Inner Line Permit Name Mismatch in Uttarakhand: Adi Kailash and Border Valleys
For travellers to Adi Kailash, Om Parvat and the state's other border valleys whose identity paper, police verification, medical certificate and affidavit do not carry one name: what the permit is, who issues it, where each paper is put right, and why it has to be done before the long road to Dharchula.
An inner line permit name mismatch is avoided by writing every paper from one identity paper: the police verification, the medical certificate and the affidavit, each in the same name, father's name and age. Check them side by side before leaving home. A slip is corrected by whoever issued that paper. No Gazette is needed.

The road to Adi Kailash and Om Parvat runs close to the border, and a traveller needs a permit to go beyond Dharchula. The permit office reads four papers written in four places. An inner line permit name mismatch is those four not agreeing, discovered far from home. This page explains inner line permit name mismatch, and how to avoid it.
We do not arrange permits, and this subject needs no Gazette. Who issues the permit and which papers are asked for are from the state tourism corporation's own tour terms, a news report and travel guides. We did not read any official order. For the Char Dham, read Char Dham Yatra registration name mismatch.
What Is the Permit, in Brief?
- What it is. Written permission to travel beyond a line drawn short of the international border.
- Where it applies. The road above Dharchula to Gunji, Adi Kailash and Om Parvat, and some other high valleys of the state.
- Who issues it. The sub-divisional magistrate for the area.
- What it names. The traveller, with father's name and age, and the days allowed.
- Where it is shown. At check posts along the road, kept by the security forces.
It is a security paper before it is a tourist paper. That is why it asks who you are, who your father is, and what your own police station says of you.
What Is Published?
From the state tourism corporation's terms for its Adi Kailash tours, a news report on permits in Chamoli, and travel guides. The permit office has the present requirements.
| Point | What is published |
|---|---|
| Who issues | For Adi Kailash and Om Parvat, the sub-divisional magistrate at Dharchula. For Chamoli's border areas, the office at Joshimath |
| Police verification | A form signed and stamped by the traveller's local police station. Not required where a passport is submitted as the identity paper and the original is carried |
| Affidavit | A notarised affidavit, as an indemnity bond, for the Adi Kailash journey |
| Medical | A certificate of fitness |
| The permit | Guides say it carries the name, father's name, age, permit number and the days allowed |
| How to apply | At the office, or as guides say through an online portal for the district |
One guide adds, from experience, that name mismatches across the identity paper, the medical certificate and the application form are a common cause of delay. That is the subject of this page.
Which Four Papers, Written by Whom?
| Paper | Written by | Who corrects it |
|---|---|---|
| The identity paper | Its issuing authority | That authority, not in a week |
| The police verification | A constable or officer at the home police station | The same police station |
| The medical certificate | A doctor, by hand | The same doctor, or a fresh certificate |
| The affidavit | A typist, before a notary | A fresh affidavit |
| The application | The traveller, or an agent | The traveller |
| The permit | The permit office, from all the above | The permit office |
The first row cannot be changed in time for a journey. So every other paper is brought to it. A traveller who settles that before leaving has nothing to settle at Dharchula.
What Kinds of Mismatch Are There?
A certificate in a loose name
- A constable or a doctor wrote what he heard
- A short form, an initial, a missing surname
- Corrected by the same hand, before travel
The father's name or the age differs
- The permit carries both
- A certificate gives the father as the family says him, or a rounded age
- Written from the identity paper
Two identity papers in play
- The verification was made on Aadhaar, the application on a voter card
- The two cards differ from each other
- One card for everything
The first is the commonest. A doctor filling many certificates for a tour group writes names as they are called out. Nobody reads them until the permit window.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The police verification has a short name | A corrected one from the same police station | No |
| The medical certificate has a slip | The doctor corrects it, or a fresh certificate | No |
| The affidavit has a slip | A fresh affidavit | No |
| The father's name differs between papers | All written from the identity paper | No |
| Two identity papers disagree | Use one for everything. Correct the other afterwards | No |
| A married name on the application, a maiden name on the identity paper | Apply in the identity paper's name | No |
| The permit itself has a slip | The permit office, before leaving the town | No |
| The traveller changed a name by choice | Apply in the name the identity paper now carries | For the papers, not the journey |
No journey needs a Gazette. Each paper is corrected by the hand that wrote it.
How Do You Prepare the Papers?
This is the general method. Begin two or three weeks before the journey.
Step 1: Choose the one identity paper you will apply on and carry
Aadhaar, a voter card or a passport. Everything else is written from it.
Step 2: Write your name and your father's name from it, on a slip of paper
Take the slip to the police station, the doctor and the notary.
Step 3: Get the police verification in exactly that name
At your own police station, within the time the permit office asks.
Step 4: Get the medical certificate in exactly that name
Ask the doctor to write it in capitals, with your age as on the identity paper.
Step 5: Have the affidavit typed in exactly that name
And read it before the notary signs.
Step 6: Lay the four papers side by side before you travel
Identity paper, verification, medical certificate, affidavit. One name, one father's name, one age.
Step 7: Apply, online or at the permit office, from the identity paper
And read the permit when it is issued.
Step 8: Carry the permit and the original identity paper to every check post
With copies, which posts may keep.
What About the Police Verification?
- The state tourism corporation asks for a verification form signed and stamped by the traveller's local police station.
- The station checks its records for the name, father's name and address given.
- It writes the certificate in the name on the form you hand in. So hand in the name on your identity paper.
- Guides say the certificate has to be recent, and give fifteen days. Time it to the journey.
- A slip is corrected at the same station. That is easy from home and nearly impossible from Dharchula.
- Read it at the counter: name, father's name, address and the stamp.
Read name mismatch in police verification and character certificate.
What About the Medical Certificate?
- The journey goes high, and a certificate of fitness is asked for.
- A doctor writes it by hand, often on a printed form, with the name as he hears it.
- Hand him the slip with your name and age as on the identity paper, and ask for capitals.
- See that it carries his name, registration number and stamp, which the permit office looks for.
- A slip is corrected and countersigned by him. A certificate with a scratched-out name and no signature beside it invites a question.
- Guides say a certificate can also be had at the Government hospital at the starting point. That costs a day.
Read hospital discharge summary and medical records name correction, on a doctor's papers generally.
What About the Affidavit?
- For the Adi Kailash journey a notarised affidavit is asked for, as an indemnity bond.
- It is typed from a format, with the traveller's name, father's name, age and address filled in.
- A typist at a court compound fills these from what he is told.
- Give him the slip, read the draft, and only then sign before the notary.
- An affidavit with a wrong name is not corrected by pen. A fresh one is made.
- This affidavit is about the journey's risks. It is not an affidavit about your name, and does nothing to join two names.
Read name change affidavit in Uttarakhand, for what an affidavit can and cannot do.
Why Might a Passport Be Simpler?
- The state tourism corporation says the police verification form is not required where a passport is submitted as the identity paper and the original is brought on the journey.
- A passport is itself issued after a police check, which is the likely reason.
- That removes one certificate, and one hand that might write the name loosely.
- The other papers are then written from the passport, in the passport's form of the name.
- A passport's name is in two boxes. Write both on the certificates, in an order that plainly matches.
- If your passport and Aadhaar differ from each other, choose one and use it throughout.
Confirm with the permit office that this still holds before relying on it. Read name change in passport.
What About Agents, Groups and Check Posts?
- Most travellers go with a tour operator, who collects the papers and applies for the whole group.
- He types the application from what he is sent. Send a photograph of the identity paper, not a message.
- Ask for the permit, or the group list, before the vehicles leave Dharchula, and read your line.
- Check posts on the road compare the permit with the identity paper in your hand.
- Carry the original paper named on the permit, and some photocopies.
- A slip found at the first post means a drive back to the permit office, with the group waiting.
Residents of villages beyond the line, and foreign nationals, are dealt with under different rules. Ask the sub-divisional magistrate's office. Read Corbett safari permit name mismatch, on another permit tied to named people.
Where Does a Gazette Come In?
Nowhere in a journey. It comes in only where a person has changed a name by choice and wants identity papers to carry it. After that, permits are applied for in the new name.
Step 1: Confirm that your own name has in fact changed
A doctor's short form or a constable's spelling is not a change of name.
Step 2: Do not think of a Gazette for a journey
It takes weeks. The certificates for a permit are valid for days.
Step 3: Fix the exact new name
The spelling that will go on Aadhaar and the passport.
Step 4: Make the affidavit and publish the newspaper notice
Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update your identity papers
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Apply for later permits in the name the papers then carry.
Which Gazette Should You Use?
For identity papers, the Gazette of India. A permit needs neither.
Step 1: Remember what a permit office reads
An identity paper, and certificates written from it.
Step 2: Remember who issues the identity paper
A central body, for most people.
Step 3: Prefer the Gazette of India
For your identity papers, not for any permit.
Step 4: Contact the Roorkee press only if a state notice suits another purpose
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 5: Download the notice
Search egazette.gov.in and save the PDF.

Which Mistakes Do Travellers Make?
| Mistake | What to do |
|---|---|
| Saying your name aloud to the doctor and the constable | Hand over a written slip |
| Reading the certificates at Dharchula | Read them at home |
| Two identity papers for different certificates | One for everything |
| A police verification taken too early | Time it to the journey |
| Correcting a certificate by pen yourself | The issuer corrects and signs |
| Giving the father's name as the family says it | As on the identity paper |
| Leaving it all to the tour operator | Read your own line on the permit |
| Carrying only copies to the check post | The original identity paper |
| Thinking the affidavit will explain a different name | It is about the journey, not the name |
A typical case we handle: a retired teacher from Dehradun reached Dharchula with a tour group for Adi Kailash. His Aadhaar read "Jagdish Prasad Nautiyal". His medical certificate read "J P Nautiyal", and his police verification "Jagdish Nautiyal". The permit window asked for papers in one name. His daughter rang us to ask if an affidavit from us could join the three. It could not, and none was needed. He took a fresh medical certificate at the Government hospital there, and the police station at home issued a corrected verification. He left behind his group. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us

That a notarised affidavit and a police verification form from the local police station are asked for the Adi Kailash journey, and that the verification is not required where a passport is submitted and carried, are from the state tourism corporation's terms for its own tours. That the permit office for Chamoli's border areas is at Joshimath, and that an online system was being made ready there, is from a news report. That the Dharchula office issues the permit, what the permit carries, the medical certificate, the fifteen days and the online portal for Pithoragarh are from travel guides. We did not read any official order on the inner line, or the rules for residents and foreign nationals. How certificates come to be written loosely is general knowledge. The sub-divisional magistrate's office is the authority.

Names on an Inner Line Permit: The Short Version
An inner line permit is issued on an identity paper, a police verification, a medical certificate and, for the yatra, an affidavit. The permit carries your name, your father's name and your age. Choose one identity paper and have every other paper written from it. Hand a written slip to the constable, the doctor and the typist. Read all four at home. A passport may spare you the police verification. Carry the original to every check post.
No Gazette is needed for a journey. If your own identity papers disagree, send photos on WhatsApp 9540005064, and we will reply in working hours.
