | An application for a change of name, with the marriage certificate and identity proof in the new name | Ask the state body |
| A practitioner has changed a name by choice | An application for a change of name, with the Gazette notification | Yes |
| The clinic's papers name the practitioner differently from the register | An amendment at the office concerned, on the registration certificate | No |
Most rows need a form and patience. Only one needs a Gazette.
How Do You Put the Papers in Order Before Registering?
Step 1: Lay out class 10, class 12 and the homoeopathy degree or diploma
With every marksheet and the internship certificate. Write the name and the father's name from each.
Step 2: Mark any paper that differs from class 10
Have it corrected by the board or university that issued it.
Step 3: Correct Aadhaar to the name on the qualification
The state body takes identity proof.
Step 4: If your name has changed, complete the Gazette
Before the registration form, and ask the state body how it will enter you.
Step 5: Read the state body's current registration instructions
For its list of papers and its fee.
Step 6: Fill the form in the name on the qualification, in full
With the father's name as it reads there.
Step 7: Check the certificate the day you receive it
A slip pointed out at once is the office's to correct.
Step 8: See that the national record carries the same name
It is drawn from the state register.
How Is a Name Corrected or Changed After Registration?
We could not find the state body's procedure. Councils of other professions treat these as two services, and that is a safe way to think about it.
A correction
- The register or the certificate does not match what you applied in
- You are asking the office to put right its own entry
- The proof is your degree, your class 10 certificate and identity proof
- No Gazette is involved
A change of name
- The register is right for the name you had, and you now have another
- For marriage, such bodies commonly ask for the marriage certificate and identity proof in the new name
- For a change by choice, expect to be asked for a Gazette notification, with an affidavit
- The registration number ordinarily stays, and a fresh certificate is issued
Ask the office for its form, fee and list before you prepare anything. Where it asks for a Gazette notification, that is the paper we prepare. Where a marriage certificate is enough, you do not need us.
What About the National Register?
- The central Act has a practitioner's name and qualifications enrolled first in the state register, and then in the national register.
- The national commission's own page says its registration board maintains a national register of all licensed practitioners.
- The lists it compiles show each practitioner's name, address, registration number and date, and which register the name is in.
- A slip in the state entry is carried up. A slip made when the national entry is created is a fresh one.
- The order for any correction is the same: the state register first, and the national record after it.
- Look up your own entry once, and read the name letter by letter against the degree.
A practitioner moving to another state, or applying to a central body, is read on the national record. It is worth having both right before that day.
What About Older Registrations, and Diplomas?
- Many senior practitioners qualified with a diploma, before the degree became the usual course. Their registration was made on that diploma.
- Under the 1973 law, a central register was built from copies of the state registers. That record has since passed to the national commission.
- Each such move is a re-entry of names, and a chance of a slip.
- A practitioner registered before the state was formed may have an entry in the parent state's register. Ask the state body how such entries are treated.
- Hand-written certificates of the time often carry initials or an older spelling. The cure is a correction on the degree or diploma and identity proof, not a new name.
- Keep the oldest certificate you hold. It is the best proof of how you were first entered.
Ask the state body about renewal as well. We did not confirm its rule, period or fee.
What About the Clinic, and Government Service?
- A clinic. The offices that register it read the practitioner's name against the registration certificate.
- The signboard and the letterhead. They carry the name, the qualification and the registration number, and patients read them.
- Prescriptions and certificates. Insurers and employers read the name and number on them.
- Government service. The state appoints homoeopathic medical officers on the registration, and opens the service record in that name.
- A practitioner who owns the clinic appears twice: as proprietor on PAN and the business papers, and as practitioner on the registration.
- When the register is corrected or the name changed, each of these is amended in turn, on the fresh certificate.
We could not confirm which permissions the state requires of a homoeopathic clinic, or their forms. Ask the offices directly. Read name change in trade licence and shop establishment and name change for government employees.
What About Moving in From Another State?
- Most homoeopathic practitioners in the state studied in colleges elsewhere, and first registered there or here.
- A practitioner registered elsewhere asks the state body what it needs to enter the name here.
- A slip in another state's register is corrected in that state, before it is carried over.
- A woman who married and moved in the same year often has a degree in one name and an Aadhaar in another. Decide which name this registration is to carry, and take the marriage certificate.
- A holder of a qualification from outside the country has further steps before registration, which this page does not cover.
What If the State Body Says No?
- Ask for the reason in writing. "Name does not match" should say which paper, against which.
- Most refusals are answered by a paper: a corrected degree, a marriage certificate, an updated Aadhaar.
- Where the office says a correction is in truth a change of name, it is usually right. Apply for the change, with the proof a change needs.
- The central Act gives the national board an appellate role over actions of a state council.
- What can be appealed, and in what time, is a lawyer's question, not ours.
Do not sign papers in a form of the name the register does not carry while a request is pending. Sign as registered, with the number.
How Does the Central Gazette Route Work?
Use this where a practitioner or a student has changed a name by choice. It does nothing for a slip on a certificate, which the issuing body corrects.
Step 1: Ask the state body what it requires for a change of name
And which Gazette it accepts.
Step 2: Fix the exact new name
The spelling that will go on the register, Aadhaar and the clinic's papers.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then apply to the state body
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar and PAN, then apply for the change on the register, and then update the clinic's papers.
A practitioner in Government service follows the service procedure for a change of name as well.
Would the State Body Accept the Uttarakhand Gazette?
It may. A practitioner also needs the national register, Aadhaar and another state's body, on a later move, to read the same paper. That points to the Gazette of India.
Step 1: Ask the state body which Gazette it accepts
In writing.
Step 2: Think about the national register, Aadhaar and practice in other states
The Gazette of India serves everywhere.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Practitioners Make?
| Mistake | What to do |
|---|---|
| Registering on a degree that misspells the name | Have the university correct it first |
| Registering in the Aadhaar name where it differs from the degree | Bring Aadhaar to the degree |
| Asking for a "correction" when the name has in fact changed | Apply for a change of name, with the right proof |
| Getting a Gazette for a typing slip on the certificate | The state body corrects its own slip |
| Never looking up the national entry | Read it once against the degree |
| Putting a different form of the name on the signboard | Use the registered name and number |
| Changing the register and forgetting the clinic's papers | Have each amended on the fresh certificate |
| Throwing away an old hand-written certificate | Keep it. It shows the first entry |
| Not reading the certificate on receipt | Check it that day |
A typical case we handle: a homoeopathic doctor in Kashipur was registered as "Shabnam Khan", the name on her degree. After marriage her Aadhaar and PAN read "Shabnam Khan Siddiqui", and the office licensing her new clinic questioned the papers. She asked us for a Gazette. We told her to ask the state body first, since a marriage certificate is commonly enough for a married name. She chose to keep her registered name, applied for the clinic in it, and gave the marriage certificate to explain her Aadhaar. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
For a slip against the degree, on Aadhaar or on the certificate, and usually for a married surname. We are useful where a practitioner has changed a name by choice, or the state body asks for a Gazette notification.

Who Wrote and Reviewed This Page?
This page is written by the Documentation Desk Team of UttarakhandNameChange.com, the team that prepares Gazette files for people across Uttarakhand.
It is reviewed by Advocate Vipin Chauhan.
The state and national registers, the licence to practise and the board's appellate role are from the published text and index of the National Commission for Homoeopathy Act, 2020, and the national commission's own page on its registration board. The particulars listed for each practitioner are from rules of 2021 under the Act. That the earlier law of 1973 kept a central register from the state registers is from its published text. We did not read the state's own law on homoeopathy, and found nothing published by the state body on correcting or changing a name. What such bodies commonly ask for, and the notes on clinics, are general. The state body's office is the authority.

A Homoeopathic Practitioner's Registered Name: The Short Version
The state register is written from the degree or diploma, in the full name, and the national register follows it. Bring Aadhaar to that name, and have the degree itself corrected if it is wrong, before registering. A slip on the certificate is corrected by the state body. A change of name is asked for separately, usually on a marriage certificate for a married name and a Gazette notification for a change by choice. Then bring the clinic's papers to the same name.
