Medical Fitness Certificate Name Mismatch in Uttarakhand
For people in Uttarakhand who need a medical fitness certificate for a driving licence, a Government job, a permit or work abroad, and find the doctor has written a name that differs from Aadhaar or the passport: why only the doctor can correct it, what each office reads, and how to have it right the first time.
A medical fitness certificate name mismatch is corrected by the doctor or medical board who issued it, by reissuing the certificate. Do not alter it by hand. Give the doctor your identity paper at the start, so the name is copied from it, and read the certificate before you leave. No Gazette is needed.

A medical fitness certificate is a doctor's opinion, written in a few minutes at the end of an examination. It is then read by a transport office, an employer or a recruitment board against your Aadhaar or passport. A medical fitness certificate name mismatch is a name written as spoken, or a father's name left short. This page explains medical fitness certificate name mismatch, and how to avoid it.
We do not issue medical certificates, and this subject needs no Gazette. The driving licence rule is Rule 5 of the Central Motor Vehicles Rules, 1989, and the Government job rule is Fundamental Rule 10. For hospital records, read hospital discharge summary and medical records.
What Kinds of Fitness Certificate Are There?
| Certificate | Issued by | Read by |
|---|---|---|
| Form 1-A for a driving licence | A registered medical practitioner | The transport office |
| Fitness on first appointment | A medical officer or board named by the employer | The appointing office |
| Fitness for work abroad | An approved examination centre | The foreign employer or its agent |
| Fitness for a trek, permit or yatra | A doctor or a camp | The permit office |
| Sick leave or return to duty | The treating doctor | The employer |
All are statements by a doctor about a named person. If the name is not the person's name as his papers give it, the statement may not be taken as being about him.
What Do the Rules Say?
From the Central Motor Vehicles Rules, 1989 and the Fundamental Rules for central Government service. State rules for state jobs differ in detail.
| Point | What the rule says |
|---|---|
| Driving licence | Rule 5 of the Central Motor Vehicles Rules, 1989 requires a medical certificate in Form 1-A for a licence to drive a transport vehicle |
| Over 40 | The same rule requires it for an applicant above 40 years of age |
| Who signs | A registered medical practitioner, as the rule provides |
| Government service | Fundamental Rule 10 requires a new entrant to central Government service to produce a medical certificate of health |
| State service | State rules make similar provision. The appointing office names the examining doctor or board |
Neither rule speaks of names. They do not need to. A certificate is attached to an application or a file in your name, and is read against it.
Where Does the Name Come From?
| Detail on the certificate | Usually comes from | Read against |
|---|---|---|
| Your name | What you say, or the clinic's register | Aadhaar or the passport |
| Father's name | What you say | The same papers |
| Age or date of birth | What you say, or a guess from the examination | Your date of birth on record |
| Identification marks and photograph | The examination | You, in person |
| The purpose | What you ask for | The office receiving it |
The first three rows come from your mouth unless you hand over an identity paper. That is where nearly every slip begins.
What Kinds of Mismatch Are There?
A name written as spoken
- A short form, a home name, a spelling heard wrongly
- The doctor wrote what he heard
- Reissued from your identity paper
A detail left short
- A father's name in part, an age instead of a date
- The office cannot match it to you
- Reissued with the full details
The identity paper is the odd one
- The doctor copied Aadhaar, and Aadhaar is wrong
- Other papers disagree with it
- Aadhaar is corrected, then the certificate
The first is the commonest, and the easiest to prevent. Hand over the card before the examination begins.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| Form 1-A has a short name | The same doctor reissues it | No |
| A job's medical certificate has a spelling slip | The examining doctor or board | No |
| The father's name is wrong | The same | No |
| A certificate for work abroad differs from the passport | The examination centre, before it is sent | No |
| The doctor copied a wrong Aadhaar | Correct Aadhaar, then a fresh certificate | No |
| A disability certificate differs | The medical board | No |
| A married surname is now on your papers | A fresh certificate in that name | No |
| The person changed a name by choice | Gazette, then Aadhaar, then fresh certificates | Yes |
A Gazette appears once in eight rows. The doctor, the board and the Aadhaar centre settle the rest.
How Do You Put It Right?
This is the general method. Most of it is done before the certificate is written.
Step 1: Before the examination, give the doctor your identity paper
Ask that the name be copied from it, not from what you say.
Step 2: Read the certificate before you leave the clinic
Name, father's name, age or date of birth, and the purpose.
Step 3: Compare it with the paper the receiving office will read
Aadhaar for most offices. The passport for work abroad.
Step 4: If it is wrong, ask the same doctor or hospital to reissue it
A certificate is the doctor's statement. Only the doctor can correct it.
Step 5: Do not alter it yourself
A hand-corrected certificate is likely to be refused, and worse.
Step 6: For an online form, ask the doctor to correct the entry before it is submitted
Some offices take the certificate electronically from the doctor.
Step 7: For a medical board's certificate, write to the board
With your identity paper and the certificate.
Step 8: Keep a copy of the corrected certificate
Offices often ask for it again.
What About a Driving Licence?
- A transport vehicle licence, and any licence for a person above 40, needs a medical certificate in Form 1-A.
- Drivers renewing a commercial licence in the hills need it again and again.
- The transport office reads it with the licence application, which follows Aadhaar.
- So the name on Form 1-A should be the name on the application, as Aadhaar has it.
- Some offices take the certificate electronically from the doctor. Ask the doctor to read the entry to you before submitting.
- A wrong certificate is replaced by the doctor, not corrected at the counter.
Read name change in driving licence and taxi permit name mismatch.
What About a Government Job?
- A new entrant to Government service produces a medical certificate of health before joining.
- The appointing office names the doctor or board, and often sends a form.
- The certificate goes into the service file, beside the appointment letter and the verification report.
- All three should carry the name on the application, as on the class 10 certificate and Aadhaar.
- A slip found at joining is corrected by the board before the papers go into the file.
- A slip found years later is harder. Read it the day you receive it.
Read name change for Government employees, UKPSC and UKSSSC applications and police verification.
What About Work Abroad and Permits?
- Foreign employers and their agents commonly require an examination at an approved centre.
- The certificate is read against the passport, letter by letter, and often by a machine.
- Give the centre your passport, and ask that both name boxes be copied as printed.
- Treks, high-altitude permits and the Kailash Mansarovar Yatra may ask for medical fitness too.
- The permit office reads it against the identity paper on the permit form.
- Read the certificate before you leave the centre. There is often no time to go back.
Read overseas job name mismatch, inner line permit name mismatch and Kailash Mansarovar Yatra application.
What About a Medical Board's Certificate?
- Some certificates are signed by a board of doctors at a Government hospital.
- The board's clerk fills the details from the form you submit.
- A slip there is corrected by the board, through the hospital's office.
- Write with your identity paper and the certificate, and ask for a corrected one.
- The board may want to see you again. That is its right.
- Keep the receipt and any reference number until the new one comes.
Read hospital discharge summary and medical records, on your right to correct medical records.
Why Not Alter the Certificate Yourself?
- A medical certificate is the doctor's statement, over his signature and registration number.
- A name written over or crossed out by anyone else is no longer his statement.
- An office is likely to refuse it, and may treat it as tampered.
- Altering a certificate can amount to an offence, whatever the intention.
- The doctor can reissue a corrected certificate quickly.
- Go back to the doctor. It costs less than the trouble that follows a refusal.
Read medical council registration, on how a doctor's own registration is recorded.
Where Does a Gazette Come In?
Only where a person has changed a name by choice. A doctor's slip is the doctor's to correct.
Step 1: Confirm that your own name has in fact changed
A doctor's slip on a certificate is not a change of name.
Step 2: Remember that a certificate follows your identity paper
Change the identity paper, and later certificates follow.
Step 3: Fix the exact new name
The spelling that will go on Aadhaar and every later paper.
Step 4: Make the affidavit and publish the newspaper notice
Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update Aadhaar
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Get new certificates in the new name.
Which Gazette Should You Use?
A medical certificate is read with Aadhaar or the passport. The Gazette of India is the one to use.
Step 1: Remember who reads a medical certificate
Employers, the transport office, recruitment bodies and foreign employers.
Step 2: Remember the paper it is read with
Aadhaar or the passport, which are central.
Step 3: Prefer the Gazette of India
It serves Aadhaar and the passport.
Step 4: Contact the Roorkee press only if a state notice suits another purpose
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 5: Download the notice
Search egazette.gov.in and save the PDF.
For the difference between the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Do People Make?
| Mistake | What to do |
|---|---|
| Saying your name aloud | Hand over the identity paper |
| Not reading the certificate | Read it before you leave |
| Correcting it by hand | Never. Ask for a reissue |
| An age instead of a date of birth | Ask for the date, if the office wants it |
| A father's name in part | Ask for it in full |
| Using a passport's name for a local job, or the reverse | The paper the office will read |
| Buying a certificate without an examination | Never. It is a false document |
| Leaving a slip for the joining day | Correct it as soon as you see it |
| Asking for a Gazette for a certificate | The doctor reissues it |
A typical case we handle: a taxi driver in Rishikesh went to renew his commercial licence. His Form 1-A read "Bablu", the name the doctor's assistant had written. His licence and Aadhaar read "Rakesh Kumar". The counter returned the application. A friend told him a Gazette would join the names. It would not, and he had changed no name. He went back to the doctor, who reissued the certificate in his Aadhaar name the same afternoon. This is an illustrative example of the type of case we handle, not a named client review.

It is reviewed by Advocate Vipin Chauhan.
That a medical certificate in Form 1-A is required for a transport vehicle licence and for an applicant above 40 is from Rule 5 of the Central Motor Vehicles Rules, 1989. That a new entrant to central Government service produces a medical certificate of health is from Fundamental Rule 10. We did not read the state's own service rules, or any office's practice on electronic certificates, examination centres for work abroad, or permit medicals. That only the issuing doctor or board can correct a certificate, and why not to alter one, are general guidance. The doctor, the medical board and the receiving office are the authority.

Names on a Medical Certificate: The Short Version
A medical fitness certificate is a doctor's statement about a named person, read against your Aadhaar or passport. Hand the doctor your identity paper before the examination, and read the certificate before you leave. A slip is corrected only by the doctor or board who wrote it, by reissuing the certificate. Never alter it by hand. A driving licence, a Government job and work abroad each read it closely.
No Gazette is needed. To find out whether to go back to the doctor, send a photo on WhatsApp 9540005064, and we will reply in working hours.
Only the Doctor Can Correct It. Hand Over Your Card Before the Examination.
