PMJJBY and PMSBY Claim Name Mismatch in Uttarakhand
For families claiming under the Jeevan Jyoti life cover or the Suraksha accident cover, where the dead member's or the nominee's name on the bank's record differs from the death certificate or identity papers: which names are compared, who corrects what, and how the claim is filed.
A PMJJBY and PMSBY claim name mismatch is settled at the bank branch that took the premium. Have the Registrar correct the death certificate if it is wrong. Give the paper that links the nominee's recorded name to the nominee's Aadhaar, such as a marriage certificate. Then file the claim form. No Gazette is needed for a spelling.

These two covers cost very little and are taken with a tick on a form, often when a Jan Dhan account is opened. Families forget them until a death. A PMJJBY and PMSBY claim name mismatch is then found at the worst time, by people who have never filed a claim. This page explains PMJJBY and PMSBY claim name mismatch for families in Uttarakhand.
We do not file claims or deal with banks, and this subject seldom needs a Gazette. Where a claim is made, what proof is taken and who claims for a minor or without a nominee are from the schemes' published claims procedures and claim form, as banks reproduce them. For a bank deposit left by the same person, read nominee name mismatch in a bank claim.
What Are the Two Covers?
Jeevan Jyoti (PMJJBY)
- A life cover, for death from any cause
- For one year at a time, renewed by a yearly debit
- Taken through a savings account at a bank or a post office
- Paid to the nominee on the member's death
Suraksha (PMSBY)
- An accident cover, for death or disablement by accident
- For one year at a time, renewed by a yearly debit
- Taken through the same kind of account
- Paid to the nominee on death, or to the member on disablement
Neither has a policy document in the usual sense. The member's identity is the bank account. The nominee is whoever was written on a one-page form at the branch. So every name in a claim comes from the bank's books.
What Does the Claims Procedure Say?
From the schemes' claims procedures and claim form, as banks publish them. We left out the amounts and the premium, which change.
| Point | What the procedure says |
|---|---|
| Where | The claim form is given to the bank or post office branch concerned |
| When | Preferably within 30 days of the death, or of the accident |
| Proof of death | A death certificate from the Registrar of births and deaths, a hospital's discharge summary or certificate for the dead person, or a certificate from the District Magistrate or an officer authorised |
| The claimant's account | The first two pages of the passbook, a statement or a cancelled cheque of the nominee, appointee or claimant |
| The branch's check | The branch verifies the particulars in the form from the enrolment data and its own records |
| A minor nominee | The appointee named in the enrolment form claims |
| No nominee | A legal heir claims, with a succession certificate or a legal heir certificate from the competent court or authority |
| Payment | Remitted to the bank or post office account of the nominee or claimant |
The fifth row is the one that matters here. The branch does not take the family's word for who was insured and who was nominated. It reads the claim against what it wrote down years earlier.
Which Names Are on a Claim?
| Name | Comes from | Read against |
|---|---|---|
| The insured member | The bank or post office account the premium was debited from | The death certificate, and for an accident the police and medical papers |
| The nominee | The enrolment form, as the member wrote it | The nominee's Aadhaar and other identity papers |
| The nominee's bank account | The passbook or cheque attached | The nominee's name on the claim form |
| The appointee, for a minor | The enrolment form | The appointee's own papers |
| A legal heir, where there is no nominee | The heirship certificate | The heir's own papers |
Many of these accounts were opened in camps, by a bank's agent with a hand-held machine, from whatever card the customer carried. The enrolment form for the cover was filled in the same minute. A wife written there as "Guddi" is still "Guddi" in the bank's record eight years later.
What Kinds of Mismatch Are There?
The member's name
- The death certificate and the bank account spell the dead person differently
- Or one has a surname and the other has not
- Cured by correcting the death certificate, or by linking papers
The nominee's name
- A home name, a maiden surname or a bare first name on the enrolment form
- The nominee's Aadhaar reads differently
- Explained by a marriage certificate or the family register. It cannot be corrected now
The papers of an accident
- A police report or a post-mortem report names the person as bystanders gave it
- Only in a claim under the accident cover
- Linked to the account by the death certificate and identity papers
The first can still be put right, since the death certificate is a living record that the Registrar can correct. The second cannot. The member who wrote the nomination is the one person who could have changed it.
Which Case Is Yours?
| The situation | Route | Gazette? |
|---|---|---|
| The death certificate has a spelling slip | The Registrar corrects it, then the claim is filed | No |
| The bank account is in initials or a short form | The member's Aadhaar and other papers, given with the claim | No |
| The nominee is recorded by a home name | The marriage certificate or family register, with the nominee's Aadhaar | No |
| The nominee is a married daughter, recorded in her maiden name | Her marriage certificate | No |
| The nominee's own bank account is in another form of the name | The nominee's bank corrects it on Aadhaar | No |
| The police or post-mortem report names the person differently | Linking papers, with the accident claim | No |
| There is no nominee, or the nominee has died | A legal heir claims, with an heirship or succession certificate | No |
| The nominee changed a name by choice | The Gazette, shown with the claim | Yes |
A Gazette appears once in eight rows, and only for a living nominee. Nothing can be gazetted for the member who has died.
How Do You File the Claim?
This is the general method. Use the claim form the branch gives you.
Step 1: Find out which branch the cover was taken through
The premium is debited once a year from one account. That branch takes the claim.
Step 2: Ask the branch for the names on its record
The insured member, as the account holds it, and the nominee, as the enrolment form gave it.
Step 3: Get the death certificate, and read it against the account
The name, the father's or husband's name and the date of birth.
Step 4: Have a wrong death certificate corrected by the Registrar
Before it is filed. A claim sent with a paper that does not match comes back.
Step 5: Read the nominee's name against the nominee's own papers
Aadhaar and the passbook of the account the money is to go into.
Step 6: Collect the paper that explains any difference
A marriage certificate, the family register, or the nominee's other identity papers.
Step 7: Fill the claim form in the names as the bank holds them
And attach the linking paper. Do not write a new form of either name on the form.
Step 8: Hand it to the branch, and take an acknowledgement
With the date. The branch verifies it and sends it on to the insurer.
What About the Death Certificate?
- The procedure lists a death certificate from the Registrar of births and deaths first among the proofs of death.
- It is written from what the family or the hospital reported. In a hurry, a relative gives the name as it was spoken at home.
- The branch reads it against the account: the name, the father's or husband's name, and often the age.
- A certificate that is wrong is corrected by the Registrar who issued it, on the dead person's own identity papers.
- Ask for the correction at once. The same certificate is needed for the bank balance, the pension, the land and everything else.
- Where a hospital's discharge summary or certificate is used in its place, the same reading applies.
Read name correction in a death certificate and legal heir certificate, which sets out how a death entry is corrected.
What About the Nominee's Name?
- The nominee's name is whatever the member wrote, or said to the agent, on the day of enrolment.
- Men in the hills commonly name a wife by the name the household uses. It may appear on no paper of hers.
- The branch compares that entry with the nominee's Aadhaar and the passbook she attaches.
- A difference in the surname is answered by the marriage certificate. A difference in the first name needs more: the family register, a ration card, the member's own records naming his wife.
- An affidavit by the nominee that both names are hers may be asked for. It supports the other papers. It does not replace them.
- The nominee's own bank account has to be in her Aadhaar name, or the payment will be returned.
Read Kumari, Devi and the husband’s surname, name correction in marriage registration certificate and name correction in the family register.
What About an Accident Claim?
- A claim under the accident cover has to show that the death or disablement was by accident.
- The procedure lists the first information report, the post-mortem report or a certificate, among the papers taken for that.
- A police report is written at the scene, from what people there say. A road accident far from home may record a nickname, or only a first name and a village.
- The post-mortem report copies the name from the police papers.
- The branch and the insurer then have three or four papers, each with its own form of the name, to set beside the account.
- For disablement the member is alive and claims in person, with medical papers and a disability certificate in the member's own name.
- The accident scheme's procedure has the insurer check whether another insurer has also paid. One person, one cover.

What If the Nominee Is a Minor, or There Is None?
- A minor nominee. The enrolment form has a place for an appointee, who claims for the child. The procedure names the appointee as the claimant in that case.
- No appointee named. Ask the branch. A guardian may have to show the right to act for the child.
- No nominee at all. The procedure provides for a legal heir, with a succession certificate or a legal heir certificate from the competent court or authority.
- A nominee who died first. The position is the same as having none.
- Several heirs. The certificate names them. The branch will want the others' consent before paying one.
- The names. Each heir's name on the certificate against that heir's own papers, and the dead member's name on it against the account.

What Can Be Done While the Member Is Alive?
- Ask the branch if you are enrolled, through which account, and who is recorded as nominee.
- See that the account itself is in your Aadhaar name. The cover takes its name from the account.
- Ask for the nomination to be changed to the nominee's name as on the nominee's Aadhaar. Guides say a fresh nomination is given at the same branch.
- Name an appointee where the nominee is a child.
- Keep one account for these covers. Guides say a person may join through one account only.
- Tell the family that the cover exists, and at which branch. A claim nobody knows of is never made.

Where Does a Gazette Come In?
Only where a living nominee changed a name by choice, so that the enrolment form and the nominee's identity papers carry two different names. It has no part in a spelling, a home name, a married surname or the dead member's name.
Step 1: Confirm that it is a change of name, and not a spelling or a married surname
Those are settled on identity papers and a marriage certificate.
Step 2: Ask the branch what it needs where the nominee's name was changed
Many ask for a Gazette for a change of name by choice.
Step 3: Check whether a Gazette already exists
A nominee who changed a name years ago may hold one. A second is not needed.
Step 4: Make the affidavit and publish the newspaper notice
Sign before a notary. Publish in a daily local leading newspaper, and keep the original newspaper.
Step 5: Type the form and sign the undertaking
Two typed copies of the specimen, signed in your old name with two witnesses, and the undertaking.
Step 6: Prepare the CD, photographs and certificate
The same text on a CD in MS Word, two self-attested photographs with identity proof, and a certificate that both copies match.
Step 7: Pay the charge on BharatKosh
Commonly Rs 1,100 for an adult. It is not refunded.
Step 8: Send the file yourself, and do not hold the claim back for it
Send it to the Controller of Publications, Department of Publication, Civil Lines, Delhi 110054. File the claim meanwhile, and add the Gazette when it comes.
Would the Uttarakhand Gazette Do?
These are central schemes, run through banks. The Gazette of India is the one to prefer.
Step 1: Ask the branch which Gazette it accepts
The insurer follows the bank's record and central identity papers.
Step 2: Think about Aadhaar and your own bank account
The Gazette of India serves them all.
Step 3: Contact the Roorkee press only if a state notice suits you
Take the contact from govtpress.uk.gov.in and ask if a private change of name notice is being accepted.
Step 4: Ask for the papers, the format and the fee, and submit
Keep photocopies of everything.
Step 5: Download the notice
Search gazettes.uk.gov.in and save the PDF.

Which Mistakes Do Families Make?
| Mistake | What to do |
|---|---|
| Not knowing the cover existed | Look for a small yearly debit in the passbook |
| Taking the death certificate without reading it | Read it against the bank account |
| Filing the claim with a paper that does not match | Correct or link it first |
| Writing the nominee's Aadhaar name over the recorded one | Use the recorded name, and attach the link |
| Giving a relative's account for the payment | The nominee's own account |
| Waiting months to file | File early, and say why if late |
| Asking for a Gazette in the dead member's name | There is no such thing. Use linking papers |
| Paying for a Gazette to explain a home name | The marriage certificate and family register do it |
| Paying an agent a share of the claim | The branch takes the claim without a fee |
A typical case we handle: a driver from Tehri died in a road accident near Rishikesh. His bank account read "Makan Singh". The police report read "Makkhan, son of Gabbar Singh", and the enrolment form named his wife "Guddi Devi". Her Aadhaar read "Godambari Devi". The branch returned the claim, and her brother asked us for a Gazette. Nobody had changed a name. The Registrar issued the death certificate in the account's spelling on his Aadhaar. The family register showed Godambari Devi as his wife, and the village head certified that she was known as Guddi. The claim was filed again with those. This is an illustrative example of the type of case we handle, not a named client review.
When you do not need us
For a spelling, a home name, a married surname and an heir's claim. The Registrar and the branch settle those on papers. We are useful only where a living nominee changed a name by choice and a Gazette notification is asked for.
Which Areas of Uttarakhand Do We Cover?
All 13 districts: Dehradun, Haridwar, Pauri Garhwal, Tehri Garhwal, Rudraprayag, Chamoli, Uttarkashi, Nainital, Udham Singh Nagar, Almora, Bageshwar, Champawat and Pithoragarh. The claim is made at the branch that took the premium. For the papers behind it, send photos on WhatsApp and we will tell you which one links the names.

Where a claim is made, the 30 days, the proofs of death, the papers for the claimant's account, the branch's check against enrolment data, the appointee for a minor and the legal heir where there is no nominee are from the schemes' revised claims procedures and claim form, read in copies that banks publish. That the accident scheme's insurer checks for another payment is from the same procedure. That a person joins through one account only, and that a fresh nomination is given at the branch, are from guides. We did not read the schemes' rules on the Government's own portal, and left out the amounts, the premium and the time limits for settlement. How a branch reads a home name is general practice. The branch that took the premium is the authority.

Names in a PMJJBY or PMSBY Claim: The Short Version
Both covers hang on a bank or post office account, and the claim is made at that branch. The branch reads the dead member's name on the death certificate against the account, and the nominee's name on the enrolment form against the nominee's Aadhaar. Have a wrong death certificate corrected by the Registrar. Explain a nominee's home name or maiden name with a marriage certificate or the family register. An accident claim adds police and medical papers, read the same way.
A spelling needs no Gazette. To find out which paper links the names, send photos on WhatsApp 9540005064, and we will reply in working hours.
