Name Change in ESIC Records in Uttarakhand
For factory and shop workers covered by employees' state insurance: when the insured person's name, or a wife's or a child's, does not match Aadhaar and treatment or a cash benefit is held up. Who corrects it, on what proof, and what to do before changing jobs.
A name change in ESIC records is asked for through the employer's portal or the insured person's own, and approved by the branch office. The corporation's circular says a beneficiary can change personal details to match Aadhaar that way, or at the branch office with documents. Correct Aadhaar first if it is the wrong paper.

Many workers in the state's industrial estates, in Haridwar, Pantnagar, Selaqui and Kashipur, are covered by employees' state insurance, and so are their families. A name change in ESIC records is needed when the name an employer once typed no longer matches Aadhaar. This page explains name change in ESIC records for insured workers in Uttarakhand.
We do not deal with the corporation or with employers, and this subject rarely needs a Gazette. The position on Aadhaar is from the corporation's circulars as published and reported. The portal steps are as guides describe them. For the provident fund, read name change in EPFO and UAN records.
What Is the Insurance, and Which Names Does It Record?
- What it is. A social insurance for employees of covered factories and establishments who earn within the wage limit the law sets. It gives medical care to the worker and family, and cash benefits in sickness, maternity, injury at work and death.
- Who runs it. The Employees' State Insurance Corporation, through regional and branch offices, dispensaries and hospitals.
- The insured person. The employee, who is given an insurance number for life.
- The card. An identity card for the insured person and family. Guides say a temporary certificate is issued first and becomes the permanent card once the worker's Aadhaar is linked.
- Names on the record. The insured person, the father's or husband's name, each family member with relationship and date of birth, and a nominee.
The employer enters all of this when the worker joins. Most wrong names begin there, with a personnel clerk typing from a form filled in a hurry on the first day.
What Does the Corporation Say About Aadhaar?
From a circular of the corporation's headquarters dated August 2024, as published, and a 2023 clarification as a law firm reports it. Check the corporation's site for later instructions.
| Point | Position |
|---|---|
| Linking Aadhaar | Done through the insured person's portal or the employer's portal, for the worker and the family members. Employers can also link in bulk |
| Where the details do not match | The circular says Aadhaar will not be linked in case of a mismatch |
| Changing details to match Aadhaar | A beneficiary who wishes to change personal details as per Aadhaar may do so through the insured person's or the employer's portal, or contact the designated branch office with the documents required |
| Small changes | Reported as approved by the branch office manager on an Aadhaar-based change request |
| Several discrepancies at once | Where name, father's name, gender and date of birth all differ, the manager may return the request, noting the discrepancies, and make enquiries with the employer and employee |
| Where Aadhaar itself is wrong | Corrected at an Aadhaar centre, not by the corporation |
So the corporation treats Aadhaar as the measure. A record that differs from it by a letter is brought into line with little fuss. A record that differs in several ways at once looks like a different person, and is examined.
Why Does the Name Matter?
- At the dispensary or hospital. The card is read with identity proof before treatment. A wife or a child whose name on the card differs from her Aadhaar may be turned back.
- Treatment in another state. A guide notes that a family whose Aadhaar is not linked cannot use the card at a dispensary in another state. That matters to a worker whose family lives in the village, or in the home state.
- Cash benefits. Sickness, maternity and disablement benefits are paid to the insured person's bank account, in the insured person's name.
- Referral to a larger hospital. The referral papers and the hospital's record are made from the card.
- A claim after a death. Dependants are paid as the record names them.
A worker finds out on the day somebody is ill. That is the wrong day to begin a correction.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The record misspells the insured person, and Aadhaar is right | A change request through the portal, on Aadhaar | No |
| Aadhaar is the wrong one | Correct Aadhaar, then the record | No |
| A wife's or a child's name on the card differs from her Aadhaar | The employer updates the family particulars | No |
| A woman worker married and changed her surname on Aadhaar | A change request, with the marriage certificate | No |
| Name, father's name and date of birth all differ from Aadhaar | The branch office, with documents, in person | Sometimes |
| The worker has two insurance numbers from two jobs | The branch office, to bring them together | No |
| The worker has changed a name by choice | Gazette, then Aadhaar, then the record | Yes, for Aadhaar |
A Gazette appears twice, and in neither is it the corporation that asks for it by name. It is Aadhaar that needs it, or a branch office that wants one paper to settle several differences.
How Do You Get the Record Corrected?
As guides to the corporation's portals describe it. The screens change, so follow the portal as it stands.
Step 1: Get a copy of your card or your record
From your employer, or from the insured person's portal with your insurance number.
Step 2: Put Aadhaar beside it, for yourself and each family member
Compare the name, father's or husband's name, date of birth and gender.
Step 3: Correct Aadhaar first, if it is the odd one
The corporation's record is brought to Aadhaar, not the other way.
Step 4: Ask your employer's personnel office to update the particulars
Guides say the employer does this under the option to update particulars of an insured person. Give them copies of the proof.
Step 5: Or raise the request yourself on the insured person's portal
Where it offers the change you need.
Step 6: Note the reference, and wait for the branch office's approval
Name and date of birth changes are verified there.
Step 7: If it is returned or stalls, go to the branch office
With your card, Aadhaar, the supporting papers and a letter from your employer.
Step 8: Get a fresh card, and check every name on it
Then link Aadhaar for each family member.
What Proof Goes With Each Kind of Change?
As guides list it. The branch office's own list governs.
| Change | Proof listed |
|---|---|
| The insured person's name | Aadhaar, PAN or the employer's certificate |
| Date of birth | The birth certificate, the class 10 marksheet or Aadhaar |
| A dependant's or nominee's details | That person's Aadhaar or birth certificate |
| A married name | The marriage certificate, with Aadhaar in the new name |
| A name changed by choice | Aadhaar in the new name, with the Gazette notification behind it |
An employer's certificate carries weight here, because the insurance exists through the employment. Ask the personnel office for a letter stating that the person on its rolls and the person on the Aadhaar card are one.
What About Family Members on the Card?
- The card covers the insured person's family as the law defines it: usually the spouse, dependent children and dependent parents.
- Guides say an employee cannot add or change family members directly. The employer does it on its portal, on the worker's declaration.
- Each member is entered with name, relationship and date of birth. Each should be exactly as on that member's own Aadhaar.
- A wife entered by a home name, a child entered before a name was fixed, and parents entered from memory are the usual faults.
- A new wife or a newborn is added on the marriage certificate or the birth certificate. A member who has died, or a daughter who has married, is removed.
- A family living in the village, or in another state, should have Aadhaar linked for each member, or the card may not serve them there.
Read name change in Aadhaar card and health insurance policy name correction, for the same point in private cover.
What Happens When You Change Jobs?
- The insurance number is meant to stay with the worker for life. A new employer should register the worker under the same number.
- Where the new employer types the name differently and cannot find the old record, a second number is sometimes created. Contributions then sit in two places.
- Give every new employer your insurance number and a copy of your card on the first day.
- A correction is asked for through the present employer. A former employer has no access.
- Two numbers are brought together at the branch office. Take both cards and your Aadhaar.
- A worker between jobs, or who has left covered work, goes to the branch office directly.
The same worker usually has a provident fund account with the same name trouble. Do both in one effort. Read name change in EPFO and UAN records.
What About Cash Benefits and the Bank Account?
- Sickness, maternity and disablement benefits are paid into the insured person's own bank account.
- The account's name should match the record and Aadhaar. A difference can send the payment back.
- A maternity benefit is claimed by the insured woman herself. Where she has changed her surname on one paper and not the others, the claim is the moment it shows.
- A medical certificate from the dispensary is in the name on the card. See that it is the name on the claim.
- Correct the record before a claim if you can. A claim made under a wrong name is paid, if at all, after the correction.
What Happens After the Insured Person's Death?
- Where death results from an injury at work, the dependants are entitled to a benefit. A sum towards funeral expenses is paid in any case of death.
- The claim is made at the branch office by the widow, children or other dependants, with the death certificate and their own identity papers.
- The dependants are compared with the family particulars on the record. A widow whose name there differs from her Aadhaar has to show both are hers.
- The dead worker's name on the record is compared with the death certificate. Where they differ, his Aadhaar and the employer's records tie them. A Gazette cannot be taken out for a person who has died.
- The employer's help matters most here. Ask for it in writing.
All of this is far simpler where the family particulars were right while he lived. Read name correction in a death certificate and legal heir certificate.
Where Does a Gazette Come In?
Rarely. It comes in where a worker has changed a name by choice and Aadhaar needs a Gazette for that. The corporation's record then follows Aadhaar.
Step 1: Confirm that it is a change of name, and not a spelling
A spelling is corrected on Aadhaar through the portal.
Step 2: Fix the exact new name
The spelling that will go on Aadhaar, the record and the provident fund.
Step 3: Make the affidavit
State the old name, the new name, your father's or husband's name and your address. Sign before a notary.
Step 4: Publish the newspaper notice
In a daily local leading newspaper. Keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update in order
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. With the published Gazette, update Aadhaar, tell your employer, and ask for the record to be changed.
That is a real cost for a worker on a factory wage. Be sure it is a change of name you want. See gazette name change in Uttarakhand.
Does the Uttarakhand Gazette Matter Here?
Not to the corporation, which follows Aadhaar. The question is only which Gazette Aadhaar accepts.
Step 1: Ask the Aadhaar centre which Gazette it accepts
The record follows Aadhaar.
Step 2: Think about the provident fund, the bank and work in other states
The Gazette of India serves everywhere.
Step 3: Contact the Roorkee press
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.
To weigh the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Do Workers Make?
| Mistake | What to do |
|---|---|
| Never looking at the card after joining | Read it in the first week |
| Letting the clerk enter the family from memory | Give a copy of each member's Aadhaar |
| Changing the record while Aadhaar is wrong | Aadhaar first |
| Paying for an affidavit or a Gazette to fix a spelling | A change request on Aadhaar is enough |
| Not telling a new employer the old insurance number | Give it on the first day |
| Leaving the family's Aadhaar unlinked | Link each member |
| Starting the correction on the day of an illness | Do it while everyone is well |
| Paying a contractor's man to "get it done" | Use the employer's personnel office or the branch office |
| Fixing the insurance and forgetting the provident fund | Do both together |
A typical case we handle: a machine operator at a unit in Haridwar took his wife to the dispensary and was sent back. The card gave her as "Guddi", and her Aadhaar as "Sunita Devi". A man at the gate offered to arrange a Gazette. She did not need one. Her own name had never changed. His personnel office updated the family particulars on her Aadhaar and their marriage certificate, and linked her Aadhaar. He lost a week, and nothing else. This is an illustrative example of the type of case we handle, not a named client review.
