Sports Certificate Name Mismatch in Uttarakhand

For players claiming a sports quota whose merit or eligibility certificate carries a name that differs from the class 10 certificate: who signs which certificate, where its name comes from, how the association's record is put right, and why the date of birth matters even more.

Quick answer

A sports certificate name mismatch is put right only by the body that issued the certificate: the state association, the national federation, the university or the school games authority. Ask it to correct its record on your class 10 certificate, and then to issue the eligibility certificate in the set form. A slip needs no Gazette.

A sports quota certificate is signed from the association record, so ask the issuing body to correct that record on your class 10 certificate first.

A medal at a national meet can be worth a Government post. The post is claimed on paper, though, and the paper was filled by a coach at a registration table years ago. A sports certificate name mismatch is found when a recruiter lays that paper beside a class 10 certificate. This page explains sports certificate name mismatch for players in Uttarakhand.

We do not deal with sports bodies. We prepare the Gazette notification, where a name has in fact been changed. Who may sign a certificate, and in which form, are from the central personnel department's rules for recruiting sportspersons. For another certificate that earns marks, read NCC certificate name correction.

What Are the Two Kinds of Sports Certificate?

The merit or participation certificate

  • Given at or after the event, by its organisers
  • Printed from the team's entry list
  • Often filled by hand, on the day
  • The player's own record of having played

The eligibility certificate

  • Asked for by a recruitment, in a set form
  • Signed by a named officer of the federation, association, university or education directorate
  • Given from the body's own record
  • The paper the quota is claimed on

A player collects the first kind through a career, and needs the second kind once, when applying for a post. The second is written from the same record as the first. A slip in that record therefore appears on both.

Who Signs, and in Which Form?

From the central personnel department's list of authorities competent to award certificates for the recruitment of sportspersons, as its instructions and central recruitment notices set it out.

Authorities competent to award sports certificates for central recruitment and the form for each
CompetitionWho awards the certificateForm
InternationalThe secretary of the national federation of the gameForm 1
NationalThe secretary of the national federation, or the secretary of the state association of the gameForm 2
Inter-universityThe dean of sports, or other officer in overall charge of sports of the universityForm 3
National games for schoolsThe director, or other officer in overall charge of sports for schools, in the state's education directorateForm 4

The forms carry two notes that matter here. One says the certificate is valid only when signed personally by the secretary named. The other says it is given on the basis of the record available in the association's office. Each form names the player as the son, wife or daughter of a named person, with an address.

Where Does the Name Come From?

  1. The player's registration with a club, a school or a district association, often as a child.
  2. The team's entry form for an event, filled by a coach or a manager.
  3. The organisers' list of participants and results, typed from the entry forms.
  4. The merit certificate, written from that list.
  5. The association's or federation's record of the event.
  6. The eligibility certificate, signed years later from that record.

The player does not write the name at any of the first four steps. A coach writes what he calls the child. That is how "Bittu Rawat" wins a state medal and "Vivek Singh Rawat" applies for a post.

What Kinds of Mismatch Are There?

A slip in the name

A wrong or missing parent's name

  • The merit certificate gave only the player's name
  • The eligibility form needs a parent's or spouse's name
  • Supplied from class 10, when the form is filled

A different date of birth

  • The entry form carried a date other than the class 10 one
  • In age-group events, the date decides who may play
  • The most serious of the three. See the section on it

A fourth situation is a name that has in fact been changed since the event. That is not a slip. It is shown by a Gazette, or a marriage certificate, kept beside the certificates.

How Do You Have It Put Right?

This is the general method. Each association has its own office and ways.

Step 1: Lay out every sports certificate you mean to claim on

With your class 10 certificate. Read your name, your father's name and the date of birth on each.

Step 2: Note who issued each one

A state association, a national federation, a university or the school games authority.

Step 3: Read the recruitment notice for the form it wants

A central recruitment asks for a certificate in a set form, signed by a named officer.

Step 4: Write to the body that issued the certificate with the slip

State the entry as printed and as it should read, with your class 10 certificate and the event's details.

Step 5: Ask it to correct its record, and then the certificate

The eligibility certificate is given from that record.

Step 6: Ask your coach, school or district association to support the request

They entered the team, and can confirm who played.

Step 7: Bring Aadhaar to the same name

Players are now registered on it in most sports.

Step 8: Get the eligibility certificate in the set form only after the record is right

Signed personally by the officer the form names.

What Does "On the Basis of Record Available" Mean for You?

  • The set forms say the certificate is given on the basis of the record available in the association's office.
  • So the secretary does not certify what your old merit certificate says. The secretary certifies what the association's record says.
  • Where that record carries a nickname, the eligibility certificate will carry it too, however carefully you fill the request.
  • The request, then, is two requests: correct the record, and issue the certificate from it.
  • An association is a private body run by office-bearers who change. Its records are often thin. Bring everything that helps: the merit certificate, the team photograph, the result sheet, a newspaper cutting, your coach's letter.
  • Ask for the decision in writing, on the association's letterhead. A recruiter may ask how a name came to be changed.

Do this well before a notification. A secretary who must sign personally cannot be hurried.

Why Does the Date of Birth Matter More?

  • Much junior sport is played in age groups. A date of birth decides whether a child may enter at all.
  • A date on an entry form that differs from the class 10 certificate is therefore not read as a typing slip. It can be read as playing in the wrong age group.
  • Often it is innocent. A coach guessed, or copied the date from a school register that the board later corrected.
  • A recruiter cannot tell the innocent case from the other. It sees two dates.
  • Raise it yourself with the association, with the class 10 certificate and the birth certificate, and ask for the record to be corrected and the reason noted.
  • Never alter a certificate, and never present one you know to be wrong. A sports quota claim on a false paper costs the post, and can bring a criminal case.

Read date of birth correction and name correction in the birth certificate.

What About School and University Games?

  • School games. A state school team plays in the national games for schools. The central form for it is signed by the officer in overall charge of sports for schools in the state's education directorate.
  • A school player's name comes from the school's own list, sent up through the block and the district. The school's register is the proof for any correction.
  • University games. A player represents the university in inter-university tournaments. The form is signed by the dean of sports or the officer in charge of sports.
  • A university player's name comes from the college's team list. It should match the university's enrolment.
  • A correction for either goes to that office, with the class 10 certificate and the school's or college's letter.

Read Kumaun University degree name correction and name change in education documents.

What About the State's Quota?

  • In January 2024 the state cabinet approved bringing a law to give a four per cent horizontal reservation in Government jobs to sportspersons of the state, as reported.
  • The report says it covers those who have taken part in or won medals at national and international events.
  • We did not read the law or the rules made under it, and cannot say which certificates, forms or signing officers they name.
  • Each recruitment notice of the state's commissions will say what proof of a sporting record it accepts.
  • A horizontal reservation is claimed inside the candidate's own category. So the sports certificate is read alongside the caste and residence certificates. All should carry one name.

Read UKPSC and UKSSSC application name mismatch and name change in residence certificate.

How Does a Recruiter Verify?

  • A sports quota is a small number of posts for which few are eligible. Recruiters check each claim closely.
  • They read the eligibility certificate against the application, the class 10 certificate and the merit certificates.
  • They may write to the federation or association to confirm the event, the team and the player. The answer comes from its record.
  • A trial of playing ability is often held as well. That shows the candidate can play. It does not show the candidate is the person named.
  • A certificate signed by someone other than the officer the form names is not accepted, whatever it says.
  • Where you know of a difference, say so in the application and attach the association's letter.

Read name mismatch at document verification.

How Does the Central Gazette Route Work?

Use this where your name has in fact been changed by choice, so that your certificates are right for a name you no longer use. It does nothing for a slip, which the issuing body puts right.

Step 1: Confirm that your name has in fact changed

A slip against the class 10 certificate is put right by the body that issued the certificate.

Step 2: Ask the association how it treats a changed name

A certificate records who played on a day. A later change is usually shown by a Gazette kept beside it.

Step 3: Fix the exact new name, and copy the old name from the class 10 certificate

Letter by letter.

Step 4: Make the affidavit and publish the newspaper notice

Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.

Step 5: Type the form and sign the undertaking

Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.

Step 6: Prepare the CD, photographs and certificate

The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.

Step 7: Pay the charge on BharatKosh

Commonly Rs 1,100 for an adult. It is not refunded.

Step 8: Send the file yourself, then carry the Gazette with your certificates

Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Show the Gazette wherever the certificates are read.

See gazette name change in Uttarakhand.

Which Gazette Should You Use?

A player may apply to central and state recruiters alike. The Gazette of India serves both.

Step 1: Remember who reads the certificate

Central departments, the forces, railways and state recruiting bodies.

Step 2: Prefer the Gazette of India

It is the one every central reader recognises.

Step 3: Contact the Roorkee press only if a state notice suits you

Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.

Step 4: Ask for the papers, the format and the fee, and submit

Keep photocopies of everything.

Step 5: Download the notice

Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Players Make?

Common mistakes with a name on a sports certificate
MistakeWhat to do
Letting a coach enter a nicknameGive your name in writing, as on school records
Never reading a merit certificateRead it at the venue, and ask then
Asking a district office to correct a state body's paperGo to whoever issued it
Getting the set form signed by the wrong officerThe form names who must sign, personally
Correcting the paper and not the recordThe record is what gets verified
Hiding a different date of birthRaise it yourself, with class 10
Waiting for a notificationStart months before
Getting a Gazette for a nicknameThe association corrects its record
Altering a certificate by handNever. It can end a career, and worse

A typical case we handle: a boxer from Pithoragarh won a medal at a national championship as a junior. His merit certificate read "Sonu Chand". His class 10 certificate read "Sandeep Singh Chand". When a central department advertised sports quota posts, his father asked us for a Gazette saying the two were one. A Gazette is for a changed name, and his had never changed. He wrote to the state association with his certificate, his coach's letter and the result sheet. It corrected its record and its secretary signed the certificate in the set form. This is an illustrative example of the type of case we handle, not a named client review.

When you do not need us

Who may award a certificate for each level of competition, the four forms, that a certificate is valid only when signed personally by the secretary and that it is given on the basis of the association's record are from the central personnel department's instructions on recruiting sportspersons and from central recruitment notices that reproduce them. That the state cabinet approved bringing a law for a four per cent reservation is from a newspaper report of January 2024. We did not read that law or its rules. How an association corrects its record, and what a recruiter does to verify, are general guidance. The body that issued your certificate, and the recruitment notice, are the authority.

Names on Sports Certificates: The Short Version

A sports quota is claimed on a certificate in a set form, signed personally by the officer the form names, from the association's own record. That record began with a coach's entry form. Read every certificate against your class 10 certificate. Where the name, a parent's name or the date of birth differs, ask the body that issued it to correct its record, with your proofs, and then to sign the certificate. Start long before a notification.
