Name Change Request Letter to Bank Manager: Format and Samples

Ready wording for a letter asking a bank to change or correct the name on an account: after marriage, after a Gazette change, for a spelling slip, and where a branch asks for more than the rule requires.

Quick answer

A name change request letter to a bank manager states the account, the name as it stands, the name as it should read, the reason, and the papers enclosed. Enclose a marriage certificate or a Gazette notification with your identity proof, as the Reserve Bank's rule names, sign it, and take a stamped acknowledgement.

A bank customer in Uttarakhand handing a one-page name change request letter with enclosures to a branch officer for a stamped acknowledgement
A name change request letter to a bank manager fits on one page: the account, both names in capitals, the reason, the enclosures and a stamped copy for you.

A name change request letter to bank manager is a one-page paper, and most people overthink it. A bank needs five facts and the right enclosure, not a long story. This page gives the format of a name change request letter to bank manager, with four sample letters you may copy and adapt.

The letters are free to use. We do not write to banks for anyone. Our work is the Gazette notification, which one of the letters refers to. For the rule behind them, read name change in bank records in Uttarakhand.

When Do You Need a Letter?

  • Where the bank has no form. Many branches ask for a plain application for a change of name.
  • With the bank's form. A form has boxes. A covering letter says what you want and lists what you have enclosed.
  • Where you want a record. A letter with the branch's stamp and date on your copy is proof that you asked, and when.
  • Where the branch has refused orally. A written request has to be answered. A spoken one does not.

Ask the branch first if it has its own form for a change in customer details. If it has, fill that, and attach one of the letters below as a covering note. The letter never replaces a form the bank insists on.

What Must Every Letter Contain?

Parts of a name change request letter to a bank
PartWhat to write
Date and placeThe day you hand it in
ToThe Branch Manager, the bank's name, the branch and its address
SubjectOne line: change or correction of name, with the account number
Who you areYour name as the bank has it, the customer number and every account number
The two namesThe name as it stands in the bank's record, and the name as it should read, each in capitals, letter by letter
The reasonMarriage, a change of name notified in the Gazette, or a mistake in the record. One sentence
What you ask forThat the name be changed in all records, and a new passbook, cheque book and cards issued
EnclosuresA numbered list of every paper attached
SignatureAs on the bank's record, and the new signature if it has changed, with your mobile number

That is all. Do not explain your family history, and do not apologise. A clerk reading forty requests a day acts fastest on the one that is shortest and complete.

What Does a Letter for a Spelling Correction Look Like?

Date: [date]

To
The Branch Manager
[Name of bank]
[Branch and address]

Subject: Correction of spelling of name in account number [account number]

Sir or Madam,

I hold savings account number [account number], customer number [customer number], at your branch.

My name is recorded in the bank's records as [NAME AS RECORDED]. The correct spelling, as in my identity documents, is [CORRECT NAME]. I have not changed my name. The difference is an error in the record.

2. Self-attested copy of PAN
3. Passbook, unused cheque leaves [numbers] and debit card

[Correct name]
Mobile: [number]

The sentence "I have not changed my name" matters. It tells the branch this is a correction, for which the Reserve Bank's two papers are not the point, and your identity document is.

What If the Branch Asks for More Than the Rule Requires?

A branch sometimes tells a married woman to bring a Gazette, or tells a customer with a Gazette to bring Aadhaar in the new name first. The Reserve Bank's directions do not require either. This letter says so, politely.

Date: [date]

To
The Branch Manager
[Name of bank]
[Branch and address]

Subject: Request for change of name in account number [account number]: papers already submitted

Sir or Madam,

On [date] I applied for a change of my name in the above account from [OLD NAME] to [NEW NAME], and enclosed [the marriage certificate issued by the State Government or the Gazette notification] with a copy of my [identity document] in my existing name. I was told at the counter that [what you were told].

I respectfully draw your attention to the Reserve Bank of India's directions on Know Your Customer. The explanation to the definition of Officially Valid Document provides that a document shall be deemed to be an officially valid document even if there is a change in the name subsequent to its issuance, provided it is supported by a marriage certificate issued by the State Government or Gazette notification, indicating such a change of name.

The papers I have submitted meet this requirement. I request you to carry out the change, or to let me know in writing the reason it cannot be done and the provision relied on.

Enclosures:
1. Copy of my application dated [date], with your acknowledgement
2. Copies of the papers submitted with it

Yours faithfully,

[Signature]
[Name]
Mobile: [number]

Use this only after a first request has been made and refused. Keep it civil. Its purpose is to get either the change or a written reason, and a written reason is what a complaint needs.

The directions have been reissued since the text quoted. The wording has been carried forward, but check the current direction on the Reserve Bank's website before you cite a clause number.

  • Carry the originals. The officer sees them and returns them.
  • Number the enclosures in the letter in the same order as they are pinned.

How Do You Write and Submit the Letter?

Step 1: Ask the branch if it has its own form

If it has, fill it, and use the letter as a covering note.

Step 2: Choose the sample that fits

Marriage, a Gazette change or a spelling correction. Do not mix them.

Step 3: Copy the names from the papers, in capitals

The old name from the passbook. The new name from the marriage certificate, the Gazette or your identity document.

Step 4: List every account

Savings, deposits, loans and the locker. One letter can cover them all.

Step 5: Type or write it on one page, and make two copies

One for the bank, one for you.

Step 6: Sign both copies, and pin the enclosures

In the order the letter lists them.

Step 7: Hand it in at the home branch, with the originals

Go yourself. A change of name is not done on a letter brought by someone else.

Step 8: Get your copy stamped and dated

With the branch's seal and the receiving officer's initials, or a service request number.

How Should You Sign?

  • Sign the letter the way the bank knows you, as on the account opening form. The bank checks the request against that signature.
  • If your signature will change with your name, sign a second time in the new form, with the new name written under it.
  • The bank will take the new signature on its own card as well. Sign it at the counter, in front of the officer.
  • If your signature is in Hindi, sign as you always do.
  • A person who cannot sign gives a thumb impression, which the bank attests in its own way. Ask the branch.

Use the new signature for every cheque after the change, and never both. The letter may be written in Hindi or in English. Write the names in English capitals as well, as they appear on the identity papers, since that is how the bank's system holds them.

What About Joint Accounts, Minors and Firms?

A joint account

The holder whose name has changed writes the letter. Add a line that the other holder has no objection, and have that holder sign below. Some banks require it.

A minor's account

The parent or guardian who operates the account writes and signs, and encloses the child's birth certificate or the Gazette notification for the child, with the guardian's own identity proof.

A proprietor's or firm's account

A change in the proprietor's own name needs the same papers, and the business registrations in the new name as well. A change in the firm's name is a different matter, with its own papers.

For an account of someone who has died, a letter asking for a change of name is the wrong paper. The nominee or heirs make a claim. See name correction in a death certificate and legal heir certificate.

How Do You Follow Up?

  1. Ask at the counter how long it takes. Note the answer on your copy.
  2. Check the passbook or internet banking after that time.
  3. If nothing has changed, write a short reminder, quoting the date and number of your first letter, and get that stamped too.
  4. If it is still not done, or is refused, ask for the reason in writing, and send Sample 4.
  5. Then write to the bank's grievance officer, whose address is on the bank's website and in the branch.
  6. Then the Reserve Bank's ombudsman, online, where the bank has not resolved it in the time allowed.

A reminder, in two lines: "I refer to my application dated [date], acknowledged by your branch on [date], for change of my name in account number [account number] from [OLD NAME] to [NEW NAME]. The change has not yet been made. I request you to carry it out, or to inform me in writing of the reason for the delay."

How Do You Get the Gazette Notification the Letter Refers To?

Sample 2 quotes a Gazette notification. This is how one is obtained from the Gazette of India. A surname on marriage and a spelling correction do not need it.

Step 1: Confirm that yours is a change of name, not a marriage or a spelling

For those, use Sample 1 or Sample 3.

Step 2: Fix the exact new name

The spelling that will go into the letter, Aadhaar and PAN.

Step 3: Make the affidavit

State the old name, the new name, your father's or husband's name and your address. Sign before a notary.

Step 4: Publish the newspaper notice

In a daily local leading newspaper. Keep the original newspaper.

Step 5: Type the form and sign the undertaking

Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.

Step 6: Prepare the CD, photographs and certificate

The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.

Step 7: Pay the charge on BharatKosh

Commonly Rs 1,100 for an adult. It is not refunded.

Step 8: Send the file yourself, then write the letter

Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. When the Gazette is published, copy its date and page into Sample 2.

See gazette name change in Uttarakhand.

Can the Letter Quote the Uttarakhand Gazette?

The Reserve Bank's rule says Gazette notification, without naming one. A letter can quote a state notice. PAN and the passport are why most people choose the Gazette of India.

Step 1: Ask your branch if it accepts a state Gazette

Before you choose.

Step 2: Think about PAN, the passport and other banks

The Gazette of India serves them all.

Step 3: Contact the Roorkee press

Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.

Step 4: Ask for the papers, the format and the fee, and submit

Keep photocopies of everything.

Step 5: Download the notice, and quote it in the letter

Search gazettes.uk.gov.in, save the PDF, and write "Uttarakhand Gazette" with its date in Sample 2.

To weigh the two, read Uttarakhand Gazette vs Gazette of India.

A typical case we handle: a retired teacher in Almora had written three pages to her bank explaining how her name came to be different on her passbook and her Aadhaar, and had heard nothing. Her passbook left out a surname that every identity paper carried. We suggested Sample 3: one page, both spellings in capitals, the sentence that she had not changed her name, and a copy of her Aadhaar and PAN. She handed it in and took a stamped copy. The passbook was corrected on her next visit. This is an illustrative example of the type of case we handle, not a named client review.

When you do not need us

To write the letter, never. Copy a sample from this page. We are useful only where your change of name needs the Gazette notification that Sample 2 refers to.

Should I sign in my old name or my new name?

Both. As the bank knows you, and then the new signature.

What if the bank does not act on my letter?

Send a reminder, ask for the reason in writing, then go to the grievance officer.

Can a bank mitra or business correspondent change my name?

No. Under the Reserve Bank's KYC amendment of 12 June 2025, a business correspondent may take only a self-declaration where there is no change in KYC information or a change only in the address. A change of name goes to the branch, with your letter.

Will the bank remind me before my KYC falls due?

Yes. Before the due date for periodic updation of KYC, a bank has to give at least three advance intimations, including at least one by letter. Update your name at the same time if it has changed.

Which Reserve Bank directions does Sample 4 rely on?

The Reserve Bank of India (Know Your Customer (KYC)) Directions, 2016, as amended, most recently by the Amendment Directions of 12 June 2025. Check the current text on the Reserve Bank's website before you cite a paragraph.

Who is responsible for keeping my KYC up to date?

The bank. The 2025 amendment says the ultimate responsibility for periodic updation of KYC remains with the bank concerned, even where a business correspondent collects the papers.

Was there extra time for low-risk customers to update KYC?

Yes. For low-risk individual customers, the June 2025 amendment let all transactions continue and asked banks to complete KYC updation within one year of its falling due, or up to 30 June 2026, whichever was later.

Do banks hold KYC camps in villages?

The Reserve Bank asked banks to hold special camps and campaigns for KYC updation, especially in rural and semi-urban branches. Take your name change letter and its enclosures to such a camp only if the branch confirms names are changed there.
