Documents Required for Name Change After Marriage in Uttarakhand
One checklist for each route and each office, so you carry the right papers the first time and pay for nothing you do not need.
The documents required for name change after marriage depend on your route. With a registered marriage certificate, most offices ask for the certificate, your identity proof in the maiden name and your husband's identity proof. The Gazette route needs more: an undertaking, a newspaper notice, the typed form, a CD, photographs and the fee receipt.

Women lose weeks after the wedding because each office asks for a slightly different set of papers. This page lists the documents required for name change after marriage in Uttarakhand, route by route and office by office. Use it to check the documents required for name change after marriage before you leave home.
To decide which route is yours, read name change after marriage in Uttarakhand first. Remember that a change of name is not compulsory, and if you keep your maiden name you need none of these lists.
Why Does Your Route Decide the Documents?
There are two ways to carry a new name into your records after marriage, and they use different papers.
| Route | Main proof | Fits when |
|---|---|---|
| Marriage certificate route | The registered marriage certificate | The marriage is registered and only your surname changes, to your husband's |
| Gazette route | A Gazette change of name notification | There is no certificate, your first name changes too, the names do not match, or an office refuses the certificate |
Most women are on the first route and never need the second list on this page. If you are told you need a Gazette, ask why, and check your case against the second row before you pay.
Which Documents Should Every Married Woman Keep Ready?
These papers are asked for again and again, on either route. Put them in one folder.
Yours
- The registered marriage certificate, original and copies
- Aadhaar in your maiden name
- PAN card
- Class 10 certificate, as proof of your date of birth and maiden name
- Passport, if you hold one
- Recent passport size photographs
- Proof of your present address
Your husband's
- Aadhaar
- PAN card
- Passport, if he holds one
- Proof of the address you now share
About the wedding
- The wedding card and a few photographs, kept as back-up
Scan everything once and keep the scans in your email. Half the time an office wants an upload, not a paper.
Which Documents Are Needed to Register the Marriage in Uttarakhand?
The marriage certificate is the paper the simple route rests on. In Uttarakhand, marriages are registered under the Uniform Civil Code, which came into force on 27 January 2025. A marriage is to be registered within 60 days, and registration is done on ucc.uk.gov.in or with help from a Common Service Centre.
The portal shows the exact list to upload. In practice, keep these ready for both spouses:
- Proof of identity and age
- Proof of address
- Passport size photographs
- Proof of the marriage, such as the wedding card or photographs of the ceremony
- Details and identity proof of the witnesses
Follow the portal where it differs from this list. It is the authority, and its requirements can change.
We do not register marriages. You do it yourself on the Government portal. It is on this page because every other list depends on it.
Which Documents Does Each Office Ask for After Marriage?
This is the usual position for a wife taking her husband's surname with a registered marriage certificate. Each office keeps its own list and can change it, so check that office's current list before you go.
| Office | Usually asked for | How you apply |
|---|---|---|
| Aadhaar | Marriage certificate, your Aadhaar, and identity proof as its list describes | In person at an Aadhaar Seva Kendra |
| PAN | Updated Aadhaar, and proof of the change such as the marriage certificate | PAN correction application, online |
| Bank | Updated Aadhaar and PAN, marriage certificate, a request form and a new specimen signature | At the branch |
| Passport | Current passport and the papers on the Passport Seva list for a change after marriage | Re-issue application online, then the Passport Seva Kendra |
| Voter ID | Form 8, with proof of the change | Online or through the booth level officer |
| Driving licence | Licence, proof of the change and updated identity proof | Transport office, as the Parivahan service directs |
| Employer and provident fund | Marriage certificate and updated Aadhaar and PAN, on the employer's form | Through your employer |
| Insurance and investments | Updated Aadhaar and PAN, marriage certificate and the company's form | With each company |
Notice the pattern. Aadhaar comes first, and almost every later office then asks for "updated Aadhaar and PAN" plus the marriage certificate. That is why the order matters more than any single list.
How Do You Collect and Use the Documents on the Certificate Route?
Step 1: Get the marriage certificate and read it
Check both names, the date of marriage and the address on it.
Step 2: Make the base folder
Put in the certificate, your identity papers, your husband's identity papers, photographs and address proof, with scans of each.
Step 3: Fix the exact married name
Write down the spelling and spacing you will give every office.
Step 4: Update Aadhaar with the certificate
Carry the originals to the Aadhaar Seva Kendra. Keep the acknowledgement slip.
Step 5: Update PAN with the new Aadhaar
File the PAN correction application once the Aadhaar update shows.
Step 6: Take both to the bank
Updated Aadhaar and PAN, the marriage certificate and the bank's own request form.
Step 7: Work through the remaining offices
Passport, voter ID, driving licence, employer, insurance and investments, each with its own form.
Aadhaar allows a name change only twice in a lifetime, so be sure of step 3 before step 4.
Which Documents Do Aadhaar and PAN Need?
Aadhaar
- Your Aadhaar number
- The marriage certificate, in the form the UIDAI list of supporting documents describes
- Your identity proof, in original, for the operator to see
- Your mobile number, linked to Aadhaar where possible
The Aadhaar rules list a surname change after marriage with the marriage certificate among the updates that need no Gazette. A change of first name is different, and needs the Gazette.
PAN
- Your existing PAN
- The updated Aadhaar
- Proof of the change of name. The form's own instructions list proofs for a married woman, such as the marriage certificate or a Gazette publication
- A photograph and signature, as the application asks
You keep the same PAN number. Only the name on the card changes. Never apply for a second PAN.
Do Aadhaar first and wait until the new name shows before you file for PAN. The two are matched against each other, and a PAN application in a name Aadhaar does not yet carry is likely to fail.
Which Documents Do the Bank and the Passport Office Need?
Bank
- The bank's request form for a change of name
- Updated Aadhaar and PAN
- The marriage certificate
- Your passbook, cheque book and cards, for reissue
- A new specimen signature, if your signature is changing
A useful rule sits in the Reserve Bank's directions on customer identification. An identity document issued in your earlier name is still treated as valid when it is supported by a marriage certificate issued by the State Government, or by a Gazette notification, showing the change of name. So for a plain surname change, either one of the two supports the change.
Passport
- Your current passport, in original
- The papers the Passport Seva document list gives for a change of name after marriage
- Your husband's passport, if he holds one and the list asks for it
- Updated identity and address proof
A passport is reissued, not corrected by hand. Passport Seva has its own document advisor and declaration formats, and they are revised from time to time. Open the advisor for your exact case before you book the appointment.
Which Documents Does the Gazette Route Need After Marriage?
Only where your case needs a Gazette. This is the list in the Department of Publication's guideline for a change of name in the Gazette of India, Part IV.
| Document | What it must be |
|---|---|
| 1. Undertaking | Signed by you, showing your old name and your new name |
| 2. Original newspaper | Your change of name printed in a daily local leading newspaper, with your husband's name and your address |
| 3. Prescribed form | Computer typed, in two copies, signed by you in your old name and by two witnesses |
| 4. CD | The same text in MS Word, without the witness part, with your old name typed in place of the signature |
| 5. Photographs and identity proof | Two self-attested passport size photographs and a self-attested copy of photo identity proof |
| 6. Certificate | Signed by you, that the hard copy and the soft copy carry the same contents |
| 7. Request letter with the fee | To the Controller of Publications, with the BharatKosh receipt |
To these, add a copy of the marriage certificate if you have one, as supporting proof of the reason. The guideline does not list a notarised affidavit. It is still made first in practice, because newspapers and the offices that update your records later often ask for it. See name change affidavit after marriage.
How Do You Put the Central Gazette File Together After Marriage?
Step 1: Confirm that you need a Gazette
No certificate, a new first name, mismatched names, or a written refusal of the certificate by an office.
Step 2: Publish the newspaper notice
Give your maiden name, married name, husband's name and address. Keep the original newspaper.
Step 3: Type the form and the undertaking
Two typed copies of the form, signed in your old name with two witnesses, and the signed undertaking.
Step 4: Make the CD and sign the certificate
The same words in MS Word, and the certificate that both copies match.
Step 5: Add photographs, identity proof and the marriage certificate copy
Self-attest the photographs and the identity copy.
Step 6: Pay the charge and write the request letter
Pay on BharatKosh. The charge is commonly Rs 1,100 for an adult, and giving both "daughter of" and "wife of" adds Rs 250.
Step 7: Send the file yourself
In person or by post, to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Then check Part IV on egazette.gov.in.
For each paper in detail, see documents required for gazette name change in Uttarakhand.
Which Documents Does the Uttarakhand Gazette Ask for After Marriage?
The Government Press at Roorkee sets its own list, and it has not published one for private notices that we can quote. Ask first.
Step 1: Check where the new name will be used
For a passport, a central job or a move out of the state, use the Gazette of India.
Step 2: Get the paper list from the press
Take the contact from govtpress.uk.gov.in and ask which papers are needed, in which language and in how many copies.
Step 3: Keep the common papers ready
The marriage certificate copy, your identity proof, the affidavit if the press asks for one, and the newspaper notice.
Step 4: Pay the state fee and keep the receipt
Pay the way the press directs. State fees are not paid on BharatKosh.
Step 5: Submit and keep a full photocopy
Then save the published PDF from gazettes.uk.gov.in.
To weigh the two routes, read Uttarakhand Gazette vs Gazette of India.
What If Your Papers Do Not Match Each Other?
| The mismatch | What to do first |
|---|---|
| Your maiden name is spelt differently on the marriage certificate and on Aadhaar | Get the wrong one corrected at its own office before any name update |
| Your husband's name on the certificate differs from his Aadhaar | Correct the certificate or his record first |
| The marriage certificate already shows your married name, with no maiden name | Ask the office if it will accept it. If not, the Gazette route may be needed |
| Some papers are already in the married name and some in the maiden name | Decide the final name, then use the certificate, or a Gazette notice with an alias |
| Your first name on the wedding card is a new name given by the family | That is a change of name. Plan for the Gazette |
| The marriage was never registered | Register it now, or take the Gazette route |
The rule in every row is the same: repair the base paper first. An update built on a certificate with a wrong letter carries that letter into Aadhaar, PAN and the bank, and each has to be corrected again later.
Where both names are already in use, see alias name in a gazette notification.
Which Papers Do You Not Need?
For a surname change with a registered marriage certificate, these are often demanded or sold, and are not needed.
- A Gazette notification. Not for a plain surname change with a registered certificate, on Aadhaar, PAN or at the bank.
- A newspaper notice. It belongs to the Gazette route only.
- A court order. A name change after marriage is not a court matter.
- A lawyer's letter. No advocate's certificate is needed.
- New degrees and marksheets. They stay in your maiden name and remain valid.
- A new PAN number. The existing PAN is corrected.
- A written consent from your husband or his family. Your name is your own choice. An office may ask for his identity proof, which is a different thing.
- A "name change certificate". There is no such paper.
If someone lists any of these in a quote for a simple surname change, ask what exactly it is for.
Originals, Copies and Attestation: What Do You Carry?
Carry
- Originals, to show at the counter
- Two or three photocopies of each, to leave behind
- Photographs
- A pen, and the exact spelling of your new name on a slip of paper
Self-attest a copy by
- Writing "self-attested" on it
- Signing below, the way you sign on that day's identity proof
- Writing the date
- Never leave your only original marriage certificate with an office unless it gives a written receipt. Keep several printed copies, and the digital file if the portal issued one.
- Never post an original identity document.
- Keep every acknowledgement slip and reference number until the new document arrives.
- After each update, download or collect the new document and add it to the folder before you go to the next office.
Which Document Mistakes Cause Delay After Marriage?
| Mistake | How to avoid it |
|---|---|
| Going to the bank before Aadhaar and PAN are updated | Aadhaar, then PAN, then the bank |
| A marriage certificate with a wrong spelling | Read it on the day and correct it before use |
| Carrying copies and no originals | Offices check originals at the counter |
| Only one copy of the marriage certificate | Keep several copies, and the digital file if the portal issued one |
| A different spelling of the new name at each office | Carry the spelling written down |
| Using the wedding card where the certificate is asked for | Register the marriage and use the certificate |
| A Gazette file with papers older than one year | Check the dates before posting |
| Applying for a fresh PAN in the married name | Correct the existing PAN |
| Paying for Gazette papers that the certificate route never needed | Decide the route first |
A typical case we handle: a woman in Haridwar went to her bank with her marriage certificate and was sent away twice. Her Aadhaar and PAN were still in her maiden name. She updated Aadhaar, then PAN, and the bank changed her name on the third visit with the same certificate. This is an illustrative example of the type of case we handle, not a named client review.

It is reviewed by Advocate Vipin Chauhan.

The office by office lists are the usual position and not official lists, because each office sets and revises its own. Documents for Name Change After Marriage: The Short Version
Decide the route first. With a registered marriage certificate and only a surname change, carry the certificate, your identity proof and your husband's identity proof to the Aadhaar Seva Kendra, then use the updated Aadhaar for PAN, and both for the bank and the passport. No Gazette, newspaper or court paper is needed.
The Gazette route has its own seven papers and is for cases with no certificate, a new first name or mismatched names. Check three spellings before you start, and repair the base paper first if they differ. To have your papers checked, send photos on WhatsApp 9540005064, and we will reply in working hours.
