Orphan Name in Documents in Uttarakhand: When Parents Are Unknown or Have Died
For people who grew up without parents, in a children's home or with relatives, and for those who care for such children: which name and date of birth to settle, what goes in the column for parents, which certificate stands in for a birth certificate, how the state's orphan certificate fits in, and how an adult takes a surname.
For an orphan name in documents, settle one name and one date of birth early and use them on every paper. A certificate from the children's home or the district child protection office stands in for a birth certificate at Aadhaar and the passport office. An adult who wishes to add a surname does so through the Gazette.

Most forms assume a father, a mother and a birth certificate. A person who grew up in a children's home, or lost both parents young, meets that assumption at every counter. The matter of an orphan name in documents is not one mismatch but three questions: which name, which date of birth, and what to write for parents. This page takes each in turn, for people in Uttarakhand.
What the passport office and the identity authority accept is from their own published pages. The state's orphan certificate is from a High Court order and a description of the state's procedure. We did not read the rules themselves. For a child with one parent, read a single mother’s child and the father’s name.
What Are the Three Questions?
- Which name. The one a home gave, the one a school wrote, or one the person chooses. Often a single name, with no surname.
- Which date of birth. Known from a birth record, or estimated by a doctor or the home when the child arrived.
- What for parents. Known and dead, known to one office only, or not known at all.
Each office has an answer to these, and they are not always the same answer. The work is to make the person's own answers the same everywhere.
What Do the Offices Say?
From the passport office's page on special cases of minors, the identity authority's list of acceptable documents, a 2021 order of the Uttarakhand High Court, and a description of the state's orphan certificate procedure.
| Office | What it says |
|---|---|
| Passport: date of birth | An orphan or abandoned child who has stayed in a child care home and has no proof of date of birth may submit a declaration by the head of the home, on its letterhead, confirming the date |
| Passport: parents | The application permits the name of father, or mother, or legal guardian. Only one, not both |
| Aadhaar | A certificate for enrolment of children in child care institutions, issued by the district child protection officer, serves as proof of identity and address |
| Aadhaar: small children | Enrolment through a head of family is required for children under five, except for children in child care institutions |
| The state: reservation | Five per cent horizontal reservation for orphans in Government services, on an orphan certificate |
| The state: who qualifies | The High Court's order records that rules of 2021 cover those whose mother and father both died within twenty-one years of the child's birth |
Two central offices, then, have a named paper for exactly this situation. Neither asks for a birth certificate that cannot exist.
Where Do the First Papers Come From?
| Paper | Issued by | What it fixes |
|---|---|---|
| The placement order | The child welfare committee or a court | That the child was placed in the home, and when |
| The home's record and letter | The children's home | The name used and the date of birth recorded |
| The school record | The school | The name every later certificate copies |
| The child protection office's certificate | The district child protection officer | Identity and address, for Aadhaar |
| Parents' death certificates | The Registrar, where the parents are known | That both have died, for the orphan certificate |
| Aadhaar | The identity authority | The name and date of birth other offices read |
The school record deserves the most care. A name and a date of birth written there at admission reach the class 10 certificate, and from it every form for the rest of a life.
What Kinds of Situation Are There?
Parents known, and both have died
- A birth certificate usually exists, with their names
- The child may live with relatives
- The task is to keep their names the same on every paper, and prove both deaths
Parents not known
- A child found, or left at a home
- No birth certificate, and no names for the parents' column
- The home's and the district office's papers take their place
An adult, long out of a home
- Papers made at different times, in slightly different names
- The home may have closed
- The papers are brought to one name, and a surname may be added by choice
The first is a matter of matching names, as on other pages of this site. The second and third are about building a record where the usual foundation is missing.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| No birth certificate, and a passport is needed | The home's declaration of date of birth | No |
| No identity paper for Aadhaar | The district child protection officer's certificate | No |
| The home's name and the school's name differ | Settle one. Correct the other record | No |
| A form insists on a father's name | A guardian's, or as that office directs | No |
| Parents' names differ across their death certificates and the child's papers | Correct the certificate with the slip | No |
| The child lives with a grandparent or relative | The guardian’s papers | No |
| The child has been adopted | The adoption order | As that page explains |
| An adult wishes to add a surname or take a new name | Gazette, then Aadhaar first | Yes |
A Gazette appears plainly once in eight rows: where an adult makes a choice about a name. Everything before that is about first papers, which no Gazette supplies.
How Are the Papers Put in Order?
This is the general method. A home or a guardian does it for a child. An adult does it for himself or herself.
Step 1: Gather every paper the person has
From the home, the school, the court or committee that placed the child, and any identity paper.
Step 2: Write down the name, date of birth and parents' column on each
Side by side. The differences are usually few, and plain once listed.
Step 3: Settle which name and date of birth will be the person's own
Usually the ones on the school record, which later papers follow.
Step 4: Ask the home, or the district child protection office, for its certificate
It supports identity and date of birth where no birth certificate exists.
Step 5: Take Aadhaar on that certificate, or correct it
The identity authority has a certificate for children in child care institutions.
Step 6: Fill the parents' column as each form allows
A guardian's name, one parent's, or as the office directs. Never an invented name.
Step 7: Apply for the orphan certificate, where the person qualifies
In the same name and date of birth as Aadhaar.
Step 8: Keep the home's letter and the placement order for life
They are the person's earliest papers, and stand in for a birth certificate again and again.
What Goes in the Column for Parents?
- Where the parents are known and have died, their names are written, as on the birth certificate. A dead parent is still a parent.
- Where they are not known, the truthful answer is that they are not known.
- The passport office's form permits a legal guardian's name alone.
- Other forms differ. Some accept a guardian. Some allow the box to be left blank or marked. Ask each office before filling it.
- Never invent a father's name to get past a form. It will be copied onto paper after paper, and cannot be explained later.
- Whatever is written, write the same thing everywhere. An entry that changes from form to form is what offices question.
We could not confirm what each state office accepts in this column. Read a single mother’s child and the father’s name and father’s name change and correction.
What About the Date of Birth?
- A child who arrives at a home without papers is usually given a date of birth, on an estimate of age.
- The home records it, and the school copies it at admission.
- The passport office accepts the head of the home's declaration of that date, where no other proof exists.
- That date then becomes the person's date of birth for every purpose: examinations, jobs, retirement.
- It is very hard to change later, since a correction needs better proof and none exists.
- So see that one date, and only one, is written from the start. Two papers with two estimates are a lasting difficulty.
Read date of birth correction and lost or no documents to prove your name.
What Is the Orphan Certificate?
- Uttarakhand gives orphans a five per cent horizontal reservation in Government services.
- It is claimed on an orphan certificate, issued under the state's procedure.
- The High Court's 2021 order records that the rules of that year cover a person whose mother and father both died within twenty-one years of his or her birth.
- Children living in homes in the state are covered as well.
- The certificate names the person. That name and date of birth should be exactly as on Aadhaar and the class 10 certificate, since a recruiting body will compare all three.
- Where it rests on the parents' deaths, their names on the death certificates should match the names on the child's own papers.
We did not read the rules, and do not know the present form or office. Ask the district's women and child welfare office or the tehsil. Read UKPSC and UKSSSC application name mismatch.
How Does an Adult Take a Surname?
- Many people raised in homes have a single name on their papers.
- Forms with two boxes, and a passport, then become awkward.
- An adult may add a surname, or take a full new name. The choice is entirely the person's own.
- Since papers already exist in the single name, this is a change of name, published in the Gazette.
- Choose with care. A surname suggests a family and a community, and people will read things into it.
- Afterwards Aadhaar is updated first, and the rest in order. The orphan certificate is taken afresh in the new name.
This is the one part of this subject where our work comes in. Read adding a surname to a single name and documents to update after a Gazette name change.
What Should a Young Person Take on Leaving a Home?
- The placement order, or a certified copy.
- A letter from the head of the home stating the name, the date of birth recorded, and the years of stay.
- School certificates and the transfer certificate.
- Aadhaar, with a mobile number of the person's own linked to it.
- A bank account in the person's own name.
- The orphan certificate, where the person qualifies.
A home can close, or its staff change. A letter easy to get at eighteen can be impossible at thirty. Ask for two originals. Read school transfer certificate name correction.
What Are the Steps of the Gazette of India Route?
For an adult who decides to add a surname or take a new name. It has no part in getting first papers.
Step 1: Decide, as an adult, the full name you will carry
A surname, or a second name, chosen by you. Nobody else's permission is needed.
Step 2: Check that it is a change, and not a first entry
Where your papers have one name and you are adding to it, that is a change of name.
Step 3: Fix the exact spelling and order
As it will stand on Aadhaar, PAN and a passport.
Step 4: Make the affidavit and publish the newspaper notice
Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, then update Aadhaar first
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054.
Which Gazette Should You Use?
A young person's papers will be read by employers and offices anywhere. The Gazette of India is the one to prefer.
Step 1: List the offices that will read the notice
Aadhaar, PAN, a passport office, an employer.
Step 2: Prefer the Gazette of India
A young person's papers will travel beyond the state.
Step 3: Remember the state's own certificates
The orphan certificate and the residence certificate are applied for afresh in the new name.
Step 4: Contact the Roorkee press only if a state notice suits you
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.
To weigh the two, read Uttarakhand Gazette vs Gazette of India.
Which Mistakes Are Made?
| Mistake | What to do |
|---|---|
| Inventing a father's name for a form | A guardian's name, or as the office directs |
| Two dates of birth on two papers | One date, from the start |
| A different name at school from the home's record | Settle one before the board examination |
| Leaving the home without its letter | Take it, in two originals |
| Thinking a passport is impossible without a birth certificate | The home's declaration serves |
| The orphan certificate in a different spelling | As on Aadhaar, letter for letter |
| Adding a surname on one paper only | Through the Gazette, then every paper |
| Paying someone for a "birth certificate" | A false certificate ruins every paper after it |
| Asking for a Gazette to get first papers | A Gazette changes a name. It creates no record |
A typical case we handle: a young woman who had grown up in a children's home in Dehradun was applying for a job. Her papers carried the single name "Pooja". The online form would not go forward without a surname, and a friend told her to type any. She asked us first. We told her not to type a name she did not hold. She chose a surname herself, and it was published in the Gazette. Her Aadhaar was updated on it, and she took her orphan certificate afresh in the full name. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
For first papers, Aadhaar, a passport, the parents' column and the orphan certificate. The home and the offices named settle those. We are useful only where an adult decides to add a surname or change a name, and a Gazette notification is needed.

That a declaration by the head of a child care home serves for date of birth, and that a passport application permits one parent's or a legal guardian's name, are from the passport office's page on special cases of minors. The certificate for Aadhaar enrolment of children in child care institutions is from the identity authority's list of acceptable documents. The five per cent horizontal reservation, the orphan certificate and the extension to those whose parents died within twenty-one years of birth are from a 2021 order of the Uttarakhand High Court and a description of the state's procedure. We did not read the state's rules, and do not know the present form or issuing office. We could not confirm what state offices accept in the parents' column, or how caste and residence certificates are dealt with. The rest is general guidance. Each office is the authority for its own paper.

An Orphan's Documents: The Short Version
Settle one name and one date of birth early, and use them on every paper. A children's home's declaration serves the passport office for date of birth, and the district child protection officer's certificate serves Aadhaar. In the parents' column write a guardian's name or what the office directs, never an invented name. Uttarakhand has an orphan certificate for reservation in Government services. An adult may add a surname through the Gazette, and then updates Aadhaar first.
