MCP Card Name Mismatch in Uttarakhand: Mother and Child Protection Card
For mothers in Uttarakhand whose Mother and Child Protection card, the jachcha-bachcha card, carries a name that differs from Aadhaar, the bank account or later the birth certificate: what the card records, why the maternity schemes read it, who corrects it, and how to keep the child's name the same from the start.
An MCP card name mismatch is corrected by the ANM or Anganwadi worker who filled the card, in the card and in her register. Give her the mother's Aadhaar at registration, read the family details that day, and later write the child's name as it will appear on the birth certificate. No Gazette is needed.

Almost every pregnancy in Uttarakhand begins a small card, filled by the ANM or the Anganwadi worker and kept by the mother for years. The maternity schemes read it, the vaccinations are marked on it, and a school may ask for it. An MCP card name mismatch is a name on it that differs from Aadhaar or the birth certificate. This page explains MCP card name mismatch.
We do not fill or correct cards, and this subject needs no Gazette. What the card records is from the card itself, as published by the two ministries. Its use for the maternity benefit is from that scheme's published answers. For the benefit itself, read PM Matru Vandana Yojana name mismatch.
What Is the Card, in Brief?
- What it is. A record and a guide for a pregnant woman, a new mother and her child up to early childhood.
- Whose it is. Used jointly by the Union health ministry and the women and child development ministry.
- Who fills it. The ANM, the ASHA and the Anganwadi worker.
- Who keeps it. The mother. The ANM keeps a counterfoil and a register.
- What it records. The pregnancy, the delivery, the child's vaccinations and growth, and the family's details.
The card is filled by hand, often in a crowded room at a health sub-centre. Names are written as they are said. That is where most slips begin.
What Is Published?
From the revised card published by the two ministries in 2018, the guide for health workers, and the women and child development ministry's answers on the maternity benefit.
| Point | What is published |
|---|---|
| Family details | Mother's name, father's name, age, address, mobile numbers, the RCH ID, bank and branch, account number and code |
| Identity numbers | The mother's Aadhaar number and, later, the child's |
| The child | The child's name, date of birth, birth weight, birth registration number and RCH ID |
| The counterfoil | The immunisation counterfoil is retained by the ANM |
| The maternity benefit | The ministry's answers say the card is mandatory for the benefit's instalments, and the date of the last period is taken from it |
The card holds names, numbers and a bank account in one place. A slip in any of them reaches the schemes that read it.
Which Names Does the Card Carry?
| Name on the card | Read against | Where it matters |
|---|---|---|
| The mother | Her Aadhaar and bank account | The maternity benefit payments |
| The father | His identity paper, and later the birth certificate | The child's records |
| The child | The birth certificate and the child's Aadhaar | Vaccination records, Anganwadi, school |
| The bank account holder | The passbook | Where money is paid |
The child's row is the one that lasts. A child named one way on the card and another on the birth certificate starts life with two records.
What Kinds of Mismatch Are There?
The mother's name as said
- A home name, a married name she has not yet taken on paper
- Read against Aadhaar and the bank
- Corrected by the worker, from Aadhaar
The child's name
- "Baby of", a pet name, or a name later changed
- Read against the birth certificate
- Written as it will be registered
The father's name
- A short form, or no surname
- Differs from the birth certificate
- Corrected to his identity paper
The first holds up money now. The second causes trouble for years. Both are corrected by the same worker, with a few words.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The mother's name differs from Aadhaar | The ANM or Anganwadi worker | No |
| Aadhaar is the wrong paper | Correct Aadhaar | No |
| The maternity benefit is held up | The benefit or JSY, after the card agrees | No |
| The child's name is "baby of" | Write it once decided, as on the birth certificate | No |
| The birth certificate differs from the card | The Registrar, or the worker | No |
| The mother's name differs on the birth certificate | The Registrar | No |
| The child's Aadhaar differs | Child Aadhaar correction | No |
| The mother changed her name by choice | Gazette, then Aadhaar and the bank. Tell the worker | Yes |
A Gazette appears once in eight rows. The worker, the Registrar and the Aadhaar centre settle the rest.
How Do You Put It Right?
This is the general method. Most of it takes a minute at the next visit.
Step 1: At registration, give the ANM or Anganwadi worker the mother's Aadhaar
So her name is copied from it, not from what is said.
Step 2: Read the family details on the card that day
Mother's name, father's name, address, mobile, bank account.
Step 3: Compare the mother's name with her Aadhaar and her bank passbook
The maternity schemes pay to that account.
Step 4: Ask the worker to correct a slip in the card and the register at once
The card and the worker's register should match.
Step 5: Where Aadhaar is the odd one, correct Aadhaar
Then the card and the schemes follow it.
Step 6: After birth, write the child's name on the card as it will be registered
And the birth registration number, when you have it.
Step 7: Make the birth certificate carry the same names
The child's, the mother's and the father's.
Step 8: Keep the card safe for years
It records the child's vaccinations, and is asked for at school and Anganwadi.
What About the Mother's Name?
- A young bride is often registered in her husband's family's name for her, while her Aadhaar still has her maiden name.
- The maternity benefit is checked against Aadhaar and paid to her bank account.
- So the card, Aadhaar and the account should carry one name.
- Hand the worker your Aadhaar at registration, and ask her to copy the name from it.
- If you want to use your married name, change Aadhaar and the bank first, with the marriage certificate.
- The father's name should be as on his own identity paper.
Read name change after marriage and mother's name correction.
What About the Child's Name?
- At birth the child often has no name. The card may say "baby of" the mother.
- Families choose a name weeks later, sometimes at a ceremony.
- Write the name on the card the same as it will be entered in the birth register.
- A child's name can be added to the birth registration later, within the time the law allows.
- Then the child's Aadhaar, made from the birth certificate, agrees too.
- A pet name belongs at home, not on any of these papers.
Read name change in birth certificate, child Aadhaar name correction and home name and school name.
What About the Maternity Schemes?
- The ministry's answers on the maternity benefit say the card is mandatory for its instalments.
- The benefit reads the mother's name and the date of the last period from the card.
- It pays to her bank account, checked against Aadhaar.
- Janani Suraksha Yojana payments for an institutional delivery also go to the mother's account.
- Where the card, Aadhaar and the account differ, a payment can stall.
- Correct the card and the account first, then ask the worker to resubmit.
Read PM Matru Vandana Yojana name mismatch and Janani Suraksha Yojana name mismatch.
What Happens Years Later?
- The card records the child's vaccinations, and is asked for at Anganwadi and school admission.
- A school may read the child's name on it against the birth certificate.
- A card with "baby of" or a pet name may raise a question.
- Keep the card with the birth certificate, in a plastic cover.
- Where the card was never corrected, the birth certificate is the better proof of the child's name.
- A vaccination certificate from the portal may carry the name too. Keep it the same.
Read vaccination certificate name mismatch and RTE school admission name mismatch.
What If the Card Is Lost?
- Tell the ANM or Anganwadi worker at once.
- The ANM keeps a register and the immunisation counterfoil.
- Ask for a fresh card, filled from the register.
- Read the names on the new card as carefully as on the first.
- Keep the RCH ID if you have it. It links the mother and child in the health records.
- Photograph each page of the card now, in case it is lost later.
Where Does a Gazette Come In?
Only where a mother has changed her name by choice. A slip on the card is the worker's to correct, and no care or benefit should wait for a Gazette.
Step 1: Confirm that a name has in fact changed
A slip on the card is not a change of name.
Step 2: Do not hold up care or a benefit for it
A Gazette takes weeks. Pregnancy and vaccination do not wait.
Step 3: Fix the exact new name
The spelling that will go on Aadhaar and every later paper.
Step 4: Make the affidavit and publish the newspaper notice
Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update Aadhaar and the bank
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. Tell the ANM, and keep the notice with the card.
Which Gazette Should You Use?
The card is read with Aadhaar and the bank, which are central. The Gazette of India is the one to use.
Step 1: Remember whose card this is
A joint card of the Union health and women and child development ministries, used through the state.
Step 2: Remember what it is read with
Aadhaar, the bank account and the birth certificate.
Step 3: Prefer the Gazette of India
It serves Aadhaar and the banks.
Step 4: Contact the Roorkee press only if a state notice suits another purpose
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 5: Download the notice
Search egazette.gov.in and save the PDF.
For the difference between the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Do Families Make?
| Mistake | What to do |
|---|---|
| Saying the mother's name aloud | Show her Aadhaar |
| Not reading the card that day | Read the family details before leaving |
| A married name the papers do not carry | The name on Aadhaar, or change Aadhaar first |
| A bank account in another form of the name | Bring it to the Aadhaar name |
| Leaving the child as "baby of" | Write the name once decided |
| A pet name on the card | The name for the birth certificate |
| Correcting the card, not the register | Ask the worker to correct both |
| Losing the card | Photograph it, and keep it safe |
| Asking for a Gazette for a card | The worker corrects it |
A typical case we handle: a young mother in Bageshwar was registered as "Bhawna Joshi", her married name. Her Aadhaar and bank account still read "Bhawna Pant". The maternity benefit stalled. Her husband asked us about a Gazette for her new surname. None was needed for the benefit. The Anganwadi worker corrected the card to her Aadhaar name, and the claim went through. She changed her surname on Aadhaar later, with her marriage certificate. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
For any slip on the card. The ANM, the Anganwadi worker and the Registrar settle it. We are useful only where a mother has changed her name by choice and a Gazette notification is asked for.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The card is corrected at your health sub-centre or Anganwadi. Send photos on WhatsApp, and we will tell you which name differs.

What is U-WIN?
The Union health ministry's digital platform for the Universal Immunisation Programme. Health workers record each vaccination of a pregnant woman and child on it, along with the delivery outcome and the newborn's registration, and the child's schedule is tracked until 16.
How many vaccine doses does a child get under the programme?
Each child receives 27 vaccine doses from birth to 16 years against 12 diseases, and each pregnant woman two doses of the Td vaccine. Each dose marked on the MCP card is now also recorded digitally.
Can a child have an ABHA health ID?
Yes. U-WIN lets parents create a Child ABHA ID for children up to six, based on a parent's or guardian's Aadhaar. Use the parent's Aadhaar name exactly, and the child's name as it will be on the birth certificate.
Can I download my child's vaccination certificate?
U-WIN gives a QR-based, digitally verifiable e-vaccination certificate that can be downloaded and kept in DigiLocker. It carries the child's name as registered, so check it against the MCP card.
How many mothers and children are on U-WIN?
UNDP reported that U-WIN had registered over 17 million pregnant women and 54 million children, and tracked more than 264 million vaccine doses in real time.
Can my child be vaccinated in another state?
Yes. Because U-WIN records are national, a child can be vaccinated anywhere in the country, and the dose is added to the same record.

The fields on the card are from the revised Mother and Child Protection card published by the Union health and women and child development ministries in 2018. That health workers fill it, and that the ANM retains the immunisation counterfoil, are from published guidance for health workers and descriptions of the card. That the card is mandatory for the maternity benefit's instalments, and that the date of the last period is taken from it, are from the women and child development ministry's published answers on that scheme. We did not read the state's own instructions to health or Anganwadi workers. How names come to differ, and the order of correction, are general guidance. The ANM, the Anganwadi worker and the Registrar are the authority.

Names on the MCP Card: The Short Version
The Mother and Child Protection card carries the mother's, the father's and the child's names, with Aadhaar numbers and a bank account. The maternity benefit reads it and pays to the account. Give the worker the mother's Aadhaar at registration and read the card that day. Write the child's name as it will be registered at birth.
